Foreign body in the throat migrating through the common carotid artery.

We present a 55-year-old lady who swallowed a 3 cm pointed metal foreign body whilst eating a sardine salad. The foreign body migrated from the hypopharynx through the parapharyngeal space and traversed the common carotid artery over a period of 12 days. The foreign body was removed by exploration of the neck.

Common carotid artery bifurcation: evaluation with spiral CT. Work in progress.

To determine the utility of spiral computed tomography (CT) in evaluation of carotid artery stenosis, spiral CT images of 20 patients were compared with images obtained with conventional angiography (20 patients), ultrasound (US) (15 patients), and magnetic resonance (MR) angiography (six patients). The category of stenosis was determined for each internal carotid artery on the basis of the percentage of narrowing: mild = less than 30%, moderate = 30%-69%, and severe = 70%-99%. Occlusions were also noted. The degree of carotid stenosis determined with spiral CT correlated with that determined

Subclavian-to-internal carotid artery bypass with carotid endarterectomy for total occlusion of the common carotid artery under awake neurologic monitoring: a case series.

BACKGROUND: Total occlusion of the common carotid artery (CCA) from its origin is rare, occurring in less than 1% of brachiocephalic stenoses. Surgical revascularization is technically demanding, especially with tandem severe ipsilateral internal carotid artery (ICA) stenosis. Procedures under regional anesthesia with continuous neurologic monitoring are rarely described. OBJECTIVE: To evaluate outcomes of subclavian-to-internal carotid artery bypass with carotid endarterectomy (CEA) in symptomatic patients with total CCA occlusion and severe ipsilateral ICA stenosis under regional anesthesia

Subclavian Artery-Internal Carotid Artery Bypass Using a Radial Artery Graft for Common Carotid Artery Occlusion with Immediate Improvement of Cognitive Function: A Case Report.

INTRODUCTION: Although rare, common carotid artery occlusion (CCAO) causes ischemic neurological dysfunction, which can be treated by revascularization. Although several bypass approaches for CCAO have been suggested, no consensus on the surgical revascularization approach and its functional outcome have been reached. Herein, we present a case of Rile's type 1A CCAO in which a subclavian artery (SclA)-internal carotid artery (ICA) bypass using a radial artery graft (RAG) resulted in immediate recovery of cognitive function and successfully prevented ischemic stroke. CASE DESCRIPTION: A 58-yea

Treatment of a ballistic wound of the common carotid artery revealed by a spontaneous carotid-jugular fistula.

The case reported is of a 30-year-old patient with a left internal carotid-jugular fistula secondary to the explosion of an improvised explosive device during the Afghan war. Carotid resection with arterial bypass using a venous allograft and internal jugular ligation were performed by left cervicotomy associated with sternotomy at a specialized center. The management of cervical arteriovenous fistulas that occur as a result of penetrating trauma faced during the war must be considered and it should be noted that, on battlefields, treatment is not always performed in specialized units.

Retrograde balloon angioplasty of brachiocephalic or common carotid artery stenoses at the time of carotid endarterectomy.

OBJECTIVES: This study was performed to demonstrate the value and durability of intraoperative retrograde angioplasty for stenotic lesions of the aortic arch branches at the time of carotid endarterectomy for the treatment of tandem proximal and bifurcation carotid lesions. DESIGN: Retrospective analysis of the clinical data. METHODS: Forty-four patients were included in this study when they presented with symptomatic extracranial vascular disease due to stenosis of both a proximal aortic arch branch and carotid bifurcation disease. Tandem disease was detected in the vascular laboratory and

Penetrating Neck Injury With Common Carotid Artery Involvement: A Case Report.

Penetrating neck injuries often involve vascular injuries, including the carotid arteries, posing significant morbidity and mortality risks. This report presents a case of a 23-year-old male who sustained a penetrating neck injury from a nail gun, specifically affecting the common carotid artery (CCA). Upon arrival at the emergency department, the patient exhibited hemodynamic stability, with no active bleeding or neurological deficits. Initial imaging, including CT angiography (CTA) of the neck, revealed a metallic foreign body near the CCA and air foci suggestive of a muscle tear. Following

Endovascular Repair of a Common Carotid Artery Perforation during Pacemaker Insertion.

BACKGROUND: We report the percutaneous endovascular management of an iatrogenic perforation of the left common carotid artery (LCCA) during an attempted trans-subclavian pacemaker (PM) placement. METHODS: An 87-year-old woman was urgently transferred after an attempted left subclavian vein PM implantation. Computed tomography angiography scan showed the accidental cannulation of LCCA in its most proximal segment. Owing to the significant surgical risks, the mortality rate, and the distal position of the vessel from the skin, we opted for an endovascular strategy with a balloon-expandable sten

Four collateral circulation pathways were observed after common carotid artery occlusion.

BACKGROUND: Common carotid artery (CCA) occlusion (CCAO) is a rare condition. Owing to collateral circulation, ipsilateral internal carotid artery (ICA) and external carotid artery (ECA) are often patent. METHODS: This study included 16 patients with unilateral CCAO and patent ipsilateral ICA and ECA. The pathways which supplied ICA were investigated by digital subtraction angiography (DSA), transcranial Doppler (TCD), magnetic resonance angiography (MRA) and computed tomography angiography (CTA). RESULTS: In all 16 patients, TCD found antegrade blood flow in ipsilateral ICA, which was suppl

Common Carotid Artery Dissection in Multiple Extracranial Injury: A Case Report.

Traumatic common carotid artery dissection (CCAD) is rare. To our knowledge, only 14 case reports have described traumatic CCAD previously. Here, we report a case of CCAD in a patient with severe trauma. A 50-year-old man was lying on the road after drinking alcohol when a car drove over him. Computed tomography (CT) revealed multiple rib fractures with hemopneumothorax, lung contusion, flail chest, large amount of hematoma with bladder rupture, and fractures on the C6 spinous process, sacral ala, iliac bone, and pubic ramus. Repair of the bladder rupture, exploratory thoracotomy, and open red

In Vivo Gene Transfer to the Rabbit Common Carotid Artery Endothelium.

The goal of this method is to introduce a transgene into the endothelium of isolated segments of both rabbit common carotid arteries. The method achieves focal endothelial-selective transgenesis, thereby allowing an investigator to determine the biological roles of endothelial-expressed transgenes and to quantify the in vivo transcriptional activity of DNA sequences in large artery endothelial cells. The method uses surgical isolation of rabbit common carotid arteries and an arteriotomy to deliver a transgene-expressing viral vector into the arterial lumen. A short incubation period of the vec

Automated, compliant, high-flow common carotid to middle cerebral artery bypass.

The authors describe the use of the Cardica C-Port xA Distal Anastomosis System to perform an automated, high-flow extracranial-intracranial bypass. The C-Port system has been developed and tested in coronary artery bypass surgery for rapid distal coronary artery anastomoses. Air-powered, it performs an automated end-to-side anastomosis within seconds by nearly simultaneously making an arteriotomy and inserting 13 microclips into the graft and recipient vessel. Intracranial use of the device was first simulated in a cadaver prepared for microsurgical anatomical dissection. The authors used thi

Aberrant right common carotid artery: a diagnostic challenge on transesophageal echocardiography.

Aortic arch anomalies are rare, but when present the aberrant vessels may simulate significant vascular abnormality on transesophageal echocardiography (TEE). We report a case of aberrant right common carotid artery. Diagnosis was first suspected on TEE using multiplane and off-axis imaging and subsequently confirmed on magnetic resonance angiography.

[Marked dilatation of the bilateral common carotid artery: a case report].

We reported a rare case of marked dilatation of the bilateral common carotid artery (CCA) associated with stenosis of the left middle cerebral artery (MCA). A 64-year-old female was admitted with right hemiparesis and dysarthria. She was hospitalized 2 years ago for cholecystitis. For 5 years, she has been under medical treatment for hypertension, diabetes mellitus, hyperlipidemia, cardiac failure associated with hypertrophic cardiomyopathy, and atrial fibrillation. Brain CT scan showed infarction of the left corona radiata. Angiography revealed marked dilatation of the bilateral CCA and the i

Common carotid artery occlusion. Role of non-invasive and angiographic analysis.

Doppler and B-scan imaging, periorbital directional Doppler, and occuloplethysmography correctly identified common carotid occlusion in three of four consecutive patients with atherosclerosis. High-grade common carotid stenosis in the fourth patient could not be distinguished from occlusion on the non-invasive tests. Occlusion of the internal carotid artery in one patient and patency of that artery in the other three were evident non-invasively. Modified angiography agreed with non-invasive tests in each of three patients studied. Intra-arterial digital subtraction angiography in the fourth pa
