Endobronchial hamartoma.

Pulmonary hamartoma is a benign lung neoplasm and it comprises 6% of solitary pulmonary nodules. Two clinical types have been defined according to its location: intraparenchymal (90%) and endobronchial (10%). We report on a case of endobronchial hamartoma resected with electrocautery by a flexible bronchoscope (FB). A 57-year-old male patient was admitted to our clinic because of worsening dyspnea. The patient had been smoking 1 pack per day for 37 years. A solid smooth lesion with calcification, located in the distal part of the left main bronchus and partially obstructing the lumen of bronch

Endoluminal brachytherapy: Bronchus and oesophagus.

Endobronchial brachytherapy could be proposed in case of endoluminal tumours either as a palliative or a curative treatment. As a curative treatment, endobronchial brachytherapy could obtain a high local control rate in case of limited disease. In palliative setting, endobronchial brachytherapy improved thoracic symptoms in more than 80% of cases, but it is less efficient than external beam radiation therapy for palliation. It could be also proposed to maintain the airway open after laser therapy. Oesophageal brachytherapy is a valuable option as a palliative treatment, underused at this time.

Expression of aquaporins in bronchial tissue and lung parenchyma of patients with chronic obstructive pulmonary disease.

BACKGROUND: Aquaporins AQP1 and AQP5 are highly expressed in the lung. Recent studies have shown that the expression of these proteins may be mechanistically involved in the airway inflammation and in the pathogenesis of chronic obstructive pulmonary disease (COPD). The aim of this study was to investigate the expression of AQP1 and AQP5 in the bronchial tissue and the lung parenchyma of patients with COPD and COPD-resistant smokers. METHODS: Using a case-control design, we selected a group of 15 subjects with COPD and 15 resistant smokers (smokers without COPD) as a control, all of whom were

Surgical Strategy and Clinical Outcome in Patients with Bronchial Carcinoids.

Carcinoids are malignant neuroendocrine neoplasms showing good long-term survival after oncologic therapy. The study evaluated the influence of operative strategies and individual decision-making on the outcome and long-term survival in 222 patients with bronchial carcinoids. The patients underwent preoperative pulmonary function tests and bronchoscopy to facilitate surgical decision-making. A hundred and twelve tumors were detected endoscopically, including 32 in the main and lobar bronchi. We performed 5 isolated bronchus resections, 4 segmentectomies, 15 wedge resections, 10 pneumonectomies

Endobronchial Sialolipoma: An Uncommon Salivary Gland Tumor at an Uncommon Site.

BACKGROUND: Sialolipoma is a benign neoplasm composed of neoplastic adipose tissue and normal salivary gland parenchyma. It is commonly seen in the parotid gland. The occurrence of sialolipoma in the main bronchus is exceedingly rare. CASE REPORT: A 52-year-old diabetic and hypertensive gentleman presented with shortness of breath and cough for the last 3-4 months. Computed tomography bronchial angiography showed a soft tissue lesion in the right intermediate bronchus causing its complete obliteration and subsequent collapse of the right lower lobe. A rigid tracheobronchoscope revealed a poly

Descending necrotizing mediastinitis caused by Streptococcus constellatus in an immunocompetent patient: case report and review of the literature.

Descending necrotizing mediastinitis is a severe infection of the mediastinum. This syndrome manifests as fever and chest pain following cough and sputum production. A 49-year-old woman presented with fever and a 14-day history of pneumonia. CT showed mediastinal abscesses with a giant calcified mediastinal lymph node (21 × 18 mm) and pneumonia. Bronchoscopy by EBUS-TBNA under general anesthesia was performed. The pathogen found in the puncture culture was Streptococcus constellatus, and antibiotics (mezlocillin/sulbactam 3.375 IVGTT q8h) was administered. A proximal right main bronchial neopl

Acase of advanced colon cancer arising from a colonic graft foran esophago-bronchial fistula : A case report.

Neoplasm of a colonic graft after esophageal reconstruction is rare. We treated a colon cancer patient who developed malignancy in a colonic graft after esophagectomy and reconstruction through a retrosternal route. A male had undergone esophagectomy in his 50s due to a benign esophago-bronchial fistula. His dysphagia became exacerbated 20 years later, and further examinations showed a circumferential tumor on the esophagocolonic anastomosis. He underwent resection of the colonic graft adenocarcinoma with median sternotomy after neoadjuvant chemotherapy. Gastric tube reconstruction was perform

[The pulmonary cavitary lesion: a clinical case].

The authors describe a case of hydatid cystic disease presenting as a pulmonary excavation lesion. The peculiarity of this case consisted in the absence of the typical clinical symptoms caused by echinococcal cyst rupture into the bronchial cavity, e.g., hydatid vomica or allergic reaction to parasite antigens. The patient's work history and birthplace (he was a shepherd from Sicily), exclusion of the most frequent diseases causing lung excavation, i.e., tuberculosis, abscess, neoplasm, and his response to specific serological tests (indirect hemagglutination and immunoprecipitation) enabled p

Thickening of the posterior wall of the bronchus intermedius. A sign on lateral radiographs of congestive heart failure, lymph node enlargement, and neoplastic infiltration.

The posterior wall of the bronchus intermedius (PWBI) is outlined on most lateral chest radiographs by air in the lumen and in adjacent lung. In a prospective review over a 19-month interval, the authors found 36 patients whose PWBI was thick, defined as thickness greater than the normal upper limit of 3.0 mm (33 patients) or as increase of 1.5 mm in thickness compared with control radiographs (three patients). Uniform thickening was found in 23 patients as a sign of congestive heart failure. Nine subjects with neoplastic thickening of the PWBI demonstrated either uniform (six patients) or lob

Granulomatous mediastinitis due to Aspergillus flavus in a nonimmunosuppressed patient.

A patient with granulomatous mediastinitis due to Aspergillus flavus is described. A 22 year old black man presented with cough, fever and a right hilar mass. Mediastinal biopsies revealed granulomatous fibrosing mediastinitis with fungal elements compatible with aspergillus species. A flavus was isolated on culture of this material and later from sputum and bronchial washings. Studies of the patient's immune status revealed normal humoral and cellular immunity. No underlying neoplasm was found. The patient was treated with amphotericin B and 5-fluorocytosine but esophageal and superior vena c

Radicality and prognosis of surgical treatment of thoracal carcinoid tumors: a review of 152 operated cases.

BACKGROUND: Experience of thoracal (bronchial and thymic) carcinoid tumors is discussed to add some remarkable diagnostic and therapeutic details for their treatment, based on a retrospective clinico-pathological analysis of 152 consecutive patients operated on at the Thoracic Surgery unit in Budapest between 1974 and 1988. METHODS: Prior to surgery 70 patients were symptom free, obstructive symptoms dominated in 65 patients, and hemoptysis occurred in 23 cases. In 68 patients a peripheral coin lesion was visible in radiographs and in 81 cases the tumor could be seen by bronchoscopy. In 3 pat

Endobronchial nocardial infection.

The rare presentation of nocardial infection as an endobronchial "tumour" is reported. Haematogenous dissemination occurred after fibreoptic bronchoscopy and biopsy, a phenomenon not previously described in nocardial infections. This case highlights the difficulties in diagnosing pulmonary nocardial infection and the potential for invasive procedures to disseminate the disease.

Endobronchial Hamartoma Subtotally Occluding the Right Main Bronchus and Mimicking Bronchial Carcinoid Tumor.

Hamartomas are very rarely identified as an endobronchial lesion. Herein, we describe a peculiar case of a 55-year-old woman with persistent cough and increasing dyspnea and radiological detection of a solid lesion subtotally occluding the main right bronchus. Despite the radiological and radiometabolic (18-fluoro-2-deoxy-d-glucose positron emission tomography/computer tomography scan) features were highly suspected for bronchial carcinoid, the definitive diagnosis after endoscopic removal was indicative of an endobronchial hamartoma. When considering differential diagnosis of an endobronchial

[Selective bronchial arteriography in broncho-pulmonary pathology].

104 selective bronchial arteriographies were performed in 25 patients with bronchopulmonary neoplasia, 11 with mediastinal tumours, 15 with bronchopulmonary inflammation, and 53 with haemoptysis. An assessment is made of the diagnostic value of the information obtained. It is felt that this form of examination is particularly useful in detecting the site of bleeding cases of haemoptysis, and in the provision of treatment by means of embolisation.

When and how should patients with multiple endocrine neoplasia type 1 be screened for thymic and bronchial carcinoid tumours?

Patients with multiple endocrine neoplasia type 1 (MEN1) are commonly evaluated for clinical manifestations of this syndrome with the rationale that early diagnosis and adequate treatment will result in improved survival and quality of life. Thymic and bronchial carcinoid tumours are uncommon but important manifestations of MEN1. Current practice guidelines recommend evaluation with computed tomography or magnetic resonance imaging scan of the chest every 1-2 years to detect these neoplasms. However, the certainty that patients will be better off (increased survival or quality of life) as a re

Cavitary mucoepidermoid carcinoma of lung with metastases in skeletal muscles as presenting features: a case report and review of the literature.

Mucoepidermoid carcinomas (MECs) of lung are rare neoplasms originating in bronchial submucosal glands and comprising 0.1-0.2% of primary lung cancers. MECs, the most common malignancy in salivary glands, were earlier thought to occur only in salivary glands. Later studies showed that they can arise as a primary in bronchus, esophagus, lacrimal glands, pancreas, thymus and thyroid gland. Initially described as a benign adenoma, it is now considered to be a malignant epithelial tumor. There have been reports of metastases to regional lymph nodes, other parts of the lung and distant organs. Cavi

Successfully treated lingual tonsillar metastasis from bronchial adenocarcinoma.

Tonsillar metastasis from neoplasms, including lung cancer, are extremely rare, and the prognosis for patients with tonsillar metastases is rather poor. We herein describe a case of long-term survival following radiation for lingual tonsillar metastasis from a bronchial adenocarcinoma. A 39-year-old male was diagnosed with adenocarcinoma of the right lung and was surgically treated. Four months after surgery, a mass arising from the lingual tonsil was noted at the root of the tongue and was pathologically diagnosed as metastasis from lung cancer. In addition, a computed tomography scan reveale
