Evaluating an Algorithm and Clinical Prediction Rule for Diagnosis of Bone Stress Injuries.

BACKGROUND: A novel algorithm and clinical prediction rule (CPR), with 18 variables, was created in 2014. The CPR generated a bone stress injury (BSI) score, which was used to determine the necessity of imaging in suspected BSI. To date, there are no validated algorithms for imaging selection in patients with suspected BSI. HYPOTHESIS: A simplified CPR will assist clinicians with diagnosis and decision making in patients with suspected BSI. STUDY DESIGN: Prospective cohort study. LEVEL OF EVIDENCE: Level 3. METHODS: A total of 778 military trainees with lower extremity pain were enrolled.

Optimizing imaging in the pediatric trauma patient, part 1: head and neck trauma.

Trauma is the leading cause of death in the pediatric population and is among the most common reasons for ED visits by children. Imaging is an important tool for the diagnosis and management of pediatric trauma, but there are risks associated with exposure to ionizing radiation. In pediatric head and neck injuries, clinical findings and clinical decision tools can help inform selection of the most appropriate imaging modalities for the trauma patient, while also reducing unnecessary radiation exposure. This supplement reviews evidence-based recommendations for imaging decisions and interpretat

The providing multidisciplinary ILD diagnoses (PROMISE) study - study design of the national registry of Japan facilitating interactive online multidisciplinary discussion diagnosis.

BACKGROUND: Multidisciplinary discussion (MDD), in which physicians, radiologists, and pathologists communicate and diagnose together, has been reported to improve diagnostic accuracy compared to diagnoses made solely by physicians. However, even among experts, diagnostic concordance of MDD is not always good, and some patients may not receive a specific diagnosis due to insufficient findings. A provisional diagnosis based on the ontology with a diagnostic confidence level has recently been proposed. Additionally, we developed an artificial intelligence model to differentiate idiopathic pulmon

Lower limb acute joint injuries: when to refer for X-ray?

This article summarises the clinical decision tools available to help decide when acute lower limb joint injuries should be referred for plain film radiography. The Ottawa foot and ankle rules are a tool for deciding whether to refer a patient for X-ray or not. The rules are highly sensitive, gaining National Institute for Health and Care Excellence recommendation in the UK, although they have limited benefit in some patient groups, such as those with peripheral neuropathy. The Ottawa knee rules are highly sensitive but less specific than the Pittsburgh decision rules. Although the Pittsburgh

Improvement of MEFV gene variants classification to aid treatment decision making in familial Mediterranean fever.

OBJECTIVE: FMF is an inherited autoinflammatory syndrome caused by mutations in the MEFV gene. MEFV variants are still largely classified as acvariant of uncertain significance, or with unresolved classification, posing significant challenges in FMF diagnosis. Rare Exome Variant Ensemble Learner (REVEL) is a recently developed variant metapredictor tool. To reduce the number of MEFV variants with ambiguous classification, we extracted REVEL scores for all missense variants present in the INFEVERS database, and analysed its correlation with expert-based classification and localization in the ME

Constructing a Personalized Treatment Rule for Initial Therapy in Early Parkinson's Disease.

BACKGROUND: Dopaminergic therapies such as levodopa and dopamine receptor agonists (DRA) improve motor function in people with Parkinson's disease. These therapies are also linked to the advent of motor complications such as dyskinesias and wearing-off episodes. OBJECTIVES: We illustrate a method that creates a personalized treatment rule that takes patient-specific information and provides a recommended first-line therapy for Parkinson's disease that will provide the best mean improvement in motor function while constraining the probability of a motor complication within the first 2 years of

Polyp size of 1 cm is insufficient to discriminate neoplastic and non-neoplastic gallbladder polyps.

BACKGROUND: A significant proportion of gallbladder polyps are non-neoplastic, for which resection is not necessary. However, international guidelines advocate cholecystectomy for all polyps ≥ 1 cm. This study assessed a national cohort of histopathologically proven gallbladder polyps to distinguish neoplastic from non-neoplastic polyps. METHODS: PALGA, the nationwide network and registry of histo- and cytopathology, was searched to identify all histopathologically proven gallbladder polyps between 2003 and 2013. All polyps and (focal) wall thickenings > 5 mm were included, and classified as

Designing an explainable algorithm based on XGBoost and genetic algorithm for predicting hospitalization needs of COVID-19 patients.

Timely identification of COVID-19 outpatients who are at risk of hospitalization is critical for preventing clinical deterioration and optimizing healthcare resources. Although machine-learning models have demonstrated high predictive accuracy, their limited interpretability often hinders clinical adoption. This study aims to develop a hybrid explainable framework that combines the predictive strength of XGBoost with clinically interpretable rule-based explanations to support decision-making in real clinical settings. A retrospective dataset of 1278 COVID-19 patients was analyzed after applyin

Nuking the radiation: minimizing radiation exposure in the evaluation of pediatric blunt trauma.

PURPOSE OF REVIEW: Our objective is to highlight recent literature investigating low-radiation diagnostic strategies in the evaluation of pediatric trauma. RECENT FINDINGS: In the area of minor head injury, research has focused on implementation of validated clinical decision rules into practice to reduce unnecessary computed tomography scans. Clinical observation may also serve as an adjunct to initial assessment and a potential substitute for computed tomography imaging. Subgroups of children with special needs or severe injury mechanisms may also be safely characterized by the clinical dec

External Validation of the Post-Concussion Symptoms Rule for Predicting Mild Traumatic Brain Injury Outcome.

Persistent symptoms are common after a mild traumatic brain injury (mTBI). The Post-Concussion Symptoms (PoCS) Rule is a newly developed clinical decision rule for the prediction of persistent post-concussion symptoms (PPCS) 3 months after an mTBI. The PoCS Rule includes assessment of demographic and clinical characteristics and headache presence in the emergency department (ED), and follow-up assessment of symptoms at 7 days post-injury using two thresholds (lower/higher) for symptom scoring. We examined the PoCS Rule in an independent sample. We analyzed a clinical trial that recruited parti
