BACKGROUND: Adequate emergency department (ED) anesthesia for painful ear conditions, such as ear lacerations or ear abscesses, can be challenging. Much of the sensory innervation of the ear is supplied from the anterior and posterior branches of the greater auricular nerve (GAN). The GAN is a branch of the superficial cervical plexus, which arises from the C2/C3 spinal roots. The GAN innervation includes most of the helix, antihelix, the lobule, and the skin over the mastoid process and parotid gland. Anesthesia of the GAN is commonly performed in emergency medicine as part of a landmark-base

OBJECTIVES: To minimize the risk of intraneural injection when performing nerve blocks, some authors caution against injecting through a needle placed with motor responses observed at nerve stimulator output settings of 0.3 mA or less. We present a case of placing a continuous cervical paravertebral catheter with brisk motor response while stimulating the catheter at 0.05 mA, with no adverse sequelae.
CASE REPORT: A 56-year-old man scheduled for rotator cuff repair received a continuous cervical paravertebral block for intraoperative and postoperative pain control. A stimulating catheter was

BACKGROUND: Neck pain is often chronic and disabling. Cervical facet joint injections and epidural steroid injections are frequently used to manage chronic neck pain and cervicogenic headaches. While minimal side effects are commonly associated with these treatments, severe complications are exceedingly rare.
OBSERVATIONS: The authors report 4 cases of iatrogenic neurological injury after radiofrequency ablation (RFA) and epidural steroid injections. One patient experienced left shoulder, scapular, and arm pain with left arm and hand weakness that developed immediately after RFA for chronic n

BACKGROUND: Inferior shoulder dislocation (luxation erecta) is a rare condition, accounting for approximately 0.5% of all shoulder dislocations, typically resulting from high-energy trauma, such as falls or motor vehicle accidents.
CASE PRESENTATION: This case involved a man in his late 50s who sustained an inferior left shoulder dislocation with a greater tuberosity fracture after falling 2 m during work-related activities. Due to the patient's elevated BMI and absence of an anesthesiologist, general anesthesia was not feasible and local anesthesia led to inadequate pain relief and complicat

AIM: Before the introduction of general anesthesia, thyroid operations were performed under local anesthesia. With the development of anesthesia, surgeons preferred to operate in narcosis. Nowadays regional anesthesia has become popular leading to faster recovery.
METHODS: At the Multidisciplinary Head and Neck Cancer Center of our institute, between May 2019 and October 2024, 11 patients were treated in regional anesthesia with the blockage of the superficial branches of the cervical plexus, followed by an ultrasound-guided thyroid capsule sheath space block. Patients were previously given 2

By immunohistochemistry the three neuropeptides enkephalin, substance P, and VIP were all localized to numerous nerve fibers in the smooth muscle layer and the myenteric plexus of the feline pylorus. The motor activity of the pylorus was studied by recording changes in the volume flow of body-warm saline through the sphincter at constant perfusion pressure in anesthetized cats. Efferent electric stimulation of the cervical vagi (8 V; 5 ms; 8 Hz) elicited a noncholinergic, nonadrenergic pyloric contraction, which was blocked by infusion of the opiate receptor antagonist naloxone. The peptides w

OBJECTIVE: The transaxillary approach to thoracic outlet decompression in the presence of cervical ribs offers the advantage of less manipulation of the brachial plexus and associated nerves. This may result in reduced incidence of perioperative complications, such as nerve injuries. Our objective was to report contemporary data for a series of patients with thoracic outlet syndrome (TOS) and cervical ribs managed through a transaxillary approach.
METHODS: We reviewed a prospectively maintained database for all consecutive patients who underwent surgery for TOS and who had a cervical rib. Sym

Brachial plexus avulsion is a rare and debilitating condition frequently associated with severe, intractable neuropathic pain. Interventional treatment modalities include dorsal root entry zone lesioning, stellate ganglion blockade, and neuromodulation such as spinal cord stimulation. We present a case of a 42-year-old woman with a traumatic left upper extremity brachial plexus avulsion injury after a motor vehicle accident and treatment of deafferentation pain complicated by complex regional pain syndrome type II. Previous unsuccessful interventions included repeated stellate ganglion blocks,

Horner's syndrome is rarely observed in epidural anaesthesia; it is characterized by ptosis and enophthalmos on the affected side; miosis, anisocoria, and conjunctival hyperemia in the affected eye and anhydrosis, flushing on the affected side of the face. It is usually a complication spontaneously resolved without permanent neurological deficits. Intraoral anaesthesia, stellate, cervical and brachial plexus block, thoracic, lumbar and caudal epidural anaesthesia and intrapleural analgesia are the main causes associated with Horner's syndrome in anaesthesia. Among the other causes of Horner's

An online database search with subsequent article review was performed in order to review the various analgesic modalities for breast cancer surgery. Of 514 abstracts, 284 full-length manuscripts were reviewed. The effect of pharmacologic interventions is varied (NSAIDS, opioids, anticonvulsants, ketamine, lidocaine). Likewise, data from high-quality randomized, controlled studies on wound infiltration (including liposome encapsulated) and infusion of local anesthetic are minimal and conflicting. Conversely, abundant evidence demonstrates paravertebral blocks and thoracic epidural infusions pr

Patients with Eisenmenger syndrome (ES), characterized by irreversible pulmonary vascular obstructive disease due to an intracardiac shunt, present a very high perioperative risk during noncardiac surgery. The risk is related to severe pulmonary hypertension, right ventricular dysfunction, and potential hemodynamic instability. We report the case of a 68-year-old woman with ES who underwent total thyroidectomy and parathyroidectomy for primary hyperparathyroidism associated with a compressive multinodular goiter. Following multidisciplinary evaluation, surgery was performed under bilateral cer

BACKGROUND: This randomized trial compared ultrasound-guided interscalene block (ISB) and costoclavicular brachial plexus block (CCB) for arthroscopic shoulder surgery. We hypothesized that CCB would provide equivalent analgesia to ISB 30 min after surgery without the risk of hemidiaphragmatic paralysis.
METHODS: All 44 patients received an ultrasound-guided block of the intermediate cervical plexus. Subsequently, they were randomized to ISB or CCB. The local anesthetic agent (20 mL of levobupivacaine 0.5% and epinephrine 5 µg/mL) and pharmacological block adjunct (4 mg of intravenous dexamet

With the extensive application of ultrasound in regional anesthesia, there has been rapid development of interfascial plane block techniques recently. Compared with neuraxial anesthesia or nerve plexus blocks, the interfascial plane blocks have many advantages, such as technical simplicity, fewer complications and comparable or better analgesia. The concept of fascial interconnectivity is fundamental in understanding the effects and complications of interfascial plane blocks. Many fascial planes are continuous and communicate with each other without a clear anatomical boundary. The prevertebra

BACKGROUND: Conventional anesthesia for clavicular fracture surgery poses significant challenges. General anesthesia risks pulmonary complications, while traditional nerve blocks may cause diaphragmatic paralysis or motor impairment. Ultrasound-guided techniques improve precision, but standalone superficial cervical plexus or interscalene brachial plexus blocks often fail to fully anesthetize the deep clavicular region, especially in polytrauma patients. An optimized combined approach could provide effective analgesia while preserving respiratory and motor function.

BACKGROUND: Horner's syndrome is comprised of a set of symptoms caused by a permanent or transient ipsilateral sympathetic trunk lesion or paralysis. It may occur after numerous pathologies in the cervical region, epidural, spinal anaesthesia, and interscalene, transscalene, supraclavicular, or infraclavicular brachial plexus block. The aim of this randomised, prospective clinical study was to evaluate the effect of the interscalene brachial plexus block (IBPB) technique on the occurrence rate of Horner's syndrome and identify contributing risk factors.
METHODS: 108 randomly selected patients

OBJECTIVE: Radical hysterectomy with pelvic lymphadenectomy (RHL) is the preferred treatment for early-stage cervical cancer. Although oncological outcome is good with regard to recurrence and survival rates, it is well known that RHL might result in postoperative bladder impairments due to autonomic nerve disruption. The pelvic autonomic network has been extensively studied, but the anatomy of nerve fibers branching off the inferior hypogastric plexus to innervate the bladder is less known. Besides, the pathogenesis of bladder dysfunction after RHL is multifactorial but remains unclear. We st
