OBJECTIVE: To compare leuprolide acetate to hCG as the trigger for final oocyte maturation in oocyte donor cycles.

PATIENT(S): Thirty-two healthy oocyte donors aged 21-33 years with adequate ovarian reserve.

BACKGROUND: Quantitative and qualitative human chorionic gonadotropin (hCG) tests are obtained in the emergency department (ED) to determine if a female of child-bearing age is pregnant. A positive hCG result is commonly assumed to indicate an intrauterine or other form of pregnancy. However, elevated hCG levels can also result from various other conditions, such as ovarian tumors, pituitary tumors, and thyroid disorders. Intracranial germ cell tumors, rare central nervous system tumors capable of secreting hCG, primarily affect adolescent and young adult females.

LH/hCG receptor has been found in extragonadal tissues in human and animals. The myometrium presents such receptors but their functional role is still not clear. Postmortem human chorionic gonadotrophin (HCG) blood assay can confirm postmortem diagnosis of pregnancy or document situations in which HCG levels are elevated. In some cases, however, blood sampling is not possible at autopsy. A woman presented to our emergency center after self-injection of human chorionic gonadotropin in an attempt to gain admission to the hospital. Her initial urine pregnancy test (an assay for beta-hCG) was negative. Numerous reports have dealt with the usefulness of the maternal serum alpha-fetoprotein marker (MsAFP) and human chorionic gonadotropin (MshCG) levels in the detection of Down syndrome (DS) and other autosomal trisomies. Only few reports have discussed the possible association of elevated levels of MshCG and sex chromosome aneuploidy. We wish to report on 3 cases in which this association was found.

Of 22 consecutive cases of squamous cell carcinomas of the urinary bladder, 4 expressed the beta subunit of human chorionic gonadotropin as demonstrated by immunohistochemistry. The significance of this finding is compared to similar observations made in urothelial carcinomas by others and its value as a possible marker in diagnosing and monitoring the progress of these tumors is discussed.
