Caustic esophageal injury by impaction of cell batteries.

The ingestion of cell batteries can cause serious complications (fistula, perforation or stenosis) at the esophageal level. The damage starts soon after ingestion (approximately 2 hours) and is directly related to the amount of time the battery is lodged in said location, the amount of electrical charge remaining in the battery, and the size of the battery itself. Injury is produced by the combination of electrochemical and chemical mechanisms and pressure necrosis. The ingestion of multiple cells and a size > = 20 mm are related with more severe and clinically significant outcomes. A female p

Upper digestive tract reconstruction for caustic injuries.

OBJECTIVE: The aim of the study was to compare the short- and long-term outcomes of colopharyngoplasty and esophagocoloplasty for caustic injuries of the upper digestive tract. BACKGROUND: Simultaneous esophageal and pharyngeal reconstruction by colopharyngoplasty allows regaining nutritional autonomy in patients with severe pharyngoesophageal caustic injuries. METHODS: Patients who underwent upper digestive tract reconstruction for caustic injuries by colopharyngoplasty (n = 116) and esophagocoloplasty (n = 122) between 1993 and 2012 were included. Survival and functional outcomes were anal

Microscopic acute lesions after caustic exposure.

Although lesions related to chemical burns have been studied through case reports, clinical analyses and autopsy series, microscopic lesions have not yet been precisely described. Our study analyses the microscopic lesions recorded after caustic exposure in fourteen lethal and four non-lethal cases. We find that microscopic lesions after caustic exposure are various and non-specific. Moreover, the distribution of gastrointestinal lesions is inconsistent. Histological changes affect the digestive mucosa first, with the entire wall suffering damage in some cases. Multiple factors influence the p

The oral implications of caustic soda ingestion in children.

The morbidity related to caustic soda (sodium hydroxide) ingestion is well described in the literature. The majority of publications have concentrated on the effects to the trachea and gastrointestinal tract, with little reference to the oral and peri-oral areas. Accidental ingestion of sodium hydroxide-containing substances is fortunately rare; however the consequences can be devastating. Three cases of children who drank caustic substances are described. Treatment included fitting splints, injecting steroids, local surgical procedures, and the use of dynamic appliances to maintain mouth open

A functional study of caustic strictures of the esophagus in children.

The objective of the present study was to assess esophageal motor function in 21 children (7.5 +/- 2.9 years) with caustic strictures. Esophageal manometry was performed using a water-infusion system interfaced with a polygraph and displayed on a computer screen. The data were compared with those obtained from 9 healthy children. Radionuclide transit was determined by studying deglutition of a single bolus of 99mTc pertechnetate in 10 ml of water. Non-peristaltic low-amplitude and long-duration waves were the most common findings detected in patients with strictures longer than 20% of esophage

Caustic ingestion and foreign bodies in the gastrointestinal system.

Infants and young children commonly explore their environment by placing objects in their mouth. Although accidental ingestion can happen in older children and adolescents, recurrent foreign body ingestion is usually seen in mentally retarded and psychiatric patients. These patients present the primary care physician with a challenge of distinguishing these patients who require intervention from the majority who need observation only.

Motor function of the esophagus after caustic burn.

During the subacute and chronic phases of esophagitis due to ingestion of a caustic substance, the patient commonly displays stricture, esophageal rigidity and dysphagia. We used esophageal manometry, radiology, pH monitoring and 99mTc scintigraphy to investigate esophageal motor function in 25 children (mean age 24 +/- 7 months) with chronic esophagitis after second- and/or third-degree caustic burns. The results were compared with those for a control group of 12 children (mean age 32 +/- 19 months) under surveillance for suspected gastroesophageal reflux (GER) but for whom this pathology was

Esophageal motility in patients with esophageal caustic injury.

OBJECTIVE: There are few studies of esophageal function subsequent to the ingestion of lye. We investigated the esophageal motility of patients who had ingested liquid sodium hydroxide. METHODS: Esophageal manometry was performed on 21 patients who [1-53 yr before the manometric examination (median: 13 yr)] had drunk 10-30 g of sodium hydroxide diluted in water. The results were compared with those obtained for a control group of 22 healthy volunteers. RESULTS: The lower esophageal sphincter (LES) pressure of the caustic group (14.9 +/- 1.7 mm Hg, mean +/- SE) did not differ (p > 0.05) from

Colon patch esophagoplasty for caustic esophageal stricture.

Severe caustic injury with stricture may require esophageal reconstruction. The available methods of esophageal substitution do not satisfactorily replace the peristaltic and antireflux properties of the native esophagus. The authors report the results of two children treated with colon patch esophagoplasty and preservation of the injured esophagus. Both were 2 years of age when they accidently swallowed lye. Long esophageal strictures developed, which were resistant to repeated dilatation. Eleven and 13 months after the injury, the patients underwent colon patch esophagoplasty. A segment of t

Caustic ingestion injuries of the upper aerodigestive tract.

Few reports have described in detail the injuries that occur to the oral cavity, pharynx, and larynx following caustic ingestion. The role of dynamic radiographic studies to delineate the extent of damage has been minimized. In-depth radiographic analysis of such cases has not, to our knowledge, been previously reported. In order to examine the injuries and functional abnormalities of these sites following caustic ingestion, the records of The Johns Hopkins Swallowing Center were reviewed. Five patients were identified as having significant upper aerodigestive tract caustic injuries. All patie

Caustic burns of the upper digestive and respiratory tracts.

In a series of 51 children presenting with an accidental caustic burn, symptoms were analysed for their predictive value of significant i.e. necrotizing oesophageal lesions (grade II or III). For the whole group, the incidence of significant oesophageal lesions was 37%. Vomiting and/or respiratory distress were associated with high incidence of significant oesophageal burn (84% and 75% respectively). The particular location of each caustic burn was analysed for its association with caustic burns at other anatomical sites. Of the 18 patients with a laryngeal burn, 72% also had a grade II or III

[Closed thorax esophagectomy in caustic stenosis of the esophagus].

The technique of oesophagectomy without thoracotomy using a stripper for oesophageal eversion has been applied to 6 cases of caustic stenosis of oesophagus in infants. This technic has been realized 6 to 18 months after the initial lesion in 5 cases, 13 years in a case. There is neither morbidity nor mortality. This procedure seems well adapted to pediatric indications of oesophagectomy.

Early evaluation and therapy for caustic esophageal injury.

Forty-one patients with caustic ingestion were reviewed. Eighty-three percent were children, all of whom suffered accidental injury. Liquid drain cleaner was the agent in 57 percent and was responsible for all esophageal burns. Symptoms and physical findings were unreliable in predicting the extent of injury. Endoscopy was performed in most patients within 36 hours of ingestion and accurately estimated the risk of subsequent esophageal stricture formation. Steroid administration had no influence on the development of strictures. Esophageal strictures developed in 22 percent of the patients. On

[Esophagocoloplasty in the treatment of total caustic stenosis of the esophagus].

Since October 1986 until January 1988 we have treated 3 patients with corrosive strictures of the esophagus, by substitution of esophagus with vascularized colon. Two females (4 and 8 years) and one male (5 years) had been treated on others hospitals by means of gastrostomy and periodically retrogradual dilatations; in two of them for more than 24 months. The surgical treatment was indicated upon the evolution time, the degree of dysphagia and the size of the affected segment. The coloesophagoplasty by retrosternal approach with proximal anastomosis in the neck, has been the operative techniqu

Experience with esophageal stenting for caustic burns in children.

Seven children with deep circumferential esophageal burns were treated with antibiotics, steroids, and intraluminal silastic stents. Strictures did not develop if the esophagus was healed at the time the stent was removed. However, strictures developed if healing was incomplete. Esophageal stenting needs to be continued until healing has been demonstrated endoscopically.

[Prevention of caustic stenoses of the esophagus with an intraesophageal mold].

The various methods for the management of full-thickness caustic burns of the esophagus, whether by early bougienage or by corticosteroïd treatment and/or use of an intraluminal catheter, have proved unsuccessful to this date. The use of an intraluminal splint kept in place during 3 weeks, as advocated by Reyes, slows down the growth of exuberant granulation tissue and prevents the formation of synechia and stricture during the healing process. Reyes's method was modified as follows: after a delay of 4 or 5 days, an esophagoscopy is performed along with a histologic study in order to ascertain

[Voluntary poisoning by intravenous injection of caustic soda].

A voluntary intoxication by injection in the left basilic vein of 10 ml of concentrated caustic soda is reported. The main effects were, besides local necrosis, haemolysis, acute renal failure with initial anuresis, intravascular coagulation and cyanosis with a normal Pao2 due to methaemalbuminaemia. This was confirmed by using the usual spectrophotometric methods as well as electrophoretic methods.
