The VEntriculostomy-Related Infection Score: An Antibiotic Stewardship Tool in Ventriculostomy-Related Infections.

BACKGROUND AND OBJECTIVES: Diagnosing ventriculostomy-related infection (VRI), a common complication after external ventricular drainage (EVD), is challenging and often associated with delayed initiation of antibiotic therapy. We aimed to develop a stewardship score to help in the decision of antibiotic therapy initiation when VRI is suspected. METHODS: This retrospective, single-center cohort study included patients admitted to the intensive care unit after EVD placement who were suspected of having healthcare-associated ventriculitis and/or meningitis between January 1, 2012, and August 31,

Ventriculitis after emergent arch aneurysm surgery.

A 72-year-old man was referred to our institution because of an arch aneurysm and acute aortic dissection (thrombosed Stanford type A). Anti-impulse therapy was initiated. He developed a high fever after admission. Blood culture was negative. Five days after admission, he developed back pain. Thoracoabdominal computed tomography revealed a new low-density area from the distal arch to the descending aorta. We performed emergent total arch replacement. Although we discontinued all sedative drugs after surgery, coma continued. Brain magnetic resonance imaging with diffusion-weighted imaging revea

Unexpected Ventriculitis Complication of Neonatal Meningitis Caused by Streptococcus gallolyticus Subsp. pasteurianus: a Case Report.

Streptococcus gallolyticus subsp. pasteurianus, previously recognized as S. bovis biotype II/2, is an uncommon yet important cause of invasive infection in young infants. Here, we report the first case of ventriculitis that was unexpectedly diagnosed in the course of neonatal meningitis due to S. gallolyticus subsp. pasteurianus, and we review the relevant literature. A 28-day-old male infant from Japan presented with fever, lethargy, and irritability. S. bovis was isolated from blood and the cerebrospinal fluid culture and was then identified as S. gallolyticus subsp. pasteurianus. Intravenou

[Genetic study of holoprosencephaly].

Holoprosencephaly (1/16,000 live births; 1/250 conceptuses) is a complex brain malformation resulting from incomplete cleavage of the prosencephalon, affecting both the forebrain and the face. Clinical expressivity is variable, ranging from a single cerebral ventricule and cyclopia to clinically unaffected carriers in familial dominant autosomic holoprosencephaly. The disease is genetically heterogeneous but additional environmental agents also contribute to the aetiology of holoprosencephaly. In our cohort of 143 patients, 28 heterozygous mutations were identified: 15 in the Sonic hedgehog ge

Value of MRI in the morphological evaluation of tumors of the third ventricle.

The detection or suspicion of a tumoral expansive process in the third ventricule is usually performed by MRI. The contribution of MRI to the diagnosis is unquestionable in view of its accuracy in the topographical characterization of these lesions and in the detection of small formations. The great histological variety of tumours in that region may result in different treatments which can be associated. MRI, therefore, plays an important role in pretherapeutic morphological evaluation. Fifteen patients with tumour of the third ventricle were examined by MRI before treatment. The results of th

Pneumococcal meningitis: clinical-pathological correlations (MeninGene-Path).

Pneumococcal meningitis is associated with substantial mortality and morbidity. We systematically assessed brain histopathology of 31 patients who died of pneumococcal meningitis from a nationwide study (median age 67 years; 21 (67 %) were male) using a pathology score including inflammation and vascular damage. Of the 27 patients with known time from the admission to death, 14 patients died within 7 days of admission and 13 after 7 days of admission. Eleven of 25 (44 %) patients had been treated with adjunctive dexamethasone therapy. Observed pathological processes were inflammation of medium

Cefiderocol for the treatment of difficult-to-treat resistance in Pseudomonas aeruginosa ventriculitis.

PURPOSE: A case in which cefiderocol was used for the treatment of ventriculitis caused by difficult-to-treat resistance in Pseudomonas aeruginosa (DTR-PA) is reported. SUMMARY: A 56-year-old female with a relevant past medical history significant for type 2 diabetes mellitus was admitted to the intensive care unit for an acute large intraparenchymal hemorrhage. Despite numerous attempts at neurosurgical interventions and procedures (eg, hematoma evacuation, external ventricular drain placement, ventriculoperitoneal shunt placement), the patient developed worsening hydrocephalus. A pediatric case of severe bacterial central nervous system infection successfully treated with continuous ventriculospinal drainage.

Bacterial infections of the central nervous system (CNS) in children are associated with high morbidity, particularly when complicated by antibiotic resistance or impaired cerebrospinal fluid (CSF) dynamics. We report a 14-year-old boy with systemic Streptococcus intermedius infection who developed bacterial meningitis, ventriculitis, and communicating hydrocephalus despite broad-spectrum intravenous antibiotics and ventricular drainage. Persistent inflammation and recurrent hydrocephalus with ventricular catheter obstruction necessitated an alternative strategy. Continuous intraventricular-lu

The Role of Intraventricular Antibiotics in the Treatment of Nosocomial Ventriculitis/Meningitis from Gram-Negative Pathogens: A Systematic Review and Meta-Analysis.

BACKGROUND: The emergence of multidrug-resistant pathogens has resulted in difficult-to-treat ventriculitis/meningitis (VM). We used a meta-analysis to study the role of intraventricular (IVT) antibiotic administration as an adjunct (IVT plus intravenous [IV]) to the classic intravenous antimicrobial therapy (IV-only) in the management of VM in terms of infection control, functional outcome, microbial eradication, complications, cost-benefit analysis, infectious mortality, and overall mortality. METHODS: The electronic search focused on adult neurosurgical cases complicated by gram-negative V

Therapeutic Effect of Linezolid in Children With Health Care-Associated Meningitis or Ventriculitis.

We evaluated the efficacy of linezolid treatment in 6 children with health care-associated meningitis or ventriculitis (HCAMV) caused by gram-positive cocci. All children were diagnosed and treated at the Ehime University Hospital between January 2010 and December 2017. Of these, 5 were treated with linezolid as an empirical therapy. In these 5 patients, vancomycin was initially used but was changed to linezolid because of cerebrospinal fluid (CSF) culture positivity (n = 3) and a high minimum inhibitory concentration of vancomycin (n = 2). The most common HCAMV pathogens were methicillin-resi

Metal external ventricular drainage catheters in the treatment of persistent ventriculitis - an old story made new: technical note and preliminary results.

AIM: Conventional ventricular catheters have proven to be ineffective in the drainage of the cerebrospinal fluid in the long run in terms of infection rates. Consequently, the development and clinical evaluation of new catheters is urgently needed. MATERIAL AND METHODS: We implanted a ventricular needle made of stainless steel in 3 neurosurgical patients in whom multiple conventional (silicon) ventricular catheters had been previously implanted. All of them developed persistent ventriculitis during the first 30 days post-admission. The infection did not respond to antibiotics neither intraven

Central nervous system coccidioidomycosis: a clinicopathologic study of treatment with and without amphotericin B.

The clinical and pathologic findings in 32 patients with central nervous system (CNS) coccidioidomycosis were studied. Seventeen patients had received more than 1.5 g of amphotericin B (AMB), chiefly intravenously, during treatment periods of up to eight years. Eight patients had received 246 mg to 1.3 g of AMB, and three patients had received only brief treatment (one to three days; total dose, no more than 100 mg). Fifteen patients had not received AMB. Significant clinical differences between the patients treated with and without AMB were longer survival time following diagnosis of illness

Diffusion-weighted imaging abnormalities patterns in bacterial meningitis.

PURPOSE: Information on abnormal diffusion-weighted imaging (DWI) patterns in bacterial meningitis (BM) is limited. This study aimed to investigate the frequency and patterns of DWI abnormalities in BM in patients with culture-positive BM. METHODS: The medical records of 14 consecutive patients with BM with magnetic resonance imaging evaluation, admitted to our hospital over the past 14 years, were reviewed. BM was defined by a cerebrospinal fluid (CSF) culture positive for bacteria, or the combination of CSF pleocytosis and a positive blood culture. RESULTS: Brain DWI abnormalities were ide

Frontal sinus osteoma with osteoblastoma-like histology and associated intracranial pneumatocele.

Osteomas of the cranial sinuses are rare, benign bony tumors that can be complicated by the formation of an intracranial pneumatocele. If not treated promptly, a pneumatocele can lead to abscess formation, meningitis, or ventriculitis. In the present case, an intracerebral pneumatocele was formed when an 18 cm(3) osteoma breached the posterior wall of the frontal sinus creating a one-way valve through which air could enter the intracranial cavity. The patient presented after forceful sneezing with nonspecific symptoms of headache, nausea, and vomiting. CT demonstrated a frontal collection of l

Septic cortical thrombophlebitis.

Thrombosis of cortical veins has been postulated as an important cause of seizures and focal neurologic deficits in patients with bacterial meningitis. Diagnoses from autopsies, angiograms, and medical records at Massachusetts General Hospital, 1960-1984, were reviewed to identify patients with septic cortical thrombophlebitis. Only 10 confirmed cases of septic cortical vein thrombosis without sagittal sinus thrombosis were found. Meningitis was present in nine patients; Streptococcus pneumoniae was isolated from the blood or cerebrospinal fluid of five patients. Common clinical manifestations
