PURPOSE: Open or fenestrated interference screw design that allow bone ingrowth is a concept for improved bone healing to softtissue graft and bone filling in bone tunnels after anterior cruciate ligament reconstruction (ACLR) The aim of the current study was to assess CT scanning evaluated bone ingrowth into an open architecture interference screws in the tibial tunnel of patients undergoing ACL with soft tissue grafts. It was hypothesized that open architecture interference screws would stimulate bone ingrowth into the screw cavities.
METHODS: Twelve patients requiring ACLR were included. T

BACKGROUND: Treatment for osteonecrosis of the femoral head (ONFH) in young individuals remains controversial. We developed a lantern-shaped screw, which was designed to provide mechanical support for the femoral head to prevent its collapse, for the treatment of ONFH. The purpose of this study was to investigate the efficacy and safety of the lantern-shaped screw loaded with autologous bone for the treatment of pre-collapse stages of ONFH.
METHODS: Thirty-two patients were randomly divided into two groups: the lantern-shaped screw group (core decompression and lantern-shaped screw loaded wit

A case report presents the experience of application of compression pileateless mini-screws (Gerbert's screws) in treatment of intra-articular fractures, which formed the elbow joint (44 cases). There were performed 32 operations concerning fracture of head of radius, 10 operations on the occasion of fractures of distal section of the humerus and 2 operations on the coronoid process. Long-term treatment results were followed-up in 31 patients during more than 6 months. On basis of analysis of treatment results the authors made a conclusion that the application of mini-screws in case of bone fr

We have devised a new technique to improve stabilization of fractured facial bone fractures (frontal sinus fractures, zygomatic fractures, mandibular condyle fractures) by intermaxillary fixation screw traction wires (stainless steel wires through intermaxillary fixation screws). A retrospective study evaluating intermaxillary fixation screw traction wires was performed. We have used this technique for 3 cases of frontal sinus fractures, 9 cases of zygomatic fractures, and 7 cases of mandibular condyle fractures. After dissection of a fractured site, a hole is drilled on the fractured bone whe

BACKGROUND: The most controversial problem in treating scaphoid fractures is whether bone grafts are necessary for cystic-type fractures.
METHODS: We treated 105 scaphoid fractures using Herbert screws (1988-1997), AO 3.0 mm cannulated screws (1998-2002), and Acutrak screws (2003-2006). The patients ranged in age from 14 years to 67 years (average, 26 years). Our classifications were based on the radiographic findings: linear type (51 cases); cystic type (24 cases); and sclerotic or displaced type (30 cases). Linear and cystic types did not have any displacement more than 2 mm. If the fractur

Anterior or posterolateral spondylodesis has been reported and used widely as a surgical treatment for lumbar spondylolysis or spondylolisthesis. Although spinal fusion is necessary when there is extensive vertebral slippage or spinal instability, the direct repair of the defect is thought to be anatomical, logical, and less invasive as a surgical treatment for symptomatic lumbar spondylolysis or a minimal degree of spondylolisthesis. This operation, with a few modifications, has been performed since 1985 in our clinic. The results, using Henderson's criteria, were excellent in 64.5% and good

Two groups of patients with diseased lumbar spines treated by the Steffee variable screw placement system were studied, in order to compare concomitant posterolateral fusion and posterior lumbar interbody fusion. The use of posterolateral fusion resulted in less invasiveness than posterior lumbar interbody fusion, with regard to operating time and blood loss. With the posterolateral fusion method, correction of olisthesis or kyphotic deformity, if attained at all, was difficult to maintain. Posterior lumbar interbody fusion produced better correction and maintenance, even in osteoporotic cases

Various fixation techniques, including screws and suture-based methods, are used for posterior cruciate ligament (PCL) avulsion fractures. This study compared the biomechanical effects of different suture configurations in simulated PCL avulsion fractures. Twenty cadaveric knees were assigned to four fixation groups: antegrade screw (A), 1 suture tape with 2 tunnels (B), 2 suture tapes with 2 tunnels (C), and 2 suture tapes with 3 tunnels (D). Biomechanical testing included cyclic loading and load-to-failure tests, with elongation, stiffness, yield load, and failure modes recorded. No signific

Background: Distal first metatarsal osteotomies are commonly performed operative procedures for hallux valgus deformity, and usually involve fixation with a metal screw. However, various bioabsorbable osteosynthesis materials have been in use for a number of years. One recent innovation is the Shark Screw®, a human cortical bone allograft. This study aimed to evaluate the efficacy and safety of this allogeneic screw in the stabilization of Reversed L-Shaped osteotomy, a modified Chevron osteotomy. Methods: In a prospective study, 15 patients underwent a Reversed L-Shaped osteotomy stabilized w

An ideal fixation system for guided bone (GBR) regeneration in oral surgery must fulfil several criteria that includes the provision of adequate mechanical fixation, complete resorption when no longer needed, complete replacement by bone, as well as be biocompatible and have a good clinical manageability. For the first time, a biodegradable magnesium fixation screw made of the magnesium alloy WZM211 with a MgF2 coating has been designed and tested to fulfill these criteria. Adequate mechanical fixation was shown for the magnesium fixation screw in several benchtop tests that directly compared

Although osteoporosis has negative impacts on lumbar fusion, its effects on screw loosening in dynamic stabilization remain elusive. We aimed to correlate bone mineral density (BMD) with screw loosening in Dynesys dynamic stabilization (DDS). Consecutive patients who underwent 2- or 3-level DDS for spondylosis, recurrent disc herniations, or low-grade spondylolisthesis at L3-5 were retrospectively reviewed. BMD was assessed by the Hounsfield Unit (HU) in vertebral bodies (VB) and pedicles with and without cortical bone (CB) on pre-operative computed tomography (CT). Screw loosening was assesse

The purpose of this study is to examine the effects of screw preload values on the bone-plate system. The preload value was taken differently in the literature range from 50 N to 3000 N. These preload value were examined in this study. The finite element method was used to calculate the strain and stress on the models. The long bone, plate and screws were modeled as 3D using CAD software. The finite element models were created using Ansys Workbench software. The convergence and validation study were made for the correct results. The 400 N axial load was applied to the proximal end of bone. The

OBJECT Safe and effective insertion of occipital bone screws requires morphological analysis of the occipital bone, which is poorly documented in the literature. The authors of this study present morphological data for determining the area of screw placement for optimal internal fixation. METHODS The subjects of this institutional review board-approved retrospective study were 105 individuals without head and neck disease who underwent CT imaging at the authors' hospital. There were 55 males and 50 females, with a mean age of 57.1 years (range 20-91 years). Measurements using CT were taken acc

A 14-year-old girl presented with idiopathic valgus deformity of her left great toe at the interphalangeal joint (IPJ). The deformity, which had been present but asymptomatic for the past 4 years, began to enlarge and cause discomfort. The measured IPJ angle on anteroposterior standing X-ray was 26°. We treated the toe by medial closing wedge osteotomy and fixation with a double compression headless bone screw (DCHBS). Postoperative hallux valgus interphalangeus (HVI) angle was 14°.

OBJECTIVE: Transpedicular instrumentation of the osteoporotic spine is a challenge for the spine surgeon due to the probability of screw loosening and the potential possibility of nonunion. The purpose of this study was to evaluate the clinical efficacy of bone cement augmented screw fixation in patients with severe osteoporosis.
METHODS: Between February 2004 and August 2007, 250 patients with severe osteoporosis (T-score on BMD < -3.0) that had screw fixation were included in this study. The patients were divided into two groups (Group I: 157 patients that underwent bone cement augmented sc

OBJECTIVE: To investigate direct bone graft repair with lag screw and tension band fixation technique and its value for the treatment of adolescence spondylolysis.
METHODS: Lysis was prepared and the fibrous tissue within the gap was removed in 12 patients from 12 to 26 years (average 18.4). 1 - 2 mm extra bony element was resected on both sides with fresh interface with the bone graft harvested from the iliac crest. From the entry point 8 mm away from the midline at the inferior margin of the lamina, titanium lag screws of 35 - 45 mm long and 3.5 mm in diameter were placed at 300 upward and

The results of 20 established nonunions of the scaphoid treated with resection of the pseudoarthrosis, anterior cortic-cancellous iliac bone grafting, and conventional lag screw fixation with the ASIF 2.7 mm cortical screw are presented. Union rate was 95% and the average time off work was 8.9 weeks. Review of the relevant literature uniformly shows that the most common reasons for failure are improper internal fixation techniques and/or the absence of bone grafting. Successful treatment of scaphoid nonunions with screw fixation and cast-free after-treatment does not depend on the implant used

The case describes the revision of an upper ankle prosthesis because of loosening. When ankle replacement is the first choice and actual bone quality does not allow a replacement of the prosthesis, arthrodesis is the only way of reducing pain and gaining stability. The amount of missing bone due to the removed prosthesis was severe. Shark Screws®, made of human allograft cortical bone, were used to fix an allograft femoral head and tibia as well as fibula and talus to each other for stabilization. This was performed without any autologous bone graft and without metal screws. The human matrix o

OBJECTIVE: Spondylolysis is a common cause of low back pain, especially in the young athletic population. It involves a defect, either unilateral or bilateral, of the pars interarticularis. Numerous techniques have been developed for the surgical treatment of spondylolysis, each with its own strengths and weaknesses. This article aims to describe a modified minimally invasive technique using cortical bone trajectory screws with a U-rod to stabilize the lumbar spondylolysis.
METHODS: Adult patients who presented between July 2020 and August 2024 with low back pain due to spondylolysis were inc
