Coccidioides immitis. A rare cause of pulmonary mycetoma.

Coccidioides immitis occurring as a fungus ball within a chronic pulmonary cavity has previously been reported twice and must be rare. We report a mycetoma composed of both spherule and mycelial forms of Coccidioides immitis, proved by culture and histologic sections. Fungus ball due to Coccidioides immitis is a logical progression from mycelia seen in the cavity wall in the active pulmonary coccidioidal lung cavity.

PRIMARY PULMONARY COCCIDIOIDOMYCOSIS : EXPERIMENTAL INFECTION WITH COCCIDIOIDES IMMITIS.

1. The literature concerning inhalation infection with Coccidioides immitis is briefly reviewed. 2. A laboratory technique for exposing animals to inhalation of the spores of Coccidioides immitis is described. 3. Primary pulmonary infection was produced in 42 per cent of 72 animals exposed by this method. 4. No marked weight loss was apparent in the animals infected. 5. Skin tests with coccidioidin were not entirely satisfactory in diagnosis. Positive skin tests with coccidioidin were obtained in many of the infected animals, but inconsistencies occurred. 6. The gross and microscopic lesions a

Development of a rapid, cost-effective TaqMan Real-Time PCR Assay for identification and differentiation of Coccidioides immitis and Coccidioides posadasii.

Coccidioidomycosis is an infection caused by Coccidioides immitis or C. posadasii. We developed a TaqMan real-time PCR assay that rapidly and accurately differentiates the species. This assay can be used as a tool to improve disease surveillance, increase understanding of the natural history of the infection, and assist in clinical differentiation studies.

Novel approach to designing primers for identification and distinction of the human pathogenic fungi Coccidioides immitis and Coccidioides posadasii by PCR amplification.

We developed a pair of primers that specifically identifies Coccidioides species, etiologic agents of the human fungal disease coccidioidomycosis. These primers could be used for distinguishing Coccidioides immitis and Coccidioides posadasii by simply comparing the amplicon sizes on an agarose gel.

Stability of hybridization activity of Coccidioides immitis in live and heat-killed frozen cultures tested by AccuProbe Coccidioides immitis culture identification test.

Frozen hyphal suspensions of Coccidioides immitis were evaluated for suitability as positive control cultures in the AccuProbe C. immitis culture identification test. The genetic probe hybridization activity of heat-killed and viable frozen cultures, stored at -20 and -70 degrees C and tested over a 10-month period, was compared to that of a freshly grown culture, and the results were evaluated based upon the manufacturer's established positive and negative photometric light unit (PLU) cutoff values. All C. immitis suspensions produced positive hybridization values well above the positive and

Drug Interactions for Inhibition or Killing of Coccidioides Species.

Coccidioidomycosis is of concern because of its rising incidence, the spread of its endemicity, the virulence of its progressive infections, and the difficulty of achieving successful therapy, particularly durable responses. As a result, clinicians will often attempt combination therapy with amphotericin B and an azole. However, the effects of such combinations on the inhibition or killing of the fungus are largely unknown. The present report details studies of these drug interactions against Coccidioides in vitro with checkerboard arrangements, with 239 interactions for inhibition and 76 for

Drug Discovery and Repurposing for Coccidioides: A Systematic Review.

Coccidioides immitis and C. posadasii are the causative agents of coccidioidomycosis (CM) or Valley Fever, endemic to the alkaline deserts of North and South America. Clinical treatment of CM is predominantly limited to the triazole and polyene drug classes. There are limited therapeutic options for the treatment of CM, most commonly requiring prolonged courses of therapy with established antifungal agents such as azoles and Amphotericin B, which often lead to toxicity and drug resistance. Clearly, there is a need to develop novel and better antifungal drugs against CM. This review examines bo

Was Coccidioides a Pre-Columbian Hitchhiker to Southcentral Washington?

Coccidioides immitis, a pathogenic environmental fungus that causes Valley fever (coccidioidomycosis) primarily in the American Southwest and parts of Central and South America, has emerged over the past 12 years in the Columbia River Basin region, near the confluence with the Yakima River, in southcentral Washington state, USA. An initial autochthonous Washington human case was found in 2010, stemming from a wound derived from soil contamination during an all-terrain vehicle crash. Subsequent analysis identified multiple positive soil samples from the park where the crash occurred (near the C

Asymptomatic Coccidioidal Meningitis Relapse: A Demon in Disguise.

Coccidioides spp is a soil-dwelling, dimorphic fungus that causes coccidioidomycosis. It is endemic to the western hemisphere. Although primarily a respiratory disease, it can also cause a myriad of clinical manifestations, from asymptomatic disease to meningitis. In fact, Coccidioides species is probably the most common etiologic agent of long-term meningitis in California and Arizona. Early diagnosis and treatment are critical to avoid fatal complications. With treatment, the cerebral spinal fluid analysis may return to normal. Relapse of coccidioidal meningitis is usually suspected with rec

Patient With a 42-Year History of Coccidioidal Meningitis.

This is a case of a 56-year-old man diagnosed with coccidioidal meningitis 42 years ago at the age of 14. He was treated with intrathecal amphotericin B deoxycholate by cisternal puncture for 15 years before switching to fluconazole once it became available in 1991. Over 42 years of treatment, he developed hearing loss due to auditory nerve neurotoxicity, hydrocephalus requiring ventriculoperitoneal shunting with associated malfunctions, lumbar arachnoiditis, and hypokalemic paralysis. Regular cerebrospinal fluid studies to this day do not show disease clearance. Many of the lessons from his c

Adjunctive Corticosteroid Therapy in the Treatment of Coccidioidal Meningitis.

Coccidioidal meningitis (CM) has high morbidity, and adjunctive measures to improve outcomes are needed. Using an established multicenter retrospective cohort study of CM (N = 221), we found that patients receiving adjunctive corticosteroids had a significant reduction in secondary cerebrovascular events (P = .0049). Those with CM-associated cerebrovascular events (8%) may benefit from short-term corticosteroids.

Coccidioides parenchymal cerebral abscess in the setting of lymphoma.

Coccidioides immitis is a dimorphic fungus endemic to southwestern United States of America. When symptomatic, infection usually results in a subacute respiratory infection. Disseminated coccidioidomycosis occurs in less than 1% of all cases. We report a patient with follicular lymphoma and recent travel to Arizona, who underwent resection of a cerebral Coccidioides abscess. Serology testing was negative. This case highlights the importance of clinical suspicion in patients with neurologic symptoms and travel to an endemic location.

Genomics in Coccidioides: insights into evolution, ecology, and pathogenesis.

Coccidioides immitis and C. posadasii, the causative agents of the mammalian disease coccidioidomycosis, are dimorphic fungal pathogens distributed throughout desert-like environments in North and South America. Coccidioides spp. are members of the Onygenales, a diverse group of pathogenic and nonpathogenic fungi. Recently, full genomes have been published for Coccidioides and a number of other Onygenales species. Phylogenomic comparisons and additional studies in Coccidioides population genomics and gene expression have shed light on the ecology and pathogenesis of Coccidioides and the other

Evaluation of Coccidioides antigen detection in dogs with coccidioidomycosis.

Antigen detection has been reported to be a promising method for rapid diagnosis of coccidioidomycosis in humans. Coccidioides antigen detection has not been previously reported in dogs with coccidioidomycosis and was evaluated in 60 cases diagnosed based on detection of anti-Coccidioides antibodies at titers of 1:16 or more in serum. Controls included dogs with presumed histoplasmosis or blastomycosis, other fungal infections, or nonfungal diseases and healthy dogs. Urine and serum specimens were tested using an enzyme immunoassay for Coccidioides galactomannan antigen. Antibody testing was p

[The morphologic characteristics of microscopic fungi of genus coccidioides].

The morphologic characteristics of microscopic fungi of genus Coccidioides under cultivation in nutrient mediums are studied. It is demonstrated that filamentous form of agents of coccidioidomycosis is characterized by significant polymorphism of macro- and micromorphologic signs on different stages of development ofagar culture. But C. immitis and C. posadasii have no species' differences. The dynamics of development of coccidioidomycosis strains in microcultures is analyzed as well as the intensity of sporification. The forms and sizes of arthroconidiae are also established.

Successful kidney transplantation after coccidioidal meningitis.

Coccidioidomycosis is a fungal infection primarily found in residents or visitors to geographic areas where Coccidioides species are endemic, including the southwestern United States, northwestern Mexico, and certain areas of Central and South America. The infection rarely disseminates, but certain populations are at higher risk of dissemination. One population at high risk of disseminated disease is solid organ transplant recipients. At our transplant center in Arizona, patients with proven coccidioidal infection before transplantation undergo thorough counseling about the risks of disseminat

To identify demographics, clinical manifestations, and outcomes of patients with Coccidioides fungemia, we searched our institutional medical records to identify patients with Coccidioides fungemia treated between 1998 and 2008 and conducted a comprehensive search of the medical literature to identify previously reported cases. Coccidioides fungemia is an uncommon manifestation of coccidioidomycosis, a fungal infection caused by Coccidioides sp. endemic to the southwestern United States. Six Coccidioides fungemia patients were treated at our institution during the 10-year period. All 6 had und

While the whole killed spherule vaccine, protective in mice and monkeys, did not prevent coccidioidal disease in humans, the 27K vaccine, a soluble derivative, retains protective activity in mice with little irritant action. Gel filtration and anion exchange fractions of thimerosal-inactivated spherules (T27K), when administered with alum adjuvant, also protect mice against lethal respiratory coccidioidal challenge. However, the superb protection afforded by T27K antigens is maintained for some 3 months, but may then diminish. This appears unrelated to the aging of the mice. Prolongation of th
