Spontaneous dissection of bilateral internal carotid and vertebral arteries.

Spontaneous dissection of the major arteries of the neck is known to increase the risk of stroke or transient ischaemic attack in young and middle-aged adults. Most of the reported cases of arterial dissections in the neck involve one or both paired extracranial carotid or vertebral arteries. Spontaneous dissection of the bilateral internal carotid and vertebral arteries is extremely rare. We report a case of spontaneous bilateral internal carotid artery and vertebral artery dissection while using a prescribed pill for weight loss which contained amphetamine derivative. A review of literature

[Clinical characteristics of internal carotid and vertebral arteries dissection].

OBJECTIVE: To compare demographic, clinical, and imaging characteristics of patients with internal carotid artery dissection (ICAD) and vertebral artery dissection (VAD) in a Russian population. MATERIAL AND METHODS: One hundred fifty-two consecutive patients (74 males, 49%; mean age - 37.0±10.3 years) with cervical artery dissection (ICAD - 85 patients, 56%; VA - 62 patients, 41%; ICA+VA - 5 patients - 3%) verified by MRI/MRA were studied. Five patients with both ICAD and VAD were excluded from analysis. RESULTS: Patients with ICAD more often were men (63%, p<0.0001), while patients with VA

Ultrasonographic criteria for diagnosing a dissection of the internal carotid artery.

AIM: To evaluate the usefulness of various sonographic criteria for detecting a dissection of the internal carotid artery (ICA). METHOD: Twenty-three consecutive patients displaying typical clinical symptoms of ICA dissection were identified. The diagnosis of dissection in these cases was confirmed by MRI/MRA or arteriography. The sonographic examination was the first technical investigation and involved the combined use of Doppler and duplex sonography. The value of different sonographic criteria was evaluated for the identification of ICA dissection: 1) Morphological criteria: intramural ha

Stenting of the extracranial internal carotid artery for dissecting aneurysm.

Symptomatic dissecting aneurysms of the extracranial internal carotid artery are not frequent. Medical treatment of these lesions can be ineffective and surgical options present high morbidity. We describe a case successfully treated with endovascular therapy, using a self-expanding stent. The advantages of the endovascular treatment and the rationale of the use of self-expanding stents are discussed.

[Spontaneous dissection of the internal carotid artery: apropos of a case].

Dissections of the extracranial internal carotid artery are probably underestimated in the general population. They are often unrecognized and asymptomatic as shown in this case report and are only the cause of 1 to 4% of cerebral ischaemic accidents. However, they are responsible for 5 to 20% of such accidents in adults younger than 30 years of age. The nontraumatic nature of the carotid artery dissection in our case has been suggested by the medical history although minor trauma could have been missed. A skin biopsy has enabled us to eliminate elastopathies such as Marfan's or Ehlers-Danlos'

Dissection of the internal carotid and vertebral arteries: imaging features.

Dissection of either the internal carotid artery or the vertebral artery (cervicocephalic arterial dissection) was once considered uncommon. However, in the past few decades, it has been increasingly recognized as a cause of stroke in young and middle-aged adults. Dissection causes only 0.4-2.5% of all strokes in the general population but causes 5-20% of strokes among young patients. Anticoagulant therapy is usually started immediately after the dissection is diagnosed. The clinical and radiologic features are important to recognize so that anticoagulation therapy can be started promptly, the

Report of dissection of the internal carotid artery in three cases.

Whereas previously the dissection of arteries leading to the brain was thought to be a rare occurrence, this diagnosis has been found more frequently in the last years as a result of newer imaging methods. Furthermore, in contrast to earlier opinions, there is a rather good prognosis for the recanalisation of the vessels and the restoration of the patient's health. In this paper, three cases with nearly typical constellation of symptoms and beneficial outcome are demonstrated. The related Doppler sonographical findings including duplex scan as well as the angiographical, computer tomographical

Internal carotid artery dissections: duplex ultrasound imaging.

Duplex ultrasound findings in seven patients (eight vessels) demonstrating extracranial internal carotid artery (ICA) dissections are presented. The two-dimensional (2D) sonographic findings of ICA dissection consisted of three categories: (1) normal, (2) luminal flap with or without thrombus formation, and (3) hypoechoic thrombus with or without lumen narrowing. The Doppler ultrasound waveforms were variable. In the common carotid artery (CCA) these included (1) normal, (2) resistive, damped, or biphasic CCA waveforms, and (3) positive temporal artery tap in the mid-CCA. In the ICA, the appea

Spontaneous dissection of both extracranial internal carotid arteries.

Neurological examination and Doppler sonography of a 50-year-old patient were suggestive of a spontaneous dissection of the left internal carotid artery (left-sided headaches, amaurosis fugax, Horner's syndrome and hemispheric stroke). Four-vessel angiogram performed several days after the onset of the symptoms showed dissection of both extracranial carotid arteries, more pronounced on right side where the dissection was clinically asymptomatic. Angiogram follow-up demonstrated a recanalization of both carotid arteries. A review of 15 other documented reports indicates that bilateral internal

Spontaneous Bilateral Cervical Internal Carotid Artery Dissection Treated with Simultaneous Bilateral Carotid Artery Stenting: A Case Report.

Spontaneous cervical internal carotid artery dissection (CICAD) is occasionally treated with conservative management, mainly using antithrombotics. However, we have to consider emergency interventions for bilateral CICAD occurring simultaneously and accompanied by progressive cerebral ischemia. A 46-year-old woman was brought to our hospital with a complaint of left-handed clumsiness, blurred vision in the left eye, and right hemiplegia. Acute cerebral infarction in bilateral cerebral hemispheres was evident on brain magnetic resonance imaging. Bilateral internal carotid arteries were barely v

Peripheral Facial Nerve Palsy due to Spontaneous Internal Carotid Artery Dissection.

A man in his 50s experienced novel, continuous, and progressive headache and neck pain prior to the onset of left-sided peripheral facial nerve palsy. Sequential palsies of left lower Cranial Nerves IX and XII followed. Imaging showed spontaneous cervical artery dissection (sCeAD) of the ipsilateral internal carotid artery. Lower cranial nerve palsies in sCeAD are a frequent result of a local mass effect exerted by the formation of a mural hematoma. The only close topographical relationship between the facial nerve and the internal carotid artery is within the petrous part of the temporal bone

Villaret's Syndrome Secondary to Internal Carotid Artery Dissection: A Case Report.

Villaret's syndrome is a rare neurological condition characterized by ipsilateral palsies of the cranial nerves (CNs) IX, X, XI, and XII in association with Horner's syndrome. We report a 52-year-old man presenting with headache, hoarseness of voice, and dysphagia following strenuous exercise. Physical examination revealed left-sided lower CN palsies and partial Horner's syndrome, implying pathology in the retroparotid space, the only anatomical site where these nerves are colocated. Initial stroke computed tomography head and angiogram were reported as unremarkable, but a subsequent magnetic

Cervical internal carotid artery fenestration: a rare cause of lumen "dissection''.

PURPOSE: To highlight the clinical and diagnostic importance of correctly identifying cervical internal carotid artery fenestration (fcICA), an extremely rare vascular anomaly, and to present a case where fcICA was initially misdiagnosed as a dissection in a patient with fibromuscular dysplasia (FMD). METHODS: A 47-year-old woman with pulsatile tinnitus underwent computed tomography angiography (CTA) and digital subtraction angiography (DSA) to differentiate between fenestration and dissection of the internal carotid artery. RESULTS: CTA revealed a fusiform dilatation of the distal C1 segmen

Internal Carotid Artery Dissection Presenting with Transient or Subclinical Horner Syndrome.

INTRODUCTION: The most frequently encountered symptoms in internal carotid artery dissection (ICAD) are head or neck pain and cerebral ischemia. Ocular symptoms or signs have been reported as the presenting feature in up to 50% of patients, with (painful) Horner syndrome being the most frequently associated. Horner syndrome is part of the classic triad that depicts the characteristic presentation of ICAD and that consists of pain in the ipsilateral neck, head and orbital regions, (partial) Horner syndrome, and cerebral or retinal ischemia. All patients presenting with painful Horner syndrome s
