Are "normal hips" being labeled as femoroacetabular impingement due to EE angle?

Gluteal muscle contracture (GMC) is a clinical syndrome characterized by gait abnormality and limb dysfunction, as well as secondary deformities of pelvis and femur. Femoroacetabular impingement (FAI) typically could be diagnosed on the basis of computed tomography (CT) such as the equatorial-edge angle (EE angle), but it did not work well in GMC patients. In this study, we retrospected all image data and found small EE angles in GMCs, which meant retroverted acetabulum; however, none of them showed no symptoms and signs of FAI. Therefore, we had reasons to think that, some normal hips with un

Weber osteotomy for large Hill-Sachs Defects: clinical and CT assessments.

BACKGROUND: The Weber derotation osteotomy is an uncommon procedure that typically is reserved for patients with engaging Hill-Sachs defects who have had other surgical treatments for shoulder instability fail. It is unknown whether the desired humeral derotation actually is achieved with the Weber osteotomy. QUESTIONS/PURPOSES: The purposes of this study were to answer the following questions: (1) What are the complication (including redislocation) and reoperation rates of the Weber osteotomy? (2) What are the American Shoulder and Elbow Surgeons (ASES) and functional (ROM in internal rotati

How Common Is Femoral Retroversion and How Is it Affected by Different Measurement Methods in Unilateral Slipped Capital Femoral Epiphysis?

BACKGROUND: Although femoral retroversion has been linked to the onset of slipped capital femoral epiphysis (SCFE), and may result from a rotation of the femoral epiphysis around the epiphyseal tubercle leading to femoral retroversion, femoral version has rarely been described in patients with SCFE. Furthermore, the prevalence of actual femoral retroversion and the effect of different measurement methods on femoral version angles has yet to be studied in SCFE. QUESTIONS/PURPOSES: (1) Do femoral version and the prevalence of femoral retroversion differ between hips with SCFE and the asymptomat

Early clinical and radiographic outcomes of an augmented baseplate in reverse shoulder arthroplasty for glenohumeral arthritis with glenoid deformity.

BACKGROUND: Glenoid deformity is commonly encountered in patients undergoing reverse shoulder arthroplasty (RSA). Augmented baseplates can correct glenoid deformity while potentially avoiding certain complications encountered with structural bone graft. Limited evidence exists to support the use of metallic augmented baseplates in RSA. METHODS: We performed a retrospective review to identify all patients treated with an augmented baseplate during primary RSA with a minimum of 1 year of clinical and radiographic follow-up. Preoperative radiographs and advanced imaging were used to determine gl

Three-dimensional characterization of glenoid defects in failed shoulder arthroplasties.

BACKGROUND: Glenoid vault defects are often observed in revision shoulder arthroplasty, and improper management of glenoid bone loss during revision to reverse total shoulder arthroplasty (rTSA) increases the risk of complications. However, there is limited understanding of the types of glenoid vault defects that occur in patients. Therefore, the purpose of this study was to three-dimensionally characterize the underlying glenoid morphology of patients with failed shoulder arthroplasties subsequently undergoing revision arthroplasty to rTSA. METHODS: Computed tomography scans of 100 patients,

Clinical and computed tomography-radiologic outcome after bony glenoid augmentation in recurrent anterior shoulder instability without significant glenoid bone loss.

BACKGROUND: The presence of a significant bony defect in anterior shoulder instability cases warrants glenoid reconstruction surgery typically by means of an autograft. Some surgeons use the same graft techniques even in the absence of a significant bony defect, thus augmenting the glenoid surface. The goal of this study is to investigate the clinical and computed tomography-radiologic outcome after glenoid augmentation surgery. METHODS: Between 2006 and 2011, 11 patients with recurrent anterior shoulder instability and glenoid bone loss of 5% or less were treated with an iliac crest autograf

Computed Tomography (CT)-Based Morphometric Analysis of the Glenoid Fossa and Its Clinical Significance in the Lucknow Region, Uttar Pradesh.

Background and objective The glenoid fossa is the lateral extension of the scapula's lateral angle, with a concave surface that articulates with the head of the humerus. Glenoid bone loss is frequently associated with glenohumeral instability. A thorough understanding of shoulder joint anatomy is essential, and the glenoid cavity index (GCI) provides a valuable measure for assessing the configuration and dimensions of the glenoid cavity. Restoring its normal anatomy improves joint function and reduces pain. Conventional two-dimensional (2D) and three-dimensional (3D) CT imaging remain the stan

Tumour necrosis factor-α promotes BMHSC differentiation by increasing P2X7 receptor in oestrogen-deficient osteoporosis.

The exact mechanism of tumour necrosis factor α (TNF-α) promoting osteoclast differentiation is not completely clear. A variety of P2 purine receptor subtypes have been confirmed to be widely involved in bone metabolism. Thus, the purpose of this study was to explore whether P2 receptor is involved in the differentiation of osteoclasts. Mouse bone marrow haematopoietic stem cells (BMHSCs) were co-cultured with TNF-α to explore the effect of TNF-α on osteoclast differentiation and bone resorption capacity in vitro, and changes in the P2 receptor were detected at the same time. The P2 receptor w

Periacetabular Osteotomy Provides Higher Survivorship Than Rim Trimming for Acetabular Retroversion.

BACKGROUND: Acetabular retroversion can cause impaction-type femoroacetabular impingement leading to hip pain and osteoarthritis. It can be treated by anteverting periacetabular osteotomy (PAO) or acetabular rim trimming with refixation of the labrum. There is increasing evidence that acetabular retroversion is a rotational abnormality of the entire hemipelvis and not a focal overgrowth of the anterior acetabular wall, which favors an anteverting PAO. However, it is unknown if this larger procedure would be beneficial in terms of survivorship and Merle d'Aubigné scores in a midterm followup co

Coracoid abnormalities and their relationship with glenohumeral deformities in children with obstetric brachial plexus injury.

BACKGROUND: Patients with incomplete recovery from obstetric brachial plexus injury (OBPI) usually develop secondary muscle imbalances and bone deformities at the shoulder joint. Considerable efforts have been made to characterize and correct the glenohumeral deformities, and relatively less emphasis has been placed on the more subtle ones, such as those of the coracoid process. The purpose of this retrospective study is to determine the relationship between coracoid abnormalities and glenohumeral deformities in OBPI patients. We hypothesize that coracoscapular angles and distances, as well as

Population-specific statistical shape modeling of the Japanese shoulder: a combined analysis of the scapula and humerus stratified by sex and stature.

BACKGROUND: The smaller shoulder morphology typical of East Asian, especially Japanese, populations results in challenges when using implants designed from North American and European anatomies. Previous Japanese morphologic studies have focused on average shapes without adequately capturing population variability. We aimed to develop a combined statistical shape model (SSM) of the scapula and humerus reflecting covariation, and to quantify subgroup differences by sex and stature with corresponding point clouds. METHODS: Pre-operative computed tomography scans from 114 Japanese arthroplasty p

Five-year outcomes of anatomic total shoulder arthroplasty with Aequalis Perform+ posterior augmented glenoid implants in Walch B2 and B3 glenoids.

BACKGROUND: Posterior glenoid wear and persistent static posterior subluxation of the humeral head (SPSH) are associated with early failure of anatomic total shoulder arthroplasty (aTSA). In patients with Walch B2 or B3 glenoid types, posterior augmented glenoid (PAG) implants have been reported to successfully correct SPSH on short-term follow-up. However, the medium-term and long-term clinical and radiological outcomes of PAG remain unclear. METHODS: We prospectively followed 10 consecutive patients (mean age, 67.9 ± 8.9 years; 5 females) with primary glenohumeral osteoarthritis and B2 or B

Nonoperative Treatment of Isolated Posterior Glenohumeral Instability in an Active Military Population: Effect of Glenoid and Acromial Morphology.

BACKGROUND: Nonoperative management of posterior shoulder instability is common. However, limited data are available to assess the pathomorphologic factors associated with its failure. PURPOSE/HYPOTHESIS: The purpose of this study was 2-fold: (1) to determine glenohumeral pathomorphologic features predictive of nonoperative management failure of posterior instability; and (2) to determine the relationship between nonoperative management failure and posterior glenoid bone loss (pGBL) progression. It was hypothesized that greater posterior acromial height (PAH) would adversely affect nonoperati

Will my Child Walk Funny? The Rotational Profile of Infants and Children With Classic Bladder Exstrophy.

BACKGROUND: Bladder exstrophy is a rare, complex disorder that involves the urinary, reproductive, gastrointestinal, and musculoskeletal systems and is marked by an incomplete lower abdominal wall, leaving the bladder exposed to the outside world. Skeletal findings in this condition include externally rotated innominate bones. While the resulting acetabular retroversion might be expected to cause out-toeing, there is no published data on the gait parameters or rotational profile of children with classic bladder exstrophy (CBE) in isolation. This study, therefore, characterizes the rotational p

Tornier Perform Anatomic Augmented Glenoid Implants correct posterior subluxation and maintain active joint reduction in Walch B2 glenoids.

BACKGROUND: The Walch type B2 glenoid is characterized by retroversion, posteroinferior erosion and posterior humeral head subluxation. A surgical management option for a B2 glenoid is an anatomic total shoulder arthroplasty (aTSA) with a posteriorly augmented all-posterior augmented glenoid (PAG) implant. However, little is known about what occurs actively at the glenohumeral articulation after aTSA with a PAG. Additionally, most studies on B2 glenoids, whether pre-aTSA or post-aTSA, assess subluxation statically. Presently, it is unknown if subluxation recurs after aTSA with a PAG or if it v

Radiographically Apparent Acetabular Sourcil Landmarks Are Created by Comparable Regions of the Pelvis With Extraarticular Bone Variably Confounding Estimates of Joint Coverage.

BACKGROUND: The severity of hip dysplasia is characterized by radiographic measurements that require user definition of the acetabular sourcil edge, a bony landmark for which the corresponding three-dimensional (3D) anatomy is not well defined in any imaging plane. QUESTIONS/PURPOSES: To use digitally reconstructed radiographs to determine: (1) What 3D anatomy is contributing to the "acetabular sourcil" location used to make lateral center-edge angle (LCEA) and anterior center-edge angle (ACEA) measurements in standing AP and false-profile radiographic views, respectively? Clinical and radiographic outcomes of cementless reverse total shoulder arthroplasty for proximal humeral fractures.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) has demonstrated successful outcomes in the treatment of both acute and chronic proximal humeral fractures (PHFs). The traditional RTSA surgical technique uses a methyl methacrylate cemented humeral component to restore and maintain both humeral height and retroversion. However, use of humeral bone cement has been associated intraoperatively with cardiopulmonary risk, increased operative cost, and postoperatively with difficulty if revision arthroplasty is required. We report the clinical and radiographic outcomes of a completely cementles

Outcomes of anatomic shoulder arthroplasty performed on B2 vs. A1 type glenoids.

BACKGROUND: Glenoid component malpositioning and glenoid component retroversion have been associated with higher rates of radiolucencies, raising concerns about its implications on glenoid loosening and ultimate failure of anatomic total shoulder arthroplasty (TSA). Although there is literature regarding the relative advantages of techniques to address posterior glenoid bone loss, we are not aware of studies comparing outcomes of TSA on these challenging Walch type B2 glenoids vs. more common A1 glenoids. The purpose of this study is to compare outcomes of TSA performed on A1 glenoids and B2 g

Minimally invasive lateral approach for adult degenerative scoliosis: lessons learned.

OBJECT: Lateral minimally invasive thoracolumbar instrumentation techniques are playing an increasing role in the treatment of adult degenerative scoliosis. However, there is a paucity of data in determining the ideal candidate for a lateral versus a traditional approach, and versus a hybrid construct. The objective of this study is to present a method for utilizing the lateral minimally invasive surgery (MIS) approach for adult spinal deformity, provide clinical outcomes to validate our experience, and determine the limitations of lateral MIS for adult degenerative scoliosis correction.
