Effects of cancer treatment during pregnancy on fetal and child development.

It has become clear that, for specific cancers and under well defined circumstances, oncological treatment in pregnancy is possible. In this Review, we summarise the evidence on fetal, neonatal, short-term, and long-term effects of prenatal exposure to cancer treatment on the child. So far, outcomes of children are generally reassuring, but long-term follow-up is insufficient. The most important risks of chemotherapy during pregnancy are preterm birth and babies being small for gestational age. Chemotherapy in the first trimester is contraindicated because of an increased risk of congenital ma

Prenatal head growth and child neuropsychological development at age 14 months.

OBJECTIVE: We sought to assess the association between prenatal head growth and child neuropsychological development in the general population. STUDY DESIGN: We evaluated 2104 children at the age of 14 months from a population-based birth cohort in Spain. Head circumference (HC) was measured by ultrasound examinations at weeks 12, 20, and 34 of gestation and by a nurse at birth. Head growth was assessed using conditional SD scores between weeks 12-20 and 20-34. Trained psychologists assessed neuropsychological functioning using the Bayley Scales of Infant Development. Head size measurements a

Care for child development: basic science rationale and effects of interventions.

The past few years have witnessed increasing interest in devising programs to enhance early childhood development. We review current understandings of brain development, recent advances in this field, and their implications for clinical interventions. An expanding body of basic science laboratory data demonstrates that several interventions, including environmental enrichment, level of parental interaction, erythropoietin, antidepressants, transcranial magnetic stimulation, transcranial direct current stimulation, hypothermia, nutritional supplements, and stem cells, can enhance cerebral plast

Donor insemination: child development and family functioning in lesbian mother families.

Findings are presented of a comparative study investigating the family relationships and the emotional and gender development of children raised in lesbian mother families. A total of 30 lesbian mother families with 4-8 year old children created as a result of donor insemination (DI) were compared with 38 heterosexual families with a DI child and with 30 heterosexual families who had a naturally conceived child. A variety of assessment measures, including a standardized interview and questionnaires from the parents and psychological testing of the child were used to collect the data. The quali

Otitis media, hearing loss, and child development: a NICHD conference summary.

Growing evidence indicates that a significant relationship exists between the conductive hearing loss resulting from recurrent otitis media (OM) during the first 3-5 years of life and subsequent problems in acquisition of language and academic skills. To assess current knowledge of OM and its consequences for cognitive and linguistic development, to exchange viewpoints, and, if possible, to determine directions for future research, a conference was sponsored by the National Institute of Child Health and Human Development (NICHD). Among the epidemiologic studies cited, some found a very high in

Retaining infants and young children who experience transitions in care in longitudinal studies of child health and development: Considerations from the HEALthy Brain and Child Development study.

A transition in care (TIC) is a significant change in the primary adults who provide care for a child, involving a move to informal or formal non-parental care, including kinship and foster care. In this paper, we address three issues: (1) the theoretical and empirical reasons for retaining infants and children who experience TIC in longitudinal studies of child health and development; (2) the import of retaining infants and children who experience TIC in studies focusing on parental substance use; and (3) methodological strategies for following children with TIC. We discuss the HEALthy Brain

Development and implementation of a method to assess food and nutrient intakes in the Seychelles Child Development Nutrition Study.

Foods and nutrients may influence methyl mercury (MeHg) absorption and / or metabolism in humans. Consequently, epidemiological studies of MeHg exposure should assess dietary habits. The aim of this paper is to describe the development, implementation and preliminary results of the dietary assessment method used in the Seychelles Child Development Nutrition Study (SCDNS). A researcher from the University of Ulster (UU) visited Seychelles to consult with health professionals, and to ascertain the types of foods typically eaten, population characteristics, and potential study resources. Disparities in Under-Five Child Injury Mortality between Developing and Developed Countries: 1990-2013.

OBJECTIVE: Using estimates from the 2013 Global Burden of Disease (GBD) study, we update evidence on disparities in under-five child injury mortality between developing and developed countries from 1990 to 2013. METHODS: Mortality rates were accessed through the online visualization tool by the GBD study 2013 group. We calculated percent change in child injury mortality rates between 1990 and 2013. Data analysis was conducted separately for <1 year and 1-4 years to specify age differences in rate changes. RESULTS: Between 1990 and 2013, over 3-fold mortality gaps were observed between develo

Effect of Per and Poly-Fluoroalkyl Substances on Pregnancy and Child Development.

BACKGROUND: Childhood obesity is significantly influenced by maternal exposure to Per and Poly-Fluoroalkyl Substances (PFAS) during pregnancy. PFAS exposure occurs through the Peroxisome Proliferator-Activated Receptor (PPAR-γ) receptor, leading to increased fat deposition and profound health effects in child growth and development. Despite ongoing investigations, the relationship between maternal serum PFAS concentration and child obesity requires further exploration. OBJECTIVE: This study aimed to review the possible effects of Per and poly-fluoroalkyl substances exposure and their mechanis

Development of Dysplastic Nevi in a Child with LEPR Deficiency Treated with Setmelanotide.

Setmelanotide is a recently approved medication for patients over two years of age with monogenic obesity that emerges from POMC, LEPR, PCSK1 mutations, or Bardet-Biedl syndrome. While primarily targeting melanocortin-4 receptors (MC4R), setmelanotide also weakly stimulates melanocortin-1 receptors (MC1R), which may affect pigmentation. Clinical outcomes of this treatment modality remain limited due to the rarity of disorders mentioned above. We present a 12-year-old boy with a homozygous LEPR mutation who experienced skin hyperpigmentation shortly after the initiation of setmelanotide treatme

An introduction to the HEALthy Brain and Child Development Study (HBCD) study.

The fundamental organization of the human brain is established before birth, with rapid growth continuing over the first postnatal years. Children exposed before or after birth to various biological (e.g., substance exposure) or psychosocial hazards (e.g., maltreatment) are at elevated likelihood of deviating from a typical developmental trajectory, which in turn can be associated with psychological, behavioral, and physical health sequelae. In the HEALthy Brain and Child Development (HBCD) Study, a multi-site prospective longitudinal cohort study, brain, physical, biological, cognitive, behav

The impact of family stress and resilience on child development: a scoping review.

OBJECTIVE: Children grow up spending most of their time within the family social environment, where they can experience stressful situations such as marital conflict, a non-cohesive environment, parental alcohol use disorder, parental depression, and other parental mental health issues. All these factors are associated with children's developmental delays. We aimed to conduct a scoping review on associations between family stress and family resilience and child development delays to examine familial conditions associated with child development. METHODS: We conducted a scoping review of observ

Child pornography possession/receipt offenders: developing a forensic profile.

Child pornography possession/receipt offenders are a controversial offender group due to mixed and occasionally divergent evidence about their risk profile, offending history and psychopathology. Using a population of male offenders who ever perpetrated a sexual offense from a federal jurisdiction in the central United States, the current study developed an exploratory post hoc empirical profile of these offenders. The profile has some success in the validation component of our study and showed significant associations with self-reported sexual abuse of child victims ages 3-12 years, but non-s

Myasthenia Gravis Can Have Consequences for Pregnancy and the Developing Child.

Myasthenia gravis (MG) with onset below 50 years, thymic hyperplasia and acetylcholine receptor (AChR) antibodies is more common in females than in males. For a relatively large group of MG patients, pregnancy represents therefore an important question. The muscle weakness, the circulating autoantibodies, the hyperplastic thymus, the MG drug treatment, and any autoimmune comorbidity may all influence both mother and child health during pregnancy and also during breastfeeding in the postpartum period. Mother's MG remains stable in most patients during pregnancy. Pyridostigmine, prednisolone, an

Development of bilateral coronary artery aneurysms in a child with Noonan syndrome.

Noonan syndrome is a constellation of congenital malformations including heart defects, facial anomalies and short stature. The cardiovascular defects are variable and extensive, with the most common being pulmonary stenosis and hypertrophic cardiomyopathy. Coronary artery anomalies have only been reported in a few cases. We report a child with Noonan syndrome status post pulmonary stenosis and atrial septal defect repair, who developed bilateral coronary artery aneurysms. The aneurysms were diagnosed with both cardiac magnetic resonance imaging and coronary computed tomography angiography. Th

Risk for tuberculosis in child contacts. Development and validation of a predictive score.

RATIONALE: Contact investigation of persons exposed to tuberculosis (TB) is resource intensive. To date, no clinical prediction rule for TB risk exists for use as a guide during contact investigation. OBJECTIVES: We sought to develop and validate a simple and easy-to-use predictive score for TB risk, using data routinely available during contact investigation. METHODS: The derivation cohort consisted of 9,411 children aged 0 to 12 years from 2008 to 2009 national contacts cohort. We used a multivariate Cox proportional hazards model to predict the risk of developing active TB. The validation

Management of a child with severe hypodontia in the mixed dentition stage of development.

BACKGROUND: Severe hypodontia is a condition characterized by developmental absence of six or more teeth and affects 0.14-0.3% of the overall population. Hypodontia can have a marked psychosocial effect and functional implications for a growing child. CASE REPORT: A 10-year-old girl with a medical history of hypothyroidism and repaired spinal bifida was referred to the Leeds Dental Institute as she was becoming increasingly concerned about her appearance. On clinical examination, the patient had severe wear of her over-retained maxillary primary central incisors, microdontia of mandibular ant

Pulse oximetry: technology to reduce child mortality in developing countries.

The causes of hypoxaemia in children include the commonest causes of childhood illness: pneumonia and other acute respiratory infections, and neonatal illness, particularly sepsis, low birthweight, birth asphyxia and aspiration syndromes. The systematic use of pulse oximetry to monitor and treat children in resource-poor developing countries, when coupled with a reliable oxygen supply, improves quality of care and reduces mortality. Oximetry also has a well established role in surgery and anaesthesia, but in many countries children undergo surgery without the safety of oximetry monitoring. Thi
