Onchocerciasis in the Cameroon-Chad border area after more than 20 years of annual mass ivermectin distribution.

BACKGROUND: The main vectors of onchocerciasis in Africa are Simulium damnosum sensu lato, which transmit the causative agent Onchocerca volvulus. The force of transmission is driven by the vector density, hence influencing the disease prevalence and intensity. Onchocerciasis is currently targeted for elimination using mass drug administration (MDA) of ivermectin, a potent microfilaricide. MDA in Cameroon began in 1987 in the Vina Valley, an endemic cross-border area with Chad, known for high vector densities and precontrol endemicity. Evaluations in 2008-2010 in this area showed ongoing trans

Syphilis diagnosis and serological response to Benzathine Penicillin G among patients attending HIV clinics in N'Djaména, Chad.

BACKGROUND: Syphilis is endemic in the Sub-Saharan zone and disproportionately affects at-risk populations such as men who have sex with men, sex workers and HIV infected individuals. In this study, we measure the impact of syphilis among people living with HIV in the Republic of Chad, where no data are currently available. METHOD: Outpatients attending 2 HIV clinics in N'Djamena, Republic of Chad, were tested for syphilis. Subjects who tested positive for both non-treponemal (VDRL) and treponemal (TPHA) received a single dose of Benzathine Penicillin G, 2.4 MU. An additional VDRL test was pe

African 1, an epidemiologically important clonal complex of Mycobacterium bovis dominant in Mali, Nigeria, Cameroon, and Chad.

We have identified a clonal complex of Mycobacterium bovis present at high frequency in cattle in population samples from several sub-Saharan west-central African countries. This closely related group of bacteria is defined by a specific chromosomal deletion (RDAf1) and can be identified by the absence of spacer 30 in the standard spoligotype typing scheme. We have named this group of strains the African 1 (Af1) clonal complex and have defined the spoligotype signature of this clonal complex as being the same as the M. bovis BCG vaccine strain but with the deletion of spacer 30. Strains of the

Biological aspects of the lingual papillae of the Arab Zebu cattle: a new perspicuity of its chad ecological adaptations.

BACKGROUND: Our research is the first to explore the ultrastructural features of the lingual papillary system of Arab Zebu cattle, highlighting their Chadian environmental adaptations. RESULTS: There were two types of papillary systems: gustatory (fungiform and circumvallate) and mechanical (filiform, conical, and lentiform). The dorsal surface of the apex and rostral parts of the body had well-developed filiform papillae, whereas the tip's surface had mucosal folds, tubercles, and few filiform papillae. The torus lingua's dorsal surface displayed few lentiform papillae, while two conical pap

Trypanosome infections in naturally infected horses and donkeys of three active sleeping sickness foci in the south of Chad.

BACKGROUND: Equine trypanosomiases are complex infectious diseases with overlapping clinical signs defined by their mode of transmission. Despite their economic impacts, these diseases have been neglected by the scientific community, the veterinary authorities and regulatory organizations. To fill the observed knowledge gap, we undertook the identification of different trypanosome species and subspecies naturally infecting horses and donkeys within the Chadian sleeping sickness focus. The objective of the study was to investigate the potential role of these domestic animals as reservoirs of th

Increasing rabies data availability: The example of a One Health research project in Chad, Côte d'Ivoire and Mali.

Rabies is a fatal but preventable disease that remains notoriously underreported. Weak data availability hampers advocacy, constitutes a barrier to resource allocation and inhibits effective prevention and control. To gain better insight into the global rabies burden and human vaccine demand several studies were funded through the Vaccine Alliance (GAVI) learning agenda. With the help of this funding, Swiss TPH and local in country partner organizations implemented a One Health research project in Chad, Côte d'Ivoire and Mali to collect data at household, public health facility and veterinary

Prevalence of Schistosoma Haematobium Measured by a Mobile Health System in an Unexplored Endemic Region in the Subprefecture of Torrock, Chad.

BACKGROUND: Schistosoma haematobium is a parasitic digenetic trematode responsible for schistosomiasis (also known as bilharzia). The disease is caused by penetration of the skin by the parasite, spread by intermediate host molluscs in stagnant waters, and can be treated by administration of praziquantel. Schistosomiasis is considered to be an important but neglected tropical disease. OBJECTIVE: The aim of this pilot study was to investigate the prevalence of schistosomiasis in the subprefecture of Torrock, an endemic area in Chad where no earlier investigation had been conducted and no distr

Adding tsetse control to medical activities contributes to decreasing transmission of sleeping sickness in the Mandoul focus (Chad).

BACKGROUND: Gambian sleeping sickness or HAT (human African trypanosomiasis) is a neglected tropical disease caused by Trypanosoma brucei gambiense transmitted by riverine species of tsetse. A global programme aims to eliminate the disease as a public health problem by 2020 and stop transmission by 2030. In the South of Chad, the Mandoul area is a persistent focus of Gambian sleeping sickness where around 100 HAT cases were still diagnosed and treated annually until 2013. Pre-2014, control of HAT relied solely on case detection and treatment, which lead to a gradual decrease in the number of c

Knowledge and practices surrounding malaria and LLIN use among Arab, Dazagada and Fulani pastoral nomads in Chad.

BACKGROUND: Chadian pastoral nomads are highly exposed to malaria due to their lifestyle and their mobility between various endemic areas. To inform strategies to reduce nomads' risk of malaria and associated morbidity and mortality, it is important to understand the factors associated to their knowledge of malaria transmission and prevention practices. METHODS: A cross-sectional study among Arab, Dazagada and Fulani pastoral nomadic groups was conducted in February and October 2021. A validated structured electronic questionnaire was administered to assess knowledge of malaria. Attitudes and

Concurrent therapeutic and behavioral interventions are associated with a reduced number of emerging Dracunculus medinensis worms in dogs in Chad.

Dracunculus medinensis (Guinea worm; GW) is a parasitic nematode that causes dracunculiasis (Guinea worm disease; GWD). The annual incidence of GWD in humans has been reduced by over 99.9% globally since the 1980s thanks to the implementation of complementary interventions. Dogs are now the primary hosts of GW and impede eradication efforts. The antihelmenthic drug, flubendazole (FLBZ), was suggested as a possible therapeutic intervention after it was found to be partially effective at reducing fertility of D. medinensis in experimentally infected ferrets. A 2019 clinical trial of FLBZ in Chad

Association of Diarrhea Outcomes with Drinking Water Factors, Sanitation, Hygiene, and Malaria Practices in the Population of Béré, Chad.

Chad, one of the poorest Sub-Saharan Central African countries, has one of the worst global diarrhea burdens. Project 21 seeks to enhance community health in the rural town of Béré, Chad but it is lacking. The study aims to determine diarrhea outcomes and associated factors, such as drinking water, malaria, sanitation and hygiene resources and practices, in Béré. A survey questionnaire was administered by trained community health workers using a random sampling method. The respondents (n = 484) are predominantly Nangtchéré (87%) evangelical (63%) males (88%) aged between 40-59 years old (43%)

Health economic evaluation of strategies to eliminate gambiense human African trypanosomiasis in the Mandoul disease focus of Chad.

Human African trypanosomiasis, caused by the gambiense subspecies of Trypanosoma brucei (gHAT), is a deadly parasitic disease transmitted by tsetse. Partners worldwide have stepped up efforts to eliminate the disease, and the Chadian government has focused on the previously high-prevalence setting of Mandoul. In this study, we evaluate the economic efficiency of the intensified strategy that was put in place in 2014 aimed at interrupting the transmission of gHAT, and we make recommendations on the best way forward based on both epidemiological projections and cost-effectiveness. In our analysi

Economic benefits of keeping vaccines at ambient temperature during mass vaccination: the case of meningitis A vaccine in Chad.

OBJECTIVE: To evaluate the potential economic benefits of keeping a meningitis A vaccine at or near ambient temperature for up to 4 days during a mass vaccination campaign. METHODS: During a 10-day mass vaccination campaign against meningitis A in three regions of Chad in 2011, the costs associated with storage and transport of the vaccine in a traditional cold chain system were evaluated. A mathematical model was used to estimate the savings that could have been achieved if the vaccine had been stored at or near ambient temperature--in a "controlled temperature" chain--at the peripheral leve
