Granulation tissue myofibroblasts during normal and pathological skin healing: The interaction between their secretome and the microenvironment.

The first role that was proposed for the myofibroblasts located in skin granulation tissue was to contract the edges of the wound in order to reduce the surface to be repaired. This role, linked to the presence of alpha smooth muscle actin, was very quickly confirmed and is part of the definition of granulation tissue myofibroblasts. However, myofibroblasts are cells that also play a much more central role in wound healing. Indeed, it has been shown that these cells produce large quantities of matrix components, and that they stimulate angiogenesis and can recruit immune cells. These actions t

[Transepidermal water loss of scar skin in three types of scar patients and its correlation with scar severity].

Objective: To study the transepidermal water loss (TEWL) of scar skin in patients with superficial scars, hypertrophic scars, and atrophic scars, and to explore the correlation between TEWL and scar severity. Methods: A retrospective observational study was conducted. From February 2017 to February 2019, 120 scar patients who met the inclusion criteria were admitted to the General Hospital of Jilin Chemical Industry Group, including 78 males and 42 females, aged (35±14) years. According to the diagnosis on admission, there were 40 cases of superficial scar patients, 40 cases of hypertrophic sc

IR-808 loaded nanoethosomes for aggregation-enhanced synergistic transdermal photodynamic/photothermal treatment of hypertrophic scars.

Synergistic transdermal photodynamic therapy (PDT)/photothermal therapy (PTT) has emerged as a novel strategy for improving hypertrophic scar (HS) therapeutic outcomes. Herein, a near-infrared heptamethine cyanine dye, named IR-808, has been selected as the desirable photosensitizer owing to its PDT and PTT properties. Benefitting from the transdermal delivery ability of ethosomes (ESs), IR-808 loaded nanoethosomes (IR-808-ES) have been prepared as a novel nanophotosensitizer for the transdermal PDT/PTT of HSs. The special structure of IR-808 aggregate distribution in the ES lipid membrane enh

Notch-ing up knowledge on molecular mechanisms of skin fibrosis: focus on the multifaceted Notch signalling pathway.

Fibrosis can be defined as an excessive and deregulated deposition of extracellular matrix proteins, causing loss of physiological architecture and dysfunction of different tissues and organs. In the skin, fibrosis represents the hallmark of several acquired (e.g. systemic sclerosis and hypertrophic scars) and inherited (i.e. dystrophic epidermolysis bullosa) diseases. A complex series of interactions among a variety of cellular types and a wide range of molecular players drive the fibrogenic process, often in a context-dependent manner. However, the pathogenetic mechanisms leading to skin fib

Evidences of autologous fat grafting for the treatment of keloids and hypertrophic scars.

INTRODUCTION: Since the 1980s, the use of autologous fat grafting has been growing in plastic surgery. Recently, this procedure has come to be used as a treatment for keloids and hypertrophic scars mainly due to the lack of satisfactory results with other techniques. So far, however, it lacks more consistent scientific evidence to recommend its use. The aim of this study was to review the current state of autologous fat grafting for the treatment of keloids and hypertrophic scars, their benefits and scientific evidences in the literature. METHOD: A review in the Pubmed database was performed

The correlation of in vivo burn scar contraction with the level of α-smooth muscle actin expression.

This study describes the direct association of in vivo burn scar contraction with the level of α-smooth muscle actin (α-SMA) in scar tissue, in a porcine burn model. The expression of α-SMA was investigated in 100 biopsies from 44 6-week old burn scars and in 85 biopsies from 16 2-week old burn wounds. Statistical analysis showed that the levels of α-SMA in 6-week old scars were significantly negatively correlated to scar size (r=-0.68) and the higher levels of α-SMA were observed in smaller scars. Moreover, α-SMA was also found to be significantly positively correlated to re-epithelialisation

OBJECTIVE: To explore the role of collagen degradation and scar morphology and structure in the formation of hypertrophic scar. METHODS: SDS-PAGE (sodium dodecyl sulfate polyacrylamide gel electrophoresis) plus collagen substrate electrophoresis, amino acid analysis and compound staining were employed to observe the collagenase activity within hypertrophic scar, collagen degradation and the tissue morphology of the scar. RESULTS: There exhibited deranged collagen fibres within hypertrophic scar, and large amounts of acid mucopolysaccharide closely surrounded the collagen fibres. All these le

Treatment of scars: a review.

Hypertrophic scars and keloids occur as the result of an exaggerated wound healing response of the skin following injury. In addition to presenting a cosmetic concern, hypertrophic scars and keloids may be painful or pruritic and may restrict range of motion. There is no universally accepted treatment modality resulting in permanent hypertrophic or keloid scar ablation. Atrophic scars secondary to surgery, trauma, and common conditions such as acne vulgaris and varicella may also be disfiguring. This review discusses the etiology and clinical course of hypertrophic, keloid, and atrophic scars.

The use of laser monotherapy in the treatment of keloids and hypertrophic scars: A look at the carbon dioxide laser. A systematic review and meta-analysis.

To evaluate the effectiveness of fractional CO2 laser monotherapy in treating HS and keloids compared to other laser treatments. A systematic review and meta-analysis were conducted by searching Medline, PubMed, and Cochrane databases for studies comparing fractional CO2 laser therapy to other laser treatments for hypertrophic scars and keloids. Data were analyzed using IBM SPSS Statistics, assessing Vancouver Scar Scale scores, percent improvement in scar characteristics, and patient satisfaction. The search yielded 8,656 articles, with seven studies meeting the inclusion criteria. The role of positional information in determining dermal fibroblast diversity.

The largest mammalian organ, skin, consisting of a dermal connective tissue layer that underlies and supports the epidermis, acts as a protective barrier that excludes external pathogens and disseminates sensory signals emanating from the local microenvironment. Dermal connective tissue is comprised of a collagen-rich extracellular matrix (ECM) that is produced by connective tissue fibroblasts resident within the dermis. When wounded, a tissue repair program is induced whereby fibroblasts, in response to alterations in the microenvironment, produce new ECM components, resulting in the formatio

Hypertrophic Scars and Keloids: Advances in Treatment and Review of Established Therapies.

Hypertrophic scars and keloids can have significant detrimental effects on patients both psychosocially and functionally. A careful identification of patient risk factors and a comprehensive management plan are necessary to optimize outcomes. Patients with a history of dystrophic scarring should avoid unnecessary procedures and enhance the wound-healing process using various preventive strategies. As there is no single, fully efficacious treatment modality, prevention remains the best approach in reducing aberrant scar formation. When prevention therapies fail, keloids have been shown to be re

Laser Treatment of Hypertrophic Scar in a Porcine Model Induces Change to Epidermal Histoarchitecture That Correlates to Improved Epidermal Barrier Function.

Mechanisms and timing of hypertrophic scar (HTS) improvement with laser therapy are incompletely understood. Epidermal keratinocytes influence HTS through paracrine signaling, yet they are understudied compared to fibroblasts. It was hypothesized that fractional ablative CO2 laser scar revision (FLSR) would change the fibrotic histoarchitecture of the epidermis in HTS. Duroc pigs (n = 4 FLSR and n = 4 controls) were injured and allowed to form HTS. HTS and normal skin (NS) were assessed weekly by noninvasive skin probes measuring trans-epidermal water loss (TEWL) and biopsy collection. There w
