Intrahepatic cholestasis of pregnancy: perinatal outcome associated with expectant management.

OBJECTIVE: Our goal was to compare the pregnancy outcomes of patients with intrahepatic cholestasis of pregnancy managed expectantly with antepartum testing with those of other patients who were followed up with a similar testing scheme. STUDY DESIGN: Cases of intrahepatic cholestasis of pregnancy monitored with antepartum testing at our institution over a 7-year period were reviewed. Their pregnancy outcomes were compared with those of control patients followed up with the same testing scheme for a history of stillbirth. Both groups had at least weekly nonstress tests and amniotic fluid asse

Selective surgical management of progressive familial intrahepatic cholestasis (Byler's disease).

Progressive familial intrahepatic cholestasis (PFIC) presents in early childhood with pruritus, jaundice, hepatomegaly, and growth failure. Medical therapy is unsuccessful, with progression from cholestasis to hepatic fibrosis, cirrhosis, and ultimately death before the age of 10 years. Because of evidence that biliary diversion can arrest or reverse progression to hepatic fibrosis, we have used partial biliary diversion (PBD) as primary therapy in PFIC, reserving orthotopic liver transplantation (OLT) for children who have progressive disease or established cirrhosis. Seventeen children with

Persistent intrahepatic cholestasis secondary to multiple etiologies: a case report.

Intrahepatic cholestasis may arise from multiple etiologies, but it is particularly challenging to identify its primary etiology, especially when several potential contributors overlap and the disease itself progresses over time. Herein, we report a 42-year-old man with adult-onset Still's disease (AOSD) who developed persistent severe intrahepatic cholestasis. At early stage of this disease, intrahepatic cholestasis was attributed to severe liver injury secondary to AOSD and drug-induced liver injury. Subsequently, sepsis developed, which might be associated with the use of glucocorticoids an

Odevixibat: a promising new treatment for progressive familial intrahepatic cholestasis.

INTRODUCTION: Progressive familial intrahepatic cholestasis (PFIC) refers to a group of heterogeneous, mostly autosomal recessive disorders resulting from the inability to properly form and excrete bile from hepatocytes. The resulting shared phenotype is one of hepatocellular cholestasis. Clinical management targeting refractory itch and surgical interventions to interrupt the enterohepatic circulation are often pursued with variable efficacy. Recent development of the family of IBAT inhibitor therapeutics has introduced a novel tool in the armamentarium for the treatment of PFIC. AREAS COVER

Single-center experience in management of progressive familial intrahepatic cholestasis.

BACKGROUND AND STUDY AIMS: Progressive familial intrahepatic cholestasis (PFIC) is an autosomal recessively inherited disease that causes intrahepatic-hepatocellular cholestasis. PFIC constitutes approximately 10-15% of cholestatic liver diseases in children. The aim of this study is to draw attention to this group of diseases, which pose a higher risk, in societies where consanguineous marriage is more common, and to share our experiences since the studies in the literature, regarding this group of diseases are case series with small number of patients. PATIENTS AND METHODS: This cross-secti

Outpatient versus inpatient follow-up for intrahepatic cholestasis of pregnancy.

AIM OF THE STUDY: The optimum management method and the best time of delivery still remain unclear for intrahepatic cholestasis of pregnancy (ICP). We aimed to ascertain whether there is a benefit of close monitoring at hospital. MATERIAL AND METHODS: We evaluated the maternal and neonatal records of ICPs over a recent five-year period. A total of 35 women and their 38 newborns were analyzed. The impact of hospitalization was evaluated in terms of delivery type, labor induction, preterm delivery, Apgar scores, and neonatal intensive care unit admission. RESULTS: The median maternal age was 3

Surgical treatment for intractable pruritus in progressive familial intrahepatic cholestasis.

BACKGROUND/AIM: Posttransplant cardiovascular mortality is still an important problem in renal transplant patients. In addition to conventional coronary risk factors, coagulation abnormalities play a key role in the hypercoagulable state observed in transplanted patients. Though renal transplantation eliminates cardiovascular disease risk factors by restoring renal function, it introduces new cardiovascular risks derived, in part from immunosupressive medications. We aimed to assess the effect of calcineurin inhibitors on endothelial function, platelet activation and aggregation in renal trans

Unnecessary confusion about family planning after intrahepatic cholestasis of pregnancy.

BACKGROUND: As conceptions have changed regarding the suitability of oral contraceptives for women with a history of intrahepatic cholestasis of pregnancy (ICP), we studied whether the contraindications formerly in force had affected family planning decisions and mode of contraception among women with such a history. STUDY DESIGN: ICP women and their controls who gave birth in 1969-1988 in Tampere University Hospital, Finland, were sent a questionnaire in 2010. The inquiry covered items concerning contraception, deliveries, menstruation and sex life. RESULTS: ICP women had limited their numb

Doppler umbilical artery velocimetry in pregnancies complicated by intrahepatic cholestasis.

Doppler umbilical velocimetry is a useful clinical tool for antepartum fetal surveillance of pregnancies at risk of fetal compromise. Intrahepatic cholestasis of pregnancy is associated with an increased incidence of fetal death, which might due to the toxic effect of elevated maternal serum concentrations of bile acids. To study a possible effect of the concentration of bile acids on the umbilical circulation we performed pulse-wave Doppler velocimetry of the umbilical artery in 15 patients with intrahepatic cholestasis between 34 and 38 weeks of gestation. The findings were compared to the D

[Neonatal intrahepatic cholestasis and alpha-1-antitrypsin deficiency (author's transl)].

A case of alpha-1-antitrypsin deficiency in an 11 month-old Pi ZZ infant is reported. The patient exhibited intrahepatic cholestasis in the first months of life, with jaundice and hepatomegaly. Liver biopsy confirmed the diagnosis and showed the presence of hepatic fibrosis. Pi phenotypes, serum levels of alpha-1-antitrypsin, and symptoms in the family of the patient are reported. Clinical, biochemical and pathologic findings in liver disease in alpha-1-antitrypsin deficiency are discussed.

GGT-Normal Cholestasis in an Older Child: A Suspected Case of Benign Recurrent Intrahepatic Cholestasis From Syria.

Cholestasis beyond infancy is uncommon and presents a diagnostic challenge. We report an 11-year-old girl presenting with severe pruritus followed by jaundice and a cholestatic biochemical profile with normal gamma-glutamyl transferase levels. Extensive evaluation excluded infectious, autoimmune, metabolic, and obstructive causes. Liver biopsy demonstrated bland cholestasis with preserved hepatic architecture. The patient showed complete clinical and biochemical recovery following supportive therapy with ursodeoxycholic acid. Although this represents a first documented episode, the findings ar

Late-Onset Drug-Induced Cholestasis in a Living-Related Liver Transplant Donor With Progressive Familial Intrahepatic Cholestasis.

We present a rare case of progressive familial intrahepatic cholestasis within a family. A 34-yearold female became a living-related liver transplant donor for her son, who had the disease. Nine years after the transplant, the mother developed severe intrahepatic cholestasis, for which she was evaluated after using an oral contraceptive drug. She presented with jaundice, pruritus, and increased bilirubin levels, together with elevated gamma glutamyl transferase and alkaline phosphatase levels. A liver biopsy revealed findings consistent with intrahepatic cholestasis. However, despite follow-up

Familial intrahepatic cholestasis - An uncommon cause of jaundice in young patients.

Progressive familial intrahepatic cholestasis (PFIC) is a heterogeneous group of autosomal recessive disorders resulting from mutations in genes involved in bile secretion, characterized by chronic cholestasis. The onset is typically in early childhood, with main clinical manifestations including jaundice, pruritus, and symptoms related to malabsorption, which can rapidly progress to liver failure. We present a case of PFIC secondary to myosin 5B mutations.

Current understanding of essential trace elements in intrahepatic cholestasis of pregnancy.

Trace elements are important components in the body and have fundamental roles in maintaining a healthy and balanced pregnancy process. Either deficiency or excess of trace elements, including selenium, iron, zinc, copper, and magnesium can lead to pregnancy complications. As a rare disorder during pregnancy of unknown aetiology, intrahepatic cholestasis of pregnancy (ICP) poses a significant risk to the fetus of perinatal mortality. ICP is a multifactorial complication of which the pathogenesis is still an enigma. Epidemiological studies have demonstrated the association of ICP with some trac

[Intrahepatic Cholestasis and Hyperbilirubinemia Induced by Hyperthyroi- dism: A Case Series].

Hyperthyroidism is a disorder characterized by excess endogenous production of thyroid hormones. It is common to find alterations in hepatic biochemistry related to this diagnosis. These include elevated alkaline phosphatase, elevated aminotransferases and, to a lesser extent, hyperbilirubinemia, usually mild and asymptomatic. Here we present two cases of severe direct hyperbilirubinemia induced by hyperthyroidism, one of them with associated intrahepatic cholestasis. These are rare but well-known forms of presentation of the disease.
