Transanal surgery: A tool in the colorectal anastomotic leakage.

The treatment of anastomotic leakage after oncological surgery for rectal cancer is a surgical challenge. The goal of this study is to show how transanal surgery combined with the abdominal approach is a very useful tool to decide on individualized treatment depending on the degree of dehiscence and to assist us in its local management. We present three cases of patients with colorectal anastomotic dehiscence. In two, we demonstrate the treatment of acute colorectal leakage and how transanal surgery allows us to confirm its viability and rule out any underlying ischemia. Furthermore, it facili

[Laparoscopic colorectal surgery is no longer an ugly duckling].

Laparoscopic surgery for colorectal cancer in Sweden has previously been performed infrequently but is presently increasing, and in 2014 more than one fourth of all patients with colorectal cancer underwent laparoscopic resection. Regional differences are decreasing but substantial variation persists with regard to individual hospitals and the use of laparoscopy. All Swedish patients undergoing resection for cancer in the ascending and sigmoid colon between 2012-2014 (n=8269; laparoscopic resection 17%) were assessed with regard to patient demography and short-term outcome. The present investi

Chylous ascites as a complication of laparoscopic colorectal surgery.

INTRODUCTION: Until now, no data have been available on the incidence of chylous ascites after laparoscopic colorectal surgery. METHODS: From January 2007 to December 2011, 137 patients with colorectal cancer underwent laparoscopic surgery at our institution. The clinical data of the patients with postoperative chylous ascites were compared with those of the patients without chylous ascites. RESULTS: Chylous ascites developed in 9 of the 137 patients (6.5%). Blood loss, operative time, tumor location, and extent of lymph node dissection were not associated with postoperative chylous ascites.

The current status of laparoscopic surgery for colorectal disease.

Laparoscopic surgery has an expanding role in the management of colorectal disease. As technical expertise has increased, selection of patients for laparoscopic procedures has widened without any commensurate increase in operative or postoperative complications. This article aims to summarise the current status of laparoscopic surgery in colorectal disease with particular reference to colorectal cancer, inflammatory bowel disease, diverticular disease and disorders of the pelvic floor.

What is the definition of "conversion" in laparoscopic colorectal surgery?

BACKGROUND: A web-based survey was conducted among colorectal surgeons who represented members of both SAGES and ASCRS to find out how they define conversion for laparoscopic colorectal surgery. METHODS: Questionnaires were designed based on MCQs, including three parts: surgeon information, different definitions for conversion, and four different clinical scenarios. Surgeons were asked to choose the best definition(s). RESULTS: 325 (28.5%) of 1,140 surgeons, 28.5% responded; approximately half of them were part of private-based practices. Fifty-three percent had more than 10 years experience

[Operating position in colorectal surgery. The importance of the basics].

Intra-operative positioning in colorectal surgery is very important from three points of view: the proper surgical approach and exposure, adequate anaesthetic requirements with maintenance of the airway, and the potential complications related to the position. In the present study, we analyse the indications, positioning, advantages and disadvantages of each operative position, their potential complications and how to avoid them. These complications can be of a diverse nature, the most common being injuries related to stretching or compression of peripheral nerves, followed by thromboembolic,

Evaluating the degree of difficulty of laparoscopic colorectal surgery.

OBJECTIVE: To quantify the degree of overall difficulty and the difficulty of each of the individual steps involved in the performance of laparoscopic colorectal procedures. The data should serve as a guide to surgeons in the early stages of their experience in laparoscopic colorectal surgery as to which procedures and steps to embark on first, to allow them to build experience in a stepwise fashion. METHODS: A mail-in survey of 35 experienced laparoscopic colorectal surgeons was conducted. Using a scale of 1 to 6, the surgeons were asked to rate the overall degree of difficulty of each of 12

Robotic and laparoscopic surgery for treatment of colorectal diseases.

PURPOSE: In the last ten years, several robotic systems have been developed to overcome the loss of the three-dimensional view and dexterity characteristic of laparoscopic surgery. The aim of this study was to compare the traditional laparoscopic approach and robotic techniques in the treatment of colorectal diseases. METHODS: The study compares a consecutive series of patients treated surgically for colorectal disease from June 2001 to May 2003 with the da Vinci robotic system (Intuitive Surgical) and a matched number of patients who underwent conventional laparoscopy during the same time in

The risk of thromboembolism in patients undergoing colorectal surgery.

Patients undergoing colorectal surgical procedures are at a moderately high risk for the development of thromboembolic events. Possible reasons may be that the procedures tend to be long in duration, patients are often placed in the lithotomy position, a pelvic dissection may be performed, and the indication is often inflammatory bowel disease or cancer. While prophylaxis with low dose heparin is effective at reducing this risk, approximately 10% of patients may still develop a thromboembolic event. Results on the effectiveness of low molecular weight heparin in this cohort of patients are pen

Does Colorectal Stenting as a Bridge to Surgery for Obstructive Colorectal Cancer Increase Perineural Invasion?

OBJECTIVES: To clarify whether self-expandable metallic stent (SEMS) placement for obstructive colorectal cancer (CRC) increases perineural invasion (PNI), thereby worsening the prognosis. METHODS: In total, 1022 patients with pathological T3 or T4 colon or rectosigmoid cancer who underwent resection were retrospectively reviewed. The study patients were divided into a no obstruction group (n=693), obstruction without stent group (n=251), and obstruction with stent group (n=78), and factors demonstrating an independent association with PNI, the difference in PNI incidence and severity between

The impact of previous abdominal surgery on colorectal cancer patients undergoing laparoscopic surgery.

The current study aimed to investigate whether previous abdominal surgery (PAS) could affect the outcomes of colorectal cancer (CRC) surgery. We conducted the search strategy in three databases (PubMed, Embase, and the Cochrane Library) from inception to May 26, 2022. The short-term and long-term outcomes were compared between the PAS group and the non-PAS group. Odds ratios (ORs) and 95% confidence intervals (CIs) were pooled up. Stata (V.16.0) software was used for data analysis. We included 34,827 patients from 14 studies in the current study. After pooling up all the data, we found that th

Minimally invasive versus open surgery among colorectal cancer patients.

Minimally Invasive Surgery has become an acceptable substitute for Open Surgery in the management of colorectal cancer, with the claimed benefits of reduced morbidity and quicker recovery. Therefore it is of interest to compare the clinical and oncological results of MIS and OS among 200 patients with colorectal cancer over five years. The patients were evaluated for operative time, complications, hospital stay and disease-free survival. MIS has been demonstrated to be linked to markedly reduced stays in hospital (6 vs. 10 days) and decreased complication rates (15% vs. 25%) versus Open Surger
