This review focuses on the short and bewildered history of Brazilian scientist Carlos Chagas's discovery and subsequent developments, the anatomopathological features of chronic Chagas cardiomyopathy (CCC), an overview on the controversies surrounding theories concerning its pathogenesis, and studies that support the microvascular hypothesis to further explain the pathological features and clinical course of CCC. It is our belief that knowledge of this particular and remarkable cardiomyopathy will shed light not only on the microvascular involvement of its pathogenesis, but also on the pathoge

BACKGROUND: Chagas disease, caused by the protozoan parasite Trypanosoma cruzi, is a neglected disease that affects approximately 6 million individuals worldwide. Of those infected, 20-30% will go on to develop chronic Chagas cardiomyopathy (CCC), and many ultimately to advanced heart failure. The mechanisms by which this progression occurs are poorly understood. In this exploratory study, we sought to provide insight into the physiologic changes associated with the development of early CCC.
METHODS: We used RNA sequencing to analyse the gene expression changes in the peripheral blood of six

BACKGROUND: Heart transplant (HT) remains the most frequently indicated therapy for patients with end-stage heart failure that improves prognosis in Chagas cardiomyopathy (CCM). However, the lack of benznidazole therapy and availability of RT-PCR follow-up in many centers is a major limitation to perform this life-saving intervention, as there are concerns related with the risk of reactivation. We aimed to describe the outcomes of a cohort of patients with CCM who underwent HT using a conventional protocol with mycophenolate mofetil, without benznidazole prophylaxis or RT-PCR follow-up.

Chronic Chagas disease cardiomyopathy (CCC), an especially aggressive inflammatory dilated cardiomyopathy caused by lifelong infection with the protozoan Trypanosoma cruzi, is a major cause of cardiomyopathy in Latin America. Although chronic myocarditis may play a major pathogenetic role, little is known about the molecular mechanisms responsible for its severity. The aim of this study is to study the genes and microRNAs expression in tissues and their connections in regards to the pathobiological processes. To do so, we integrated for the first time global microRNA and mRNA expression profil

Clinical symptoms of chronic Chagas disease occur in around 30% of the individuals infected with Trypanosoma cruzi and are characterized by heart inflammation and dysfunction. The pathogenesis of chronic chagasic cardiomyopathy (CCC) is not completely understood yet, partially because disease evolution depends on complex host-parasite interactions. Macrophage migration inhibitory factor (MIF) is a pleiotropic proinflammatory cytokine that promotes numerous pathophysiological processes. In the current study, we investigated the link between MIF and CCC progression.Immunohistochemical analysis d

Neurohormonal benefits of Chronic Chagas' Cardiomyopathy (CCC) remain controversial. This study aimed to assess therapeutic interventions on CCC evolution in T. cruzi-infected hamsters at pre and post-treatment (2 months with beta-blocker (CH + BB) or ACE inhibitor (CH + ACEI)). Echocardiography was performed through evolution and compared to histopathological myocardial analysis. At post-treatment, a significant reduction of LV global systolic function and segmental function was observed between the control group and all Chagas' groups. Compared to the Control, a reduction in LV regional stra

BACKGROUND: Chronic Chagas cardiomyopathy (CCM) is characterized by a unique type of cardiac involvement. Few studies have characterized echocardiographic (Echo) transitions from the indeterminate Chagas disease (ChD) form to CCM. The objective of this study was to identify the best cutoffs in multiple Echo parameters, speckle tracking, and N-terminal pro B-type natriuretic peptide (NT-proBNP) to distinguish patients without CCM (stage A) vs patients with myocardial involvement (stages B, C, or D).
METHODS: Cross-sectional study conducted in 273 consecutive patients with different CCM stages.

BACKGROUND: With the evolution of chronic Chagas cardiomyopathy (CC) and the progression towards heart failure (HF), patients may show a decline in inspiratory muscle strength, lung function, and functional capacity.
OBJECTIVE: We compared respiratory function and submaximal functional capacity in patients with CC with versus without HF.
METHODS: This observational, cross-sectional study was carried out with CC patients divided into CCG, a group without HF (n = 28), and HFG, a group with HF (n = 27). Spirometry (percent predicted forced vital capacity (ppFVC), forced expiratory volume in one

INTRODUCTION: The aim of this investigation was to establish the clinical characteristics and outcomes of patients with non-dilated left ventricular cardiomyopathy (NDLVC) secondary to chronic Chagas disease (CChD).
METHODS: All patients with CChD followed at our institution from January 2000 to January 2010 were included. Patients with a left ventricular ejection fraction (LVEF) <50%, a left ventricular diastolic diameter (LVDD) <55 mm, and segmental wall motion abnormalities (SWMA) on echocardiography were diagnosed with NDLVC secondary to CChD. The remaining patients had dilated cardiomyop

Cardiac involvement represents the main cause of mortality among patients with Chagas disease, and the relevance of trypanocidal treatment to improving diastolic dysfunction is still doubtful. In the present study, we used a canine model infected with the benznidazole-sensitive Berenice-78 Trypanosoma cruzi strain to verify the efficacy of an etiologic treatment in reducing the parasite load and ameliorating cardiac muscle tissue damage and left ventricular diastolic dysfunction in the chronic phase of the infection. The effect of the treatment on reducing the parasite load was monitored by bl

Chagas cardiomyopathy is a leading cause of heart failure (HF) with reduced ejection fraction (HFrEF) in endemic regions, yet patients with this distinct etiology have been underrepresented in pivotal HF trials. This creates significant uncertainty regarding the efficacy of guideline-directed therapies, including sacubitril-valsartan, in this population. PubMed, Cochrane (CENTRAL), Scopus, and Embase were searched from inception to December 17, 2025, for randomized controlled trials comparing sacubitril-valsartan to enalapril in patients with HFrEF due to Chagas cardiomyopathy. Primary outcome

BACKGROUND: Chagas cardiomyopathy (ChC) is associated with a high burden of ventricular arrhythmias (VA), but long-term outcomes of catheter ablation (CA) in this population remain poorly characterized, especially when compared to other cardiomyopathies.
METHODS: We performed a single-center retrospective cohort of consecutive patients with structural heart disease undergoing catheter ablation for sustained monomorphic VT (2011-2020) at a tertiary hospital in Brazil, grouped as ChC (n = 164), ischemic cardiomyopathy (ICM; n = 76) or idiopathic dilated cardiomyopathy (DCM; n = 48). Chagas disease (CD), caused by the protozoan Trypanosoma cruzi, affects millions worldwide, with a high prevalence in Latin America, representing a significant public health burden. The transition from the asymptomatic acute phase to the chronic stage, where about 30% of individuals develop cardiomyopathy, poses a scientific challenge. This study explored the role of Toll-like receptor 2 (TLR2) and interleukin-1β (IL-1β) in the pathogenesis of systolic dysfunction in chronic Chagas cardiomyopathy (CCC). Through flow cytometry analysis of CD14 + HLA-DR+ cells, after trypomastigote infection ass

Chagas disease is a tropical zoonosis caused by Trypanosoma cruzi. After infection, the host present an acute phase, usually asymptomatic, in which an extensive parasite proliferation and intense innate immune activity occurs, followed by a chronic phase, characterized by low parasitemia and development of specific immunity. Most individuals in the chronic phase remain without symptoms or organ damage, a state called indeterminate IND form. However, 20 to 40% of individuals develop cardiac or gastrointestinal complications at any time in life. Cardiomyocytes have an important role in the devel

Background: Chagas disease, caused by the protozoan Trypanosoma cruzi, is endemic in Latin America. Thirty percent of infected individuals develop chronic Chagas cardiomyopathy (CCC), an inflammatory dilated cardiomyopathy that is the most important clinical consequence of T. cruzi infection, while the others remain asymptomatic (ASY). IFN-γ and IFN-γ-producing Th1-type T cells are increased in peripheral blood and CCC myocardium as compared to ASY patients, while the Th1-antagonizing cytokine IL-10 is more expressed in ASY patients. Importantly IFN-γ-producing Th1-type T cells are the most fr

Clinical characteristics, predictors of mortality, and outcome of patients with Chagas cardiomyopathy (ChCM) with heart failure with mildly reduced ejection fraction (HFmrEF) are unknown. 215 patients with a positive serology for ChCM and a left ventricular EF (LVEF) < 50 % followed at our ambulatory from January 2000 to January 2010 were included in the study. 36 patients (20 %) had a LVEF between 41 % and 49 % (HFmrEF), whereas the remaining 179 (80 %) patients had a LVEF ≤40 % (HFrEF). No difference was observed between both groups regarding clinical characteristics. Beta-Blocker (BB) thera
