Differences in mental health symptom severity and care engagement among transgender and gender diverse individuals: Findings from a large community health center.

Mental health disparities among transgender and gender diverse (TGD) populations have been documented. However, few studies have assessed differences in mental health symptom severity, substance use behavior severity, and engagement in care across TGD subgroups. Using data from the electronic health record of a community health center specializing in sexual and gender minority health, we compared the (1) severity of self-reported depression, anxiety, alcohol use, and other substance use symptoms; (2) likelihood of meeting clinical thresholds for these disorders; and (3) number of behavioral he

IMPACT: A Structured Human-Centered Design Process for Integrating Community Health Worker Insights Into Program Policy in Pakistan.

BACKGROUND: Community health workers (CHWs), most of whom are women, are the bedrock of primary health care provision in much of the world. CHWs are often employed at the bottom of health hierarchies where they have little voice; in state programs and vertical initiatives, accountability has generally flowed downwards. Yet many programs might function better if CHWs' needs and ideas were considered in program design. METHODS: From 2020-2022, we implemented a human-centered design process called IMPACT with CHWs working on polio vaccination in one district of Pakistan. The name IMPACT draws fr

The Vital Role of a Federally Qualified Community Health Center in New Orleans, Louisiana, During the COVID-19 Pandemic.

Federally qualified health centers (FQHCs) are on the front lines of the COVID-19 pandemic. Their mission of providing essential medical care to underserved populations is now even more vital. CrescentCare, an FQHC in New Orleans, evaluated and tested 3366 patients between March 16 and July 2, with an overall rate of 12% SARS-CoV-2 positivity. The clinic's experience demonstrates how to effectively and rapidly integrate COVID-19 programing, while preserving essential health services. Strategies include developing a walk-in COVID-19 testing site, ensuring appropriate clinical evaluation, provid

Expanding Zero Suicide: Qualitative Assessment of Community Mental Health Center Partnerships Supporting Rural Suicide Prevention.

Rural communities in the United States have elevated suicide death rates and lower access to mental health services than more urbanized areas. Zero Suicide is a framework that promotes evidence-based suicide care for individuals at risk for suicide and is increasingly being adopted in rural healthcare and mental health provider organizations, including Community Mental Health Centers (CMHCs). Population-focused suicide prevention is more likely to be effective when mental health and healthcare providers engage with social services and other provider organizations (e.g., schools, law enforcemen

Community Health Center Penetration and Kidney Care Outcomes among Low-Income, Nonelderly Adults with Kidney Failure.

KEY POINTS: Populations who experience health disparities often rely on community health centers (CHCs) for ambulatory care. Among low-income populations, higher CHC penetration is associated with greater preparedness for, and better outcomes after, kidney failure onset. Our study suggests that CHCs provide essential ambulatory care for nonelderly adults who experience kidney health disparities. BACKGROUND: In the United States, historically minoritized populations experience disproportionately high incidence of progressive kidney disease but are often unprepared for kidney failure. Owing to

Rectal specimen self-collection for chlamydia and gonorrhea screening: a cross-sectional feasibility study at a community health center.

BACKGROUND: Men who have sex with men (MSM) are at increased risk for extra-genital sexually transmitted infections (STIs). Without extra-genital screening, many chlamydia and gonorrhea infections would be missed among MSM. Yet, many barriers exist to extra-genital testing, and, in particular, to rectal collection. Self-collection increases screening and detection of asymptomatic chlamydia and gonorrhea among at-risk MSM and transgender women. This feasibility study assessed use of rectal self-collection and its acceptance among patients and primary care providers (PCPs) at a large, general pr

Oral health status and barriers to care in a multiethnic mixed disability center: Rethinking disability community dental services.

AIMS: There are currently neither onsite services nor any known studies that addresses oral health of persons with special care needs (PSCN) within disability activity centers in Singapore. This study aims to report on the oral health status, disability-related barriers, and assess how these barriers limit care pathways. METHODS AND RESULTS: 29 PSCN from a multiethnic mixed disability day activity center were screened over two days. Their ages ranged from 18-59 years old (mean: 31.3). Proportion of Chinese, Malay and Indian ethnicity was 69.6%, 26.7% and 10.3% respectively. 11 were female. On

Communicating Cervical Cancer Screening Results in Light of New Guidelines: Clinical Practices at Federally Qualified Health Centers.

New guidelines for cervical cancer screening (CCS) incorporate both HPV and Pap tests, and there is a need to understand communication of these cotesting results to patients, especially in at-risk populations disproportionally affected by cervical cancer. This study used computer-assisted telephone interviews in 2017 at 51 federally qualified health centers (FQHCs) in Indiana to evaluate the characteristics of clinical communication CCS results to women. Results revealed that clinical communication practices varied on channel, timing, and content. Almost half of the clinics (n = 23, 45%) commu

Improving Treatment Completion Rates for Latent Tuberculosis Infection: A Review of Two Treatment Regimens at a Community Health Center.

BACKGROUND: Prophylactic treatment of latent tuberculosis infection (LTBI) is necessary for controlling TB in low-incidence settings. However, treatment is often limited by poor completion rates. METHODS: At a community health center serving low-income Hispanics, treatment completion among patients accepting 12 weekly doses of isoniazid (INH) plus rifapentine (RPT) administered as directly observed therapy (DOT) was compared with that among patients accepting nine months of daily self-administered INH during 2012 and 2013 (n=139). RESULTS: Among patients who agreed to treatment, INH-RPT comb

Mobile addiction treatment and harm reduction services as tools to address health inequities: a community case study of the Brockton Neighborhood Health Center mobile unit.

Opioid overdose deaths continue to increase in the US. Recent data show disproportionately high and increasing overdose death rates among Black, Latine, and Indigenous individuals, and people experiencing homelessness. Medications for opioid use disorder (MOUD) can be lifesaving; however, only a fraction of eligible individuals receive them. Our goal was to describe our experience promoting equitable MOUD access using a mobile delivery model. We implemented a mobile MOUD unit aiming to improve equitable access in Brockton, a racially diverse, medium-sized city in Massachusetts. Brockton has a
