Anti-MDA5 antibody-positive patients with clinically amyopathic dermatomyositis (CADM) are at high risk of developing rapidly progressive interstitial lung disease (ILD), which is associated with a high mortality rate. Approximately half of the patients with ILD recover; however, the long-term clinical course of these patients has not been fully reported and is not completely understood. This report describes the atypical clinical course of an anti-MDA5 antibody-positive CADM patient who experienced three deteriorations of ILD in 9 years. These findings indicate that the ILD in anti-MDA5 antib

AIMS: Cystic fibrosis (CF) is associated with the emergence of CF-related diabetes (CFRD). CFRD is associated with increased risk of accelerated weight and/or lung function loss (clinical degradation). Data in the CF pediatric population reported an association between higher 60-min oral glucose tolerance test (OGTT) plasma glucose values and reduced lung function. Our objective was to evaluate the relationship between the 60-min OGTT insulin and glucose values and markers of clinical degradation in adult patients with CF.
METHODS: This study was based on an ongoing observational cohort of CF

A 71-year-old man underwent pleural biopsy due to left pleural effusion and pleural thickening in August, 2001. An inflammatory pseudotumor (IPT) was diagnosed, and therefore systemic oral steroid therapy (prednisolone [PSL] 30 mg/day) was initiated. However, after tapering PSL to 7.5 mg/day, a complication of secondary central diabetes insipidus due to hypophysitis developed in 2008. As his pulmonary condition deteriorated over time and he began to experience exertional dyspnea, he was admitted to our hospital for re-evaluation of the disease in October, 2010. INTRODUCTION: This study aims to evaluate the clinical and imaging risk factors for early neurological deterioration (END) and long-term neurological disability in patients with Single subcortical small infarction (SSSI).
METHODS: We retrospectively included SSSI patients hospitalized. Outcomes were defined as modified Rankin Scale (mRS) score > 2 at follow-up and the occurrence of END during hospitalization. Multivariate logistic regression identified independent predictors of END and long-term outcomes. Stepwise regression analysis was used to develop a predictive model for poor outcomes. T

BACKGROUND: The COVID-19 pandemic has raised concerns about respiratory sequelae, particularly in the patients with preexisting interstitial lung disease (ILD). This study aimed to evaluate the longitudinal impacts of SARS-CoV-2 reinfection on patients with preexisting ILD, focusing on differential outcomes between single and multiple infection episodes.
METHODS: A prospective cohort study was conducted (December 2022-March 2023), enrolling 347 adults with preexisting ILD and confirmed COVID-19 infection. Follow-up assessments were performed at baseline, 6-, 12-, and 24-month intervals. Data

BACKGROUND: The Cambridge Pulmonary Hypertension Outcome Review (CAMPHOR) is a disease-specific assessment tool used for the evaluation and follow-up of patients with pulmonary hypertension (PH). We describe a novel use for this questionnaire in its potential to predict clinical deterioration (CD) in two patient cohorts with subtypes of PH, idiopathic pulmonary arterial hypertension (IPAH), and chronic thromboembolic pulmonary hypertension (CTEPH) during an 8-year period.
METHODS: We retrospectively analyzed CAMPHOR scores obtained at baseline and at follow-up visits in patients under the car

Systemic inflammation and the host immune responses associated with certain viral infections may accelerate the rate of neurodegeneration in patients with Creutzfeldt-Jakob disease (CJD), a rare, transmissible neurodegenerative disease. However, the effects of the newly emerged SARS-CoV-2 infection on the pathogenesis of CJD are unknown. In this study, we describe the case of an elderly female patient with sporadic CJD that exhibited clinical deterioration with the emergence of seizures and radiological neurodegenerative progression following an infection with SARS-CoV-2 and severe COVID-19. Anticipating the evolution of septic patients with community-acquired pneumonia (CAP) is challenging for front-line physicians in the Emergency Department (ED). Prognosis depends mainly on early identification, antibiotics, organ support, but also immune status. The objective of this proof-of-concept study was to perform a cluster analysis to investigate whether specific phenotypes, including cellular immunology parameters, are associated with the prognosis in patients with CAP presenting to the ED. We performed an exploratory study in the ED of Limoges university hospital (France) on patients

BACKGROUND: All newborns are at risk of deterioration as a result of failing to make the transition to extra uterine life. Signs of deterioration can be subtle and easily missed. It has been postulated that the use of an Early Warning Tool may assist clinicians in recognising and responding to signs of deterioration earlier in neonates, thereby preventing a serious adverse event.
OBJECTIVE: To examine whether observations from a Standard Observation Tool, applied to three neonatal Early Warning Tools, would hypothetically trigger an escalation of care more frequently than actual escalation of

Vancomycin-resistant enterococcal (VRE) infection is a growing threat. We studied the incidence, risk factors, and clinical course of early-onset VRE bacteremia in allogeneic hematopoietic stem cell transplant recipients. We carried out a chart review of 281 allogeneic hematopoietic stem cell transplant recipients from 1997-2003, including preparative regimen, diagnosis, status of disease, graft-versus-host disease prophylaxis, antimicrobial therapy, and survival. VRE bacteremia developed in 12/281 (4.3%) recipients; 10 (3.6%) were within 21 days of transplant. Diagnoses were acute leukemia (7

Scrub typhus causes substantial morbidity and mortality in children across South Asia, yet admission-time predictors of critical deterioration remain poorly characterized in the paediatric population. This study aimed to identify independent predictors of critical deterioration in hospitalized children with confirmed scrub typhus. A retrospective observational cohort study was conducted at a tertiary-care teaching hospital in South India over 6 years. Children aged 1-14 years with immunoglobulin M enzyme-linked immunosorbent assay-confirmed scrub typhus were included. Critical deterioration, a

BACKGROUND: Alkaline phosphatase (ALP) has been implicated to be associated with poor outcome in ischemic stroke patients, yet its role in aneurysmal subarachnoid hemorrhage (aSAH) patients is unknown. The current study aimed to investigate the on-admission and short-term variation trend of ALP levels in aSAH patients as well as its associations with vasospasm, delayed cerebral ischemia (DCI), and outcome after aSAH.
METHODS: Between January 2014 and May 2018, all consecutive aSAH patients were prospectively enrolled. Blood samples from patients and 78 healthy individuals were obtained. Basel

OBJECTIVE: Oral cancer treatment frequently results in postoperative oral dysfunction, which may contribute to dysphagia, malnutrition, and impaired quality of life. Although objective and patient-reported oral/swallowing function measures are used in clinical practice, the minimal clinically important differences (MCIDs) corresponding to patient-perceived deterioration remain unclear. This study aimed to determine the MCIDs of objective and patient-reported oral/swallowing function measures associated with the oral health-related quality of life in patients with oral cancer.

Necrotizing fasciitis (NF) is a life-threatening disease with high mortality and rapidly progressive clinical manifestations. Early detection and surgical management coupled with antibiotic treatment are crucial for the survival, and the patient survival is heavily dependent on clinical decisions. However, it is not widely known that NF does not always follow a typical clinical course, and there have been no case reports of NF following an atypical clinical course. Although the course of the disease depends on the individual patient, it remains a challenge for physicians to determine the preci

BACKGROUND: Timely recognition of the deteriorating inpatient remains challenging. Wearable monitoring systems (WMS) may augment current monitoring practices. However, there are many barriers to implementation in the hospital environment, and evidence describing the clinical impact of WMS on deterioration detection and patient outcome remains unclear.
OBJECTIVE: To assess the impact of vital-sign monitoring on detection of deterioration and related clinical outcomes in hospitalised patients using WMS, in comparison with standard care.
METHODS: A systematic search was conducted in August 2020
