Continuous assurance for AI-driven clinical decision support systems.

Healthcare systems are rapidly embedding adaptive and generative AI into core clinical processes. The integration of Artificial Intelligence into Clinical Decision Support Systems (AI-CDSS) highlights a fundamental transformation within healthcare delivery. This transformation enables advanced predictive analytics, multimodal data integration, and real-time augmentation of clinical decisions. However, AI introduces systemic, ethical, operational, and governance risks that challenge traditional healthcare audit and assurance frameworks. In fact, assurance methodologies designed for static softw

When clinician-rated measures reflect the assessment: HoNOS, clinical judgement, and organisational risk.

ObjectivesTo examine the clinical governance implications of Health of the Nation Outcome Scales (HoNOS) rating quality in Australian mental health services. The paper argues that HoNOS occupies a distinctive position among clinician-rated measures because it requires clinicians to translate the overall clinical assessment into structured ratings across domains of behaviour, symptoms, impairment, and social functioning. It considers why poor-quality HoNOS ratings should not be treated as a narrow technical or training problem, but as a signal of potential risk in assessment practice, documenta

Should the geriatrician host the party? Person-centred hospital care in ageing societies requires placing geriatrics at the centre of care.

As populations age, multimorbidity and frailty increasingly characterize hospital admissions, exposing the limits of traditional disease-specific and organ-centred models of care. Older adults with complex care needs face higher risks of hospitalization, readmission, functional decline, and fragmented care pathways, while hospitals remain insufficiently aligned with these realities. Despite strong evidence for the clinical and economic benefits of Comprehensive Geriatric Assessment (CGA) and geriatric-led care, geriatrics often remains peripheral to everyday clinical practice and hospital gove

The double-edged role of point-of-care testing in modern medicine.

Point-of-care testing (POCT) has emerged as a transformative force in clinical diagnostics, enabling rapid, near-patient testing that can expedite decision-making, improve patient outcomes, and expand access to care. This review critically examines the double-edged nature of POCT, balancing its clear clinical and operational advantages against persistent challenges of accuracy, quality assurance, governance, and cost-effectiveness. The scope of POCT technologies ranges from simple lateral-flow assays to advanced molecular platforms, increasingly integrated with digital health ecosystems. Their

Strategic governance of integrated nuclear medicine, diagnostic radiology, and radiation therapy in comprehensive cancer centres: A balanced scorecard framework.

Comprehensive cancer centres depend on nuclear medicine, diagnostic radiology and radiation therapy for diagnosis, staging, treatment planning, image guidance, treatment delivery, response assessment and follow-up. Although these services are clinically interdependent, they are often governed through separate departmental structures, budgets, information systems, quality routines and performance reports. This fragmentation can limit the ability of cancer-centre leaders to balance access, clinical quality, patient safety, workforce capacity, equipment sustainability, innovation and data steward

From spillover to preparedness: a One Health analysis of knowledge gaps in Nipah virus surveillance, prevention, and health system response.

BACKGROUND: Nipah virus (NiV) is a highly pathogenic zoonotic henipavirus that continues to pose a significant threat to global health security because of its high case fatality rate, ability to cause human-to-human transmission, and absence of licensed vaccines or specific therapeutics. Although more than two decades of research have advanced the understanding of NiV ecology, epidemiology, and clinical disease, major uncertainties remain regarding spillover mechanisms, environmental drivers, surveillance integration, and outbreak preparedness. These persistent knowledge gaps limit the ability

Why Luhmann? Social systems theory for a sociology of suicide in the platform age.

Suicide becomes a sociological object through the communications that make it observable, recordable, reportable, and governable. This Perspective draws on Luhmann's social systems theory to analyse how suicide-related meanings are selected and stabilised across functionally differentiated subsystems. The argument treats suicide as a limit-case perturbation that is processed differently by health care, law, education, media, and platform governance. Clinical research remains indispensable for understanding vulnerability and intervention; the sociological question concerns how those realities a

Governing AI for Pharmacovigilance in Low-Income Countries: Systems Perspective.

Gaps in pharmaceutical governance could widen with the adoption of AI, even as AI promises better pharmacovigilance in low-income countries (LICs). While advanced regulatory systems like Australia's are integrating AI into pharmaceutical governance, LICs with underdeveloped regulatory capabilities, such as South Sudan, lag behind. The potential divergence disorients the World Health Organization's "Medicine Without Harm" agenda and effective global pharmacovigilance. Moreover, evolving global governance initiatives, including the newly established United Nations scientific panel on AI, may be

[Japanese sentinel project and contribution of laboratory medicine].

Ordinary passive surveillance (generally spontaneous reporting) of adverse effects of medical products is not enough to manage medical risks/safety. The Japanese government, following the "sentinel initiative" by the US government, started the "Japanese sentinel project" as an active surveillance of medical products with the Pharmaceuticals and Medical Devices Agency(PMDA). Using entire prescription data in the hospital information system, data on laboratory examinations, diagnosis, and therapy specific to an adverse effect are checked by detecting differences between before and after prescrip
