Are large-scale volunteer community health worker programmes feasible? The case of Sri Lanka.

National community health worker (CHW) programmes supported by Ministries of Health have been introduced in a number of countries as part of their primary health care policy. Although in many of these programmes the CHWs are salaried or receive an honorarium, there are a number of large-scale programmes in which CHWs work as unpaid volunteers. This paper looks at one such programme in Sri Lanka, in order to understand the motivation of such volunteers and to consider the feasibility of relying on volunteers to support primary health care policies. The lessons from the Sri Lanka case are genera

Treatment-Seeking for Children with Suspected Severe Malaria Attending Community Health Workers and Primary Health Centres in Adamawa State, Nigeria.

OBJECTIVES: In the context of introducing pre-referral rectal artesunate for children with severe malaria in Adamawa State, Nigeria, case fatality was 19% among severely sick children visiting primary health centres (PHCs), and 6% among those visiting community health workers (CORPs). To understand this difference, we investigated illness severity, treatment-seeking, and background characteristics of these patients. METHODS: 589 children <5 years of age with fever and danger signs indicative of severe malaria attending CORPs (N = 314) and PHCs (N = 275) were enrolled. After 28 days, caregiver

Understanding the roles of community health workers in improving perinatal health equity in rural Uttar Pradesh, India: a qualitative study.

BACKGROUND: Despite substantial reductions in perinatal deaths (stillbirths and early neonatal deaths), India's perinatal mortality rates remain high, both nationally and in individual states. Rates are highest among disadvantaged socio-economic groups. To address this, India's National Health Mission has trained community health workers called Accredited Social Health Activists (ASHAs) to counsel and support women by visiting them at home before and after childbirth. We conducted a qualitative study to explore the roles of ASHAs' home visits in improving equity in perinatal health between soc

NCD-SCAN: A Digital Health Tool for Community-Based Non-Communicable Disease Risk Screening by Lady Health Workers in Pakistan.

BACKGROUND: Non-communicable diseases (NCDs) account for the majority of premature mortality in low- and middle-income countries, yet early detection remains limited within Pakistan's primary healthcare system. Lady Health Workers (LHWs) represent a large, trusted, community-based workforce with potential to support digital NCD screening. This study assessed the feasibility, acceptability, and early implementation of NCD-SCAN, a mobile application integrating validated tools for cardiovascular, metabolic, cerebrovascular, lifestyle, and mental health risk assessment. METHODS: We conducted a p

Public health surveillance through community health workers: a scoping review of evidence from 25 low-income and middle-income countries.

BACKGROUND: The last 3 years have witnessed global health challenges, ranging from the pandemics of COVID-19 and mpox (monkeypox) to the Ebola epidemic in Uganda. Public health surveillance is critical for preventing these outbreaks, yet surveillance systems in resource-constrained contexts struggle to provide timely disease reporting. Although community health workers (CHWs) support health systems in low-income and middle-income countries (LMICs), very little has been written about their role in supporting public health surveillance. This review identified the roles, impacts and challenges CH

Antibiotic prescribing practices of community health extension workers in East Central Uganda: an exploratory qualitative study.

BACKGROUND: Antibiotic misuse at the community level remains a critical contributor to antimicrobial resistance, particularly in resource-constrained settings. Community Health Extension Workers (CHEWs) play a pivotal role in primary healthcare delivery and antibiotic access, yet little is known about how they navigate prescribing decisions amid systemic limitations, social expectations and ethical pressures. OBJECTIVE: This study explored CHEWs' perceptions, practices and experiences related to antibiotic prescribing and dispensing in resource-constrained settings, with particular attention

Psychometric properties of MERIT: A concise tool for community health workers to screen priority mental disorders.

BACKGROUND: Task shifting is an accepted way to address the treatment gap for mental health disorders. Community Health Workers (CHWs) can play vital roles through screening, referral, and providing basic counseling. To address the dearth of a concise screening instrument for CHWs, the "Mental HEalth ScReenIng Tool for Community Health Workers in India [MERIT]" was designed and developed. In this paper, we have examined the psychometric properties of MERIT. METHODS: MERIT consists of nine stems (consisting of 11 questions) covering the following five domains: (a) Substance Abuse (alcohol, tob

Screening for cervical and breast cancer among community health care workers (ASHAs) of Puri district, Odisha.

BACKGROUND: Breast cancer and cervical cancer are the leading causes of cancer-related mortality in Indian women. As part of the Indian national guidelines for population-based cancer screening, village-level accredited social health activists (ASHAs) are the key mobilizers for encouraging women to undergo cervical and breast cancer screening. Assessing screening rates in these health workers themselves, can improve implementation of the national program by identifying some of the facilitators and barriers for screening. METHODS: A cross-sectional study was conducted among ASHAs in Puri, Odis

Five-year retention of volunteer community health workers in rural Uganda: a population-based retrospective cohort.

Community health workers (CHWs) effectively improve maternal, newborn and child health (MNCH) outcomes in low-to-middle-income countries. However, CHW retention remains a challenge. This retrospective registry analysis evaluated medium-term retention of volunteer CHWs in two rural Ugandan districts, trained during a district-wide MNCH initiative. From 2012 to 2014, the Healthy Child Uganda partnership facilitated district-led CHW programme scale-up. CHW retention was tracked prospectively from the start of the intervention up to 2 years. Additional follow-up occurred at 5 years to confirm rete

Adolescents left behind by migrant workers: a call for community-based mental health interventions in Nepal.

Over the past two decades, the unique health needs associated with the second decade of life have been recognized, not least the mental health of adolescents. In parallel, the negative health impacts of parental migration on the children and adolescents who are "left behind" in low- and middle-income countries (LMICs) is beginning to be acknowledged. Nepal is a growing supplier of labour migrants - an estimated 3.5 million Nepali individuals are working abroad - resulting in families being separated and thousands of adolescents being left behind. This can increase psychological and emotional s

Home visits by community health workers to prevent neonatal deaths in developing countries: a systematic review.

OBJECTIVE: To determine whether home visits for neonatal care by community health workers can reduce infant and neonatal deaths and stillbirths in resource-limited settings. METHODS: We conducted a systematic review up to 2008 of controlled trials comparing various intervention packages, one of them being home visits for neonatal care by community health workers. We performed meta-analysis to calculate the pooled risk of outcomes. FINDINGS: Five trials, all from south Asia, satisfied the inclusion criteria. The intervention packages included in them comprised antenatal home visits (all trial

Occupational rehabilitation services provided by a community workers health centre in Hong Kong: a case study.

BACKGROUND: In Hong Kong, the Employees' Compensation Ordinance (ECO), based on the model of the English Workmen's Compensation Act 1943, establishes a no-fault, non-contributory system for work injuries. It focuses merely on the minimum social expectation in compensating injured workers in monetary terms, but does not place much emphasis on prevention, rehabilitation and return to work policies for workers with injury. There is a need to reform an effective and comprehensive occupational rehabilitation system for workers with work injury in Hong Kong. METHODS: This paper presents a case stud

Risk of tuberculin skin test conversion among health care workers: occupational versus community exposure and infection.

A prospective observational cohort study to assess rates of and risk factors for tuberculin skin test (TST) conversion among health care workers (HCWs) was conducted at an urban hospital located in a high tuberculosis-incidence area in 1994-1998. All hospital employees undergoing required testing every 6 months were included. A total of 69 (1.2%) of 5773 susceptible employees had a documented TST conversion (overall rate, 0.38 per 100 person-years worked). No significant difference existed in conversion rates among employees with frequent, limited, or no patient contact. HCWs with a TST conver

Evaluating Longitudinal Community Health Worker-Led Interventions for Improving Hypertension Management in Urban U.S. Communities: A Rapid Review.

INTRODUCTION: Underserved urban U.S. communities experience a disproportionate burden of uncontrolled hypertension, driven by structural and socioeconomic barriers. Community health workers may support sustained improvements in hypertension control and self-management. METHODS: The authors systematically searched PubMed (2015-2025) for U.S.-based, urban, longitudinal community health worker-led interventions recruiting participants directly from community settings with reported pre/post blood pressure outcomes. Studies conducted in rural settings, recruiting from clinical settings, or lacking

"Our Community Needs More Information About Oral Health": Co-Designing a Culturally Grounded Oral Health Promotion Training With Aboriginal Health Workers and Practitioners.

OBJECTIVES: Oral health is central to holistic wellbeing for Aboriginal and Torres Strait Islander Peoples, yet mainstream promotion approaches often rely on individualistic models that fail to reflect Community realities. Aboriginal Health Workers and Practitioners (AHW/Ps), as trusted Community members and the only nationally registered culturally based health profession, are uniquely positioned to embed oral health within relational models of care. Despite this strength, oral health remains absent from mandatory AHW/P training and continuing professional development. This paper aims to docu

Community health workers: a narrative review of a curriculum and training program for low-income communities facing limited access to healthcare.

BACKGROUND: The aim of this narrative review is to examine a WHO-compliant program that prepares Community Health Workers (CHWs) in low-resource environments. The intended outcome of the training is to enhance healthcare access and address health equity disparities. We examined the program's curriculum, instructional methods, and a complementary continuing medical education (CME) program designed to sustain CHWs' knowledge and skills. METHODOLOGY: We review, in detail, the CHW training program-its curriculum, delivery, and implementation-launched prior to the COVID-19 pandemic and continuing
