Navigating Disparities and Overcoming Barriers to Access Diabetic Kidney Disease Therapies: A Proposed Multifactorial Approach.

<p>Background: Diabetic kidney disease (DKD) is a serious complication arising from long-term diabetes, disproportionately affecting historically marginalized populations, such as racial and ethnic minority groups, populations with low socioeconomic status, or a lower level of education. This review explores the causes of these disparities and barriers to accessing DKD therapies and proposes solutions. Summary: Socioeconomic factors like lack of health insurance, low income, and limited health literacy significantly hinder access to DKD therapies for marginalized communities. Additionally, ina

Predictors of institutional delivery service utilization among women in Northern region of Ghana.

INTRODUCTION: An increase in home delivery among expectant mothers may likely lead to high maternal and newborn morbidities and mortalities. Despite the policy on free maternal healthcare in Ghana under the National Health Insurance Scheme (NHIS) since 2007, more than 25% of deliveries still occur outside health facilities in northern Ghana. Use of safe and effective delivery services including place of delivery is an important component of the Safe Motherhood concept. Hence, assessing predictors of institutional delivery could contribute to improving birth outcomes in the Northern Region. ME

Determinants of delayed healthcare seeking among STI patients in Eastern Ethiopia: a case-control study.

BACKGROUND: Delayed healthcare seeking is one of the major impediments to successfully preventing and controlling sexually transmitted infections. A delay in healthcare seeking prolongs the period of infectiousness and increases the incidence of sexually transmitted diseases. The majority of sexually transmitted infections are curable, but several social and behavioral factors hinder people from seeking early healthcare. OBJECTIVE: To identify determinants of delay in healthcare seeking among patients with sexually transmitted infections at governmental hospitals in the Harari region, Ethiopi

Community-based strategies for tuberculosis control in migrant communities: how to integrate determinants of vulnerability into decision-making.

BACKGROUND: Tuberculosis (TB) remains a major public health challenge, particularly among migrant populations, who face increased vulnerability due to social and structural determinants of health. In Catalonia, Spain, the Model of Community and Public Action (MACIP) programme was launched in 2023 to improve TB prevention and care through culturally adapted, community-based interventions delivered by community health workers (CHWs). This study aims to describe the vulnerability determinants among migrants diagnosed with TB and enrolled in the MACIP programme, and to present the community-based

Direct costs of neonatal infection acquired in the community in full-term newborns and low risk at birth, Cundinamarca, Colombia.

Introduction: Half of the episodes of neonatal sepsis are acquired in the community with a high percentage of mortality and complications. Objective: To estimate the direct costs of hospitalizations due to systemic neonatal infection acquired in the community in low-risk newborns. Materials and methods: For the estimation of costs, we used the perspective of the health systems and the microcosting technique and we established the duration of hospitalization as the time horizon. We identified cost-generating events through expert consensus and the quantification was based on the detailed bill o

Implementation and Uptake of Rural Lung Cancer Screening.

OBJECTIVE: Given the higher rates of tobacco use along with increased mortality specific to lung cancer in rural settings, low-dose CT (LDCT)-based lung cancer screening could be particularly beneficial to such populations. However, limited radiology facilities and increased geographical distance, combined with lower income and education along with reduced patient engagement, present heightened barriers to screening initiation and adherence. METHODS: In collaboration with community leaders and stakeholders, we developed and implemented a community-based lung cancer screening program, includin

Long term outcome of a community-based hepatitis B awareness campaign: eight-year follow-up on linkage to care (LTC) in HBV infected individuals.

BACKGROUND: Chronic hepatitis B (CHB) is a major cause of liver-related morbidity and mortality. High HBV prevalence in immigrants and ethnic minorities and numerous barriers to healthcare access are associated with serious health disparities in the United States. Reportedly, self-awareness of HBV infection is low, suggesting a greater need for effective screening and education. Further, low levels of linkage to care (LTC) (completion of a first doctor's visit after the diagnosis of chronic HBV infection) may be responsible for the lack of engagement over the continuum of care and for needed s

Correlates of sexually transmitted infection testing following women's release from jail.

Given incarcerated women's frequent transitions between jail and community, it is important to seize opportunities to provide comprehensive health care. A potential time to provide care might be when getting tested for sexually transmitted infections (STIs). Our objective was to determine the proportion of women receiving STI testing and correlates, following jail release. This secondary analysis was of one-year follow-up data from women who participated in a jail-based cervical health literacy intervention in three Kansas City jails from 2014 to 2016. Most (82%) completed the survey in the co

Long-acting reversible contraception initiation after medication abortion: a retrospective cohort study.

BACKGROUND: Medication abortion (MAB) accounts for an increasing proportion of in-clinic abortions in the United States and poses unique considerations for provision of long-acting reversible contraception (LARC). Studies of LARC initiation among MAB patients mostly consist of trials where financial barriers to LARC were removed. We sought to identify correlates of LARC initiation post-MAB in a community-based setting. METHODS: This is a retrospective cohort study of patients who presented to a Planned Parenthood Health Center in Minnesota in 2016 for MAB, chose LARC as their intended post-ab
