From 1978-1986, nine transabdominal cervicoisthmic cerclage procedures were performed on eight patients at the University of California, San Francisco. In six of the patients, the decision to perform the procedure was based on failed transvaginal cerclages. In five of these six cases, the cervix had deep traumatic defects. In the remaining two patients, the cervix was extremely short. Before the procedure, the eight patients had 25 pregnancies lasting beyond the first trimester, with 20 fetal losses (fetal salvage rate 20%). After the transabdominal procedure, the eight patients had 13 pregnan

A previously healthy woman with a Shirodkar cerclage for cervical incompetence had a spontaneous rupture of the membranes at the 37th week of pregnancy. Three days later after a short period of weak labor pains, she developed a severe sepsis, uterine rupture and coagulopathy leading to renal failure, beta-hemolytic streptococcus group B and peptostreptococcous could be cultured from the amniotic fluid immediately after rupture of the membranes and from the uterus and placenta.

The objective of this study was to re-examine the effect of cerclage on the possible factors associated with preterm delivery in women who had cervical conization. This was a retrospective cohort study comparing the obstetric outcomes of women with or without prophylactic cervical cerclage in pregnancy following a prior conization and managed at our institute between 2004 and 2023. In this study, there were 75% of pregnant women with a history of cervical conization. In 13 women of these (17%), prophylactic cervical cerclage was performed at 12-17 weeks' gestation. The incidence of preterm del

BACKGROUND: Premature cervical ripening plays a significant role in spontaneous preterm birth. Vaginal progesterone is the recommended treatment in singleton pregnancy with incidental short cervix. There is lack of evidence on whether it is beneficial to reinforce the cervix with cerclage when the cervical length becomes progressively shortened <10 mm while on vaginal progesterone.
OBJECTIVE: Our aims are to determine whether cerclage with vaginal progesterone will: (1) reduce the overall spontaneous preterm birth rate, (2) prolong pregnancy latency, and (3) improve neonatal outcomes compared

Cervical cerclage is conventionally classified as history-indicated, ultrasound-indicated, or examination-indicated cerclage, the latter often performed as emergency cervical cerclage. These categories remain clinically useful because they describe common pathways of risk recognition, surveillance, intervention, and rescue. However, they are sometimes interpreted as biologically uniform or sequential stages of cervical insufficiency. This narrative review examines the historical and diagnostic basis of the conventional classification and focuses on its limitations for describing emergency cerc

Laparoscopic transabdominal cerclage (LTAC) is a well documented procedure for cervical incompetence. In this article we have done a detailed stepwise description of LTAC by broadligament window technique. This technique makes the procedure simpler, safer and easy to reproduce.

The case presentation of a transvaginal cervical cerclage performed at a 7 cm dilation in a patient in the 22nd week of pregnancy, followed by a prolongation of the pregnancy until the gestational age of 38 weeks, was reported in the context of many similar cases managed by the authors of the article during a program of screening and prevention of preterm birth. The particularity of the case was the lack of a preterm birth in the medical history of the patient and the installation of the isthmus-cervix incompetence in the second pregnancy, after an on term pregnancy. What should be evidenced i

OBJECTIVE: To assess the effectiveness of elective history-indicated cervical cerclage according to obstetrical history.
STUDY DESIGN: We analyzed pregnancy outcome of a retrospective cohort of women who have had history-indicated McDonald's cerclage. Principal outcome was gestational age (GA) at delivery.
RESULT: Between January 2003 and December 2013, 205 women were included. We analyzed population in two risk groups: 1- Low-risk (≤2 prior preterm birth (PTB)/second trimester loss (STL), or prior success of cerclage), 2- High risk (≥3 prior PTB/STL, or prior failure of cerclage). In the hi

For many years, we and others have reported the efficacy of cervical cerclage in the prevention of miscarriage in patients with uterine malformations. In this paper the experience of 275 cases collected between 1978 and 1998 is reported. Our data indicate that cervical cerclage is effective in preventing miscarriages, prevalently in those pregnancies bearing uterine malformations with simultaneous cervical incompetence.

AIM: To investigate the incidence of complications associated with cervical cerclage by indication.
METHODS: This was a retrospective cohort study of women with singleton pregnancies who underwent cervical cerclage in a single perinatal center between 2008 and 2019. The participants were divided into three groups according to indication: history-indicated cerclage (HIC) group, ultrasound-indicated cerclage (UIC) group, and physical examination-indicated cerclage (PEIC) group. The incidences of perioperative complications within 2 weeks after the procedure, including intraoperative rupture of

Cervical cerclage has always been the main treatment option in cases of so-called cervical insufficiency, a condition that is notoriously associated with a high risk of second trimester abortion and/or preterm delivery. We can distinguish between a prophylactic cerclage, to be performed electively, usually at 13-16 weeks gestation, only when the woman has a history extremely suggestive for cervical incompetence (3 or more mid-trimester abortions or preterm deliveries) and a therapeutic cerclage. This last cerclage is recommended either for women who have ultrasonographic changes consistent wit

Cervical insufficiency is a very difficult diagnosis to confirm. An evidenced based assessment of the randomized clinical trials, meta-analysis, cohort studies and, the American College of Obstetrics and Gynecologist (ACOG) practice bulletin demonstrated there is no difference between women treated with cerclage, and those who received bed rest in patients with an appropriate history for cervical insufficiency. Based on the existing literature, cerclage is rarely, if ever mandated, but rather can be offered if the following criteria are met: (1). signs and symptoms of first trimester abnormali

OBJECTIVE: The aim of this study was to evaluate the efficacy and safety of a noninvasive cerclage pessary in the management of cervical incompetence.
METHODS: This is a prospective cohort study of all pregnant women treated for cervical incompetence during a 4-year period. Women with known risk factors for preterm delivery had transvaginal ultrasonography every 2-3 weeks after 17-19 weeks of gestation. Those with progressive shortening of cervix diagnosed before 30 weeks were treated with a cerclage pessary when the cervical length was < or = 25 mm. The pessary was electively removed at 34-3

OBJECTIVES: To investigate the prevalence and timing of cervical cerclage placement in multiple gestations.
METHODS: Our perinatal database was queried for all multiple gestations delivered at Evanston Hospital from 12/95 through 12/00. This list was then cross-matched with billing and medical records for 'incompetent cervix' and 'cerclage.' The medical records of all deliveries </=26 weeks were reviewed in order to ascertain if cervical incompetence was responsible for the preterm delivery.
RESULTS: There were 802 deliveries of multiple gestations >/=14 weeks over a 5-year period. The numbe

Cervical cerclage is not recognized as the usual treatment of threatened premature delivery including uterine contractions and cervical modifications. Pregnant women with a threatened premature delivery limited to the presence of a short cervix at digital examination or internal scan may however be offered therapeutic cerclage, different from prophylactic first trimester cervical cerclage, when cervical insufficiency is suspected. Studies on cervical cerclage to prevent premature delivery suffered a poor design and do not allow any strong definitive conclusion. In the presence of a shortened o

The use of transabdominal cervico-isthmic cerclage is described in 13 patients with a diagnosis of cervical incompetence. The patients were recruited from seven Scottish Maternity Units over a period of 10 years. The 13 patients have had a successful pregnancy in 86.6% of pregnancies with this procedure compared with a success rate of 16% in their previous pregnancies. In carefully selected cases transabdominal cervico-isthmic cerclage is a worthwhile procedure in patients with cervical incompetence when the cervix is so damaged that it would be impossible to insert a vaginal suture or when a

OBJECTIVE: Our purpose was to review the indications for transabdominal cervicoisthmic cerclage to determine whether it is a valid alternative to transvaginal cerclage.
STUDY DESIGN: A retrospective review of transabdominal cerclage patients at one institution from 1978 to 1994, analysis of the indications for the transabdominal rather than the vaginal approach, and evaluation of fetal outcomes was performed.
RESULTS: Twenty-three patients underwent 24 transabdominal cerclages. The primary indication for transabdominal cervicoisthmic cerclage was failed transvaginal cerclage in 14 patients a
