Colistin resistance gene mcr-1 in gut flora of children.

The aim of this study was to determine the prevalence and transmission mechanism(s) of mcr-1 in the gut flora of children. Faecal samples (n = 173) were obtained from non-diarrhoea patients at the Children's Hospital of Zhejiang University (Hangzhou, China). PCR-based analysis indicated that 17 isolates from 9.8% of the samples were positive for mcr-1, comprising 16 Escherichia coli and 1 Citrobacter freundii. Nine mcr-1-bearing isolates co-expressed extended-spectrum β-lactamase (ESBL) genes, but plasmid-mediated quinolone resistance (PMQR) genes were not detected. Transconjugation followed b

Differential membrane lipid disruption by lipopeptide antibiotics, colistin and turnercyclamycins.

Lipopeptide natural products are essential agents against multidrug-resistant bacteria, but their clinical utility is often constrained by toxicity and resistance. Here, we compare the mechanisms of action of two superficially similar lipopeptide antibiotics: colistin, a last-line treatment for Gram-negative infections, and turnercyclamycins, a new class active against certain colistin-resistant strains. Both antibiotics require lipopolysaccharide (LPS) biosynthesis, even when LPS transport to the outer membrane (OM) is impaired. Colistin rapidly disrupts both the OM and the cytoplasmic membra

Colistin Resistance in Klebsiella pneumoniae: Mechanisms, Detection, and Clinical Impact.

Klebsiella pneumoniae (K. pneumoniae ) is a Gram-negative, encapsulated opportunistic pathogen causing pneumonia, urinary tract infections, bloodstream infections, and ventilator-associated pneumonia. Carbapenem resistance has made colistin a last-resort antibiotic for multidrug-resistant Gram-negative infections, but increased colistin use has driven resistance emergence. Resistance occurs through chromosomal mutations (mgrB, phoPQ, pmrAB) causing lipid A modification that reduces colistin binding, and plasmid-mediated mcr genes (especially mcr-1) encoding phosphoethanolamine transferase

Sorafenib analogues exhibit dual Colistin-potentiating and anti-MRSA activity.

In response to the antimicrobial resistance (AMR) global health crisis, physicians have been forced to employ antibiotics of last resort such as colistin, which had previously been removed from clinics due to toxicity concerns. The increase in colistin usage has in turn resulted in the increased incidence of colistin-resistant isolates, making novel therapeutic options for infections caused by these pathogens a priority. We previously reported that sorafenib, a eukaryotic kinase inhibitor, is a colistin adjuvant, augmenting colistin activity against colistin-resistant Klebsiella pneumoniae and

Disruption of rcnB modulates colistin susceptibility in Acinetobacter baumannii AB5075.

Acinetobacter baumannii AB5075 is a clinically relevant multidrug-resistant (MDR) isolate that poses a major therapeutic challenge. Although colistin has been reinstated as a last-resort antibiotic against MDR Gram-negative infections, the rapid emergence of colistin resistance threatens its clinical utility. Here, we employed a CRISPR-Cas9-based genome editing system to generate an A. baumannii AB5075 ΔrcnB mutant and uncovered a previously underappreciated role of rcnB in modulating colistin susceptibility. Loss of rcnB markedly potentiated colistin-mediated killing through multiple associat

Natural flavonoids disrupt bacterial iron homeostasis to potentiate colistin efficacy.

In the face of the alarming rise in global antimicrobial resistance, only a handful of novel antibiotics have been developed in recent decades, necessitating innovations in therapeutic strategies to fill the void of antibiotic discovery. Here, we established a screening platform mimicking the host milieu to select antibiotic adjuvants and found three catechol-type flavonoids-7,8-dihydroxyflavone, myricetin, and luteolin-prominently potentiating the efficacy of colistin. Further mechanistic analysis demonstrated that these flavonoids are able to disrupt bacterial iron homeostasis through conver

Colistin Loading Dose in Septic Patients with Gram Negative Infections.

PURPOSE: Intravenous (IV) colistin is commonly used to treat multidrug-resistant gram-negative infections. It is primarily eliminated renally and may induce acute kidney injury (AKI) at a rate of up to 53%. Consequently, septic patients who require colistin administration have an additional risk of developing AKI. The aim of this study is to investigate clinical failure and AKI predictors for septic patients treated with IV colistin. METHODS: This retrospective cohort study was conducted at a tertiary teaching hospital in Saudi Arabia. Adult septic patients with suspected or confirmed gram-ne

Rapid Detection of MCR-Mediated Colistin Resistance in Escherichia coli.

Colistin is one of the last-resort antibiotics for infections caused by multidrug-resistant Gram-negative bacteria. However, the wide spread of novel plasmid-carrying colistin resistance genes mcr-1 and its variants substantially compromise colistin's therapeutic effectiveness and pose a severe danger to public health. To detect colistin-resistant microorganisms induced by mcr genes, rapid and reliable antibiotic susceptibility testing (AST) is imminently needed. In this study, we identified an RNA-based AST (RBAST) to discriminate between colistin-susceptible and mcr-1-mediated colistin-resis

Putative mobilized colistin resistance genes in the human gut microbiome.

BACKGROUND: The high incidence of bacterial genes that confer resistance to last-resort antibiotics, such as colistin, caused by mobilized colistin resistance (mcr) genes, poses an unprecedented threat to human health. Understanding the spread, evolution, and distribution of such genes among human populations will help in the development of strategies to diminish their occurrence. To tackle this problem, we investigated the distribution and prevalence of potential mcr genes in the human gut microbiome using a set of bioinformatics tools to screen the Unified Human Gastrointestinal Genome (UHGG

Colistin Nephrotoxicity-Age and Baseline kidney Functions Hold the Key.

INTRODUCTION: With the emergence of multidrug-resistant gram-negative bacterial infections, there has been a surge in the use of Colistin in recent times. The most important side effect of Colistin use is its nephrotoxicity. The study was designed to assess the effect on kidney function and the risk factors for nephrotoxicity in patients treated with Colistin. METHODS: The study is a retrospective one, which included patients who received Colistin for more than 48 hours. The estimated glomerular filtration rate (eGFR) was calculated using the Modification of Diet in Renal Disease (MDRD) four-

Intraventricular CNS treatment with Colistin-Tigecycline combination: A case series.

"Healthcare-associated ventriculitis and meningitis" is a potentially devastating illness following neurosurgical procedures. Multidrug resistant (MDR) and extensively drug resistant (XDR) organisms such as Acinetobacter baumannii and Klebsiella pneumoniae have increasingly been isolated in ventriculitis and meningitis episodes. The treatment of these infections can be challenging, as the antimicrobial options are restricted. Regarding Central Nervous System (CNS) infections the transfer of the antibiotics to the Cerebrospinal Fluid (CSF) is often low which results in decreased drug levels at

Perspectives on Colistin-An Old Antibiotic with Renewed Relevance in Modern Medicine.

Antibiotic resistance among bacteria represents a major challenge in modern medicine. The absence of antibiotics effective against multidrug-resistant pathogens triggers interest in reviving older antibiotics. This review aims to provide a focused and updated perspective on the reintroduction of polymyxin antibiotics, with a particular emphasis on colistin, a cyclic oligopeptide initially used in the 1950s and 1960s. We analyze colistin from multiple perspectives, including (i) its historical and contemporary clinical applications, (ii) pharmacokinetic and pharmacodynamic properties, and (iii)
