[Imaging in cochlear implant patients].

Imaging procedures are a mainstream tool in the daily ENT workflow. Cochlear Implant patients are representing a special population with specific demands for imaging. There are different imaging techniques available for pre-operative evaluation, surgery and postoperative controls with different indications and consequences. High-resolution computed tomography and magnetic resonance imaging are mainly used in the evaluation process. New procedures, as digital volume tomography, are increasingly used intra- and postoperatively. Especially the intracochlear positioning in malformations of the inn

Cochlear implant in Cogan's syndrome.

Post-lingual deafness is a stressful condition which is rendered even more painful by the sudden emotional isolation that the patient suffers. Cogan's syndrome is a rare autoimmune cause for post-lingual deafness characterized by non-syphilitic interstitial keratitis, bilateral audio vestibular deficiencies and systemic vasculitis. World over very few cases of Cogan's syndrome have been reported. Cochlear implant surgery in such a patient is a challenging but highly satisfactory experience due to the multitude of clinical problems the patient faces. This demands a proper work up, meticulous su

Cochlear implants in Waardenburg syndrome.

OBJECTIVE: Waardenburg syndrome is an autosomal-dominant syndrome characterized by dystopia canthorum, hyperplasia of the eyebrows, heterochromia irides, a white forelock, and sensorineural hearing loss in 20% to 55% of patients. This patient population accounts for approximately 2% of congenitally deaf children. The purpose of this retrospective case review was to describe the outcomes for those children with Waardenburg syndrome who have undergone cochlear implantation. METHODS: Pediatric cochlear implant recipients with documented evidence of Waardenburg syndrome underwent retrospective ca

Malformations in cochlear implant patients.

OBJECTIVE: To report on cochlear implantation in children with bony inner ear malformations. PATIENTS: 30 children with bony inner ear malformations who have received cochlear implants. INTERVENTIONS: High-resolution spiral computed tomography is used to identify malformations. Magnetic resonance imaging is used to detect the presence of an acoustic nerve and determine the integrity of the auditory pathway and central nervous system structures. Both imaging techniques may be used intraoperatively, as well as facial nerve monitoring and electrical auditory brainstem response monitoring. Three

Cochlear implant mechanical failures.

Cochlear implants have proven to be an effective treatment for profoundly deafened individuals. Unfortunately, like most mechanical devices, these implants occasionally cease to function. The rate at which the cochlear implant fails, however, does not appear to be the same in adults and children. The failure rate for children far exceeds that observed in adults. The overall failure rate reported by Cochlear Corporation notes that whereas only 3% of the adults have had this type of problem, 9% of the children have had failed internal receivers. This research reports on the experiences of a larg

[Experiences in Kiel with the cochlear implant].

Since 1988 22 deaf patients were provided with a 22-channel cochlear implant at the Kiel University Hospital. No surgical or postoperative complications were seen in any of the patients. Side effects like facial stimulation and pain sensation during electrode activation do not reduce the utility. Tinnitus may initially be severe but usually subsides to preop levels after a few days. A tinnitus masking effect is pronounced during stimulation but hardly lasts long after switch off. Besides an individually arranged hearing training the accurate speech processor fitting to the needs of the patient

Cochlear implants for congenital deformities.

There have been few accounts of multi-channel cochlear implants in patients with congenital structural deformities of the inner ear which are associated with severe and sometimes progressive deafness. These malformations can now be recognized easily on 2 plane thin section high resolution CT studies which are mandatory for the pre-implantation assessment. However, no attempt seems to have been made to describe which of these malformations would be suitable for an implant or for which would this procedure be contra-indicated. True Mondini deformity of both the cochlea and dilated vestibular aqu

Radiographic imaging for the cochlear implant.

Imaging plays an essential role in evaluating the cochlear implant candidate and the results of implantation. Eighteen candidates for cochlear implantation were reviewed for this study. Fourteen received implants; all but one, intracochlear devices. Based on the trial use of multiple techniques, a protocol has been established for the use of radiography in implant studies. The protocol suggests preoperative high resolution semiaxial computed tomographic scanning, intraoperative anteroposterior transorbital plain films if there is doubt concerning the electrode placement, and postoperative plai

Labyrinthine ossificans and cochlear implants.

We report two clinical and five histopathological examples of labyrinthitis ossificans and discuss the possible cause of this condition. Diagnosis can be established by polytomography. The advent of the cochlear implant makes ossification of the cochlea (labyrinthitis ossificans) of more than academic interest, since it requires alteration of the insertion of an electrode into the scala tympani and minimizes the chances of successful stimulation. Hearing losses due to meningitis, advanced cochlear otosclerosis, and trauma are likely to be due to cochlear ossification. Therefore, it is importan

European consensus statements on pediatric cochlear implantation.

Existing European national guidelines agree to provide cochlear implantation to children with bilateral severe to profound hearing loss and to offer bilateral treatment depending on the hearing status of the best hearing ear and progression of hearing loss, and in the case of congenital hearing loss to do this at the youngest possible age. However, studies and reports demonstrate substantial and persistent disparities in the provision of pediatric cochlear implantation across Europe. Variations in national policies, service quality, and access to timely diagnosis and intervention continue to a

Cross-modal functional plasticity after cochlear implantation.

Despite evidence that cross-modal effects after hearing loss and cochlear implantation are primarily driven by synaptic gain and efficacy, few studies have evaluated cross-modal functional connectivity (CMFC) to assess plasticity. This study, inspired by the psychophysiological interaction (PPI) method, addresses its limitations and provides a robust approach for assessing task-induced CMFC. Twenty-three postlingually deafened cochlear implant (CI) recipients and 17 normal-hearing (NH) participants took part in the study. Functional near-infrared spectroscopy was used to measure brain activity

Bilateral cochlear implantation in infantile Bartter's syndrome.

Bartter's syndrome (BS) is a rare group of hereditary salt losing tubulopathies due to impairment of renal transport mechanism. Herein, we report a boy in early childhood who was diagnosed to have infantile Bartter's syndrome. Laboratory investigations revealed hypokalaemia and metabolic alkalosis which was managed with oral potassium chloride supplementation. The inner ear showed no anatomical abnormalities. Sequential cochlear implantation was performed 4 months apart. Postoperative electrical stimulation yielded good response and a symmetric mapping. The patient was on regular audio-verbal

Residual hearing preservation for cochlear implantation surgery.

Cochlear implantation (CI) has developed for more than four decades. Initially, CI was used for profound bilateral hearing impairment. However, the indications for CI have expanded in recent years to include children with symptomatic partial deafness. Therefore, CI strategies to preserve residual hearing are important for both patients and otologists. The loss of residual low-frequency hearing is thought to be the result of many factors. All surgical methods have the same goal: protect the delicate intracochlear structures and preserve residual low-frequency hearing to improve speech perceptio
