Thalamic aphasia associated with mitochondrial encephalopathy, lactic acidosis, and stroke-like episodes: A case report.

BACKGROUND: Mitochondrial encephalopathy, lactic acidosis, and stroke-like episodes (MELAS) with aphasia is a rare disorder, with the associated aphasia reported as either Wernicke's or Broca's. Herein, we report a patient with MELAS complicated by thalamic aphasia. CASE: A 15-year-old right-handed girl presented with headache, nausea, right homonymous hemianopsia, and aphasia. She could repeat words said by others, but had word-finding difficulty, paraphasia, and dysgraphia. Brain MRI revealed abnormal signals from the left occipital lobe to the temporal lobe and left thalamus, but Wernicke'

Training-induced brain remapping in chronic aphasia: a pilot study.

BACKGROUND: The neural correlates of training-induced improvements of cognitive functions after brain damage remain still scarcely understood. In the specific case of aphasia, although several investigations have addressed the issue of the neural substrates of functional recovery, only a few studies have attempted to assess the impact of language training on the damaged brain. AIMS: The main goal of this study was to examine the neurobiological correlates of improved picture-naming performance in 2 aphasic patients who received intensive and specific training for a chronic and severe phonolog

The Role of Human Parietal Area 7A as a Link between Sequencing in Hand Actions and in Overt Speech Production.

Research on the evolutionary basis of the human language faculty has proposed the mirror neuron system as a link between motor processing and speech development. Consequently, most work has focused on the left inferior frontal cortex, in particular Broca's region, and the left inferior parietal cortex. However, the direct link between planning of hand motor and speech actions has yet to be elucidated. Thus, the present study investigated whether motor sequencing of hand vs. speech actions has a common neural denominator. For the hand motor task, 25 subjects performed single, repeated, or seque

Is Listening in Noise Worth It? The Neurobiology of Speech Recognition in Challenging Listening Conditions.

This review examines findings from functional neuroimaging studies of speech recognition in noise to provide a neural systems level explanation for the effort and fatigue that can be experienced during speech recognition in challenging listening conditions. Neuroimaging studies of speech recognition consistently demonstrate that challenging listening conditions engage neural systems that are used to monitor and optimize performance across a wide range of tasks. These systems appear to improve speech recognition in younger and older adults, but sustained engagement of these systems also appears

Functional near-infrared spectroscopy during motor imagery and motor execution in healthy adults.

OBJECTIVES: Studies on the influence of motor imagery (MI) on brain structure and function are limited to traditional imaging techniques and the mechanism for MI therapy is not clear. By observing the brain activation mode during MI and motor execution (ME) in healthy adults, this study aims to use near-infrared brain imaging technology to provide theoretical basis for the treatment of MI. METHODS: A total of 30 healthy adults recruited to the public from June 2021 to August 2021. The MI and ME of the right knee movement served as the task mode. Block design was repeated 5 times alternately i

Neural Mechanisms of Swallowing Dysfunction and Apraxia of Speech in Acute Stroke.

Speech and swallowing utilize overlapping anatomy and are thus inherently related processes. We sought to identify common neural mechanisms between risk of swallowing dysfunction and apraxia of speech (AOS). This was a retrospective analysis using data from a prospectively collected cohort. Left hemisphere stroke patients (68 subjects) tested with the Apraxia Battery for Adults II, a swallow screen, and MRI were included in the study. Main outcome measure was the presence of AOS or aspiration risk after stroke. We identified a significant association between AOS measures and increased aspirati

Multiple control of fever production in the central nervous system of rabbits.

1. The effects of microinjection of prostaglandin D2, E2 and F2 alpha and of endogenous pyrogen on the rectal temperature of rabbits were extensively examined in sixty-eight brain regions and in the third cerebral ventricle. 2. Intracerebroventricular injection of both prostaglandins E2 and F2 alpha produced dose-dependent fever over a range of 100-1000 ng. The selective brain regions, the nucleus broca ventralis, preoptic area, anterior hypothalamus and the ventromedial hypothalamus, responded to microinjections of a small dose (less than 200 ng) of prostaglandins E2 and F2 alpha by producing

Theta Coherence Asymmetry in the Dorsal Stream of Musicians Facilitates Word Learning.

Word learning constitutes a human faculty which is dependent upon two anatomically distinct processing streams projecting from posterior superior temporal (pST) and inferior parietal (IP) brain regions toward the prefrontal cortex (dorsal stream) and the temporal pole (ventral stream). The ventral stream is involved in mapping sensory and phonological information onto lexical-semantic representations, whereas the dorsal stream contributes to sound-to-motor mapping, articulation, complex sequencing in the verbal domain, and to how verbal information is encoded, stored, and rehearsed from memory

Pathways to lexical ambiguity: fMRI evidence for bilateral fronto-parietal involvement in language processing.

Numerous functional neuroimaging studies reported increased activity in the pars opercularis and the pars triangularis (Brodmann's areas 44 and 45) of the left hemisphere during the performance of linguistic tasks. The role of these areas in the right hemisphere in language processing is not understood and, although there is evidence from lesion studies that the right hemisphere is involved in the appreciation of semantic relations, no specific anatomical substrate has yet been identified. This event-related functional magnetic resonance imaging study compared brain activity during the perform

Altered functional connectivity of the language network in ASD: role of classical language areas and cerebellum.

The development of language, social interaction and communicative skills is remarkably different in the child with autism spectrum disorder (ASD). Atypical brain connectivity has frequently been reported in this patient population. However, the neural correlates underlying their disrupted language development and functioning are still poorly understood. Using resting state fMRI, we investigated the functional connectivity properties of the language network in a group of ASD patients with clear comorbid language impairment (ASD-LI; N = 19) and compared them to the language related connectivity

Distinct frontal regions for processing sentence syntax and story grammar.

Time is a fundamental dimension of cognition. It is expressed in the sequential ordering of individual elements in a wide variety of activities such as language, motor control or in the broader domain of long range goal-directed actions. Several studies have shown the importance of the frontal lobes in sequencing information. The question addressed in this study is whether this brain region hosts a single supramodal sequence processor, or whether separate mechanisms are required for different kinds of temporally organised knowledge structures such as syntax and action knowledge. Here we show t

Cholinergic enzymes in neocortex, hippocampus and basal forebrain of non-neurological and senile dementia of Alzheimer-type patients.

Choline acetyltransferase (ChAT) activity and acetylcholinesterase (AChE) staining were examined in different cortical regions, hippocampal formation and basal forebrain of non-neurological controls and of patients afflicted with senile dementia of Alzheimer type (SDAT). Both enzymes showed a clear topographical distribution in the various regions studied. In SDAT cases, ChAT activity was reduced by 0-60% in the neocortex and by up to 97% in the hippocampus depending on the area and layer examined. In the nucleus of the diagonal band of Broca (NDB) and the medial septal nucleus (MSN), the acti

Progressive anarthria with secondary parkinsonism: a clinico-pathological case report.

The pathological process and lesion topography in patients with the syndrome of progressive aphasia are heterogeneous and few necropsy examination cases have been investigated. This is a case report of a 53 year old right handed man with progressive anarthria and secondary Parkinsonism over a period of six years. Positron emission tomography (PET) showed a decreased cerebral blood flow and metabolism in the frontal cortex, which was more pronounced on the left. Neuropathology disclosed a spongiform vacuolation in layer II of the frontal cortex, mostly in the Broca area, and neuronal loss in th

[Neuropsychological findings of functional cortical mapping in patients with brain tumors].

In 3 patients with brain tumors involving language areas, we investigated functional cortical regions and epileptogenic foci using subdural electrode stimulation before operation. These stimulation studies were useful in preserving postoperatively their language function. The following neuropsychological findings were obtained; (1) Stimulation of Broca area (Brodmann 44 and 45) did not produce anarthria (phonetic disintegration), but semantic and phonemic parahasias and word finding difficulties. (2) Stimulation of lower motor cortex elicited anarthria. (3) Buccofacial apraxia was not produced

Speech production: Wernicke, Broca and beyond.

We investigated the brain systems engaged during propositional speech (PrSp) and two forms of non- propositional speech (NPrSp): counting and reciting overlearned nursery rhymes. Bilateral cerebral and cerebellar regions were involved in the motor act of articulation, irrespective of the type of speech. Three additional, left-lateralized regions, adjacent to the Sylvian sulcus, were activated in common: the most posterior part of the supratemporal plane, the lateral part of the pars opercularis in the posterior inferior frontal gyrus and the anterior insula. Therefore, both NPrSp and PrSp were

Structural Organization of the Motor Speech Area of an Outstanding Writer.

The current study is concerned with the macroscopic and cytoarchitectonic structure of the motor speech area 44 in the left and right hemispheres of the brain of an outstanding writer (age 62 years). In a series of 20-μm frontal total paraffin sections stained with cresyl violet by the Nissl method, the density of total glia, the density of satellite glia, and the density of neurons surrounded by satellite glia were analyzed. The motor speech area of the cortex of the famous writer is characterized by a more complex macroscopic and cytoarchitectonic structure. In Broca's area, the density of t
