Prioritisation and risk ranking of epidemic-prone diseases for emergency preparedness and response in Eastern Africa using a multi-criteria decision analysis framework, 2023.

BACKGROUND: Resources to respond to emerging and re-emerging infectious disease emergencies are limited. AIM: To support preparedness planning and resource allocation, a prioritisation methodology was applied to rank epidemic-prone diseases in Eastern Africa. SETTING: The study took place in the eastern region of Africa representing 14 member states. METHODS: A multi-criteria decision analysis (MCDA) combined with a modified Delphi approach, adapted from the Africa Centres for Disease Control and Prevention (CDC's) continental prioritisation framework was employed. A planning team of epidem

Trends of malaria incidence, prevalence, mortality, and disability-adjusted life years in Eastern Africa region from 1990 to 2021: a systematic analysis from Global Burden of Disease 2021 study.

BACKGROUND: Malaria has long been a global health priority, particularly in Eastern Africa, where it remains the leading cause of child mortality and a significant barrier to socioeconomic development. Despite extensive control efforts, ineffective programs, limited treatment access, and low distribution of artemisinin-based combination therapy, contribute to the persistent challenges. Understanding malaria trends is crucial for evidence-based prevention and control strategies. The objective of the study was to estimate the trend in malaria incidence, prevalence, mortality, and Disability-Adju

Women's decision-making power and knowledge of prevention of mother to child transmission of HIV in sub-Saharan Africa.

BACKGROUND: Sub-Saharan Africa (SSA) bears the highest burden of Human Immunodeficiency Virus (HIV) in the world. Even though the prevention of mother to child transmission (PMTCT) programmme is one of the strategies to control the HIV pandemic, the uptake in SSA countries is low. Women's decision-making power has a positive influence on health seeking behavior and uptake of several maternal health services. However, its relationship with knowledge of PMTCT services is understudied in SSA. Therefore, this study aimed to examine the association between women's decision-making power and knowledg

Analysis of trichoscopic signs observed in 24 patients presenting tinea capitis: Hypotheses based on physiopathology and proposed new classification.

INTRODUCTION: Trichoscopy (hair dermoscopy) is a non-invasive and very useful technique for the diagnosis and follow-up of hair and scalp disorders. In tinea capitis, specific aspects of the hair shaft have been described, with the main ones being: comma hair, corkscrew hair, bar code-like hair (BCH) and zigzag hair (ZZH). METHOD: Herein we report on a retrospective study of 24 patients with tinea capitis (TC). All patients underwent trichoscopic examination and mycological culture. RESULTS: Trichoscopy was abnormal in all 24 patients showing hair-shaft abnormalities. We observed three types

A new XPC gene splicing mutation has lead to the highest worldwide prevalence of xeroderma pigmentosum in black Mahori patients.

Xeroderma pigmentosum (XP) is a rare, recessive disease characterized by sunlight hypersensitivity and early appearance of cutaneous and ocular malignancies. We report the first description of a very high incidence (around 1/5000) of black XP patients in the Mayotte population in the Indian Ocean. Among a cohort of 32 XP, we describe the clinical and genetic features of 18 living Comorian black XP patients. We discuss the remarkable clinical differences between white and black XPs. Skin and ocular abnormalities are remarkably precocious and severe XP phenotypes are recognized by the early ocul

Earlier onset of puberty in girls: relation to increased body mass index and race.

OBJECTIVE: A recent study conducted by the Pediatric Research in Office Settings network provided evidence that girls in the United States, especially black girls, are starting puberty at a younger age than earlier studies had found, but the reasons for this are not known. Because nutritional status is known to affect timing of puberty and there is a clear trend for increasing obesity in US children during the past 25 years, it was hypothesized that the earlier onset of puberty could be attributable to the increasing prevalence of obesity in young girls. Therefore, the objective of this study

Epidemiology of Taenia saginata taeniosis/cysticercosis: a systematic review of the distribution in southern and eastern Africa.

BACKGROUND: The beef tapeworm, Taenia saginata, causing cysticercosis in bovines and taeniosis in humans, is thought to have a global distribution. In eastern and southern Africa, cattle production plays a crucial role in the economy, but a clear overview of the prevalence of T. saginata in the region is still lacking. This review aims to summarize existing knowledge on T. saginata taeniosis and bovine cysticercosis distribution in eastern and southern Africa. METHODS: A systematic review was conducted, that gathered published and grey literature, including OIE reports, concerning T. saginata

Angiostrongylus cantonensis Infection on Mayotte Island, Indian Ocean, 2007-2012.

INTRODUCTION: Human angiostrongyliasis (HA) is a neurological helminthic disease caused by the lung worm Angiostrongylus cantonensis. It is suspected in the combination of travel or a residence in an endemic area and eosinophilic meningitis. In Mayotte, an island in the Indian Ocean, cases are rare but regular. The main objective of our study was to describe the epidemiological and diagnosis clues of HA in Mayotte. The secondary objectives were to evaluate the contribution of Real-Time Polymerase Chain Reaction (RT- PCR) for the diagnosis of HA, delineate the characteristics of the local trans

Leptospira mayottensis sp. nov., a pathogenic species of the genus Leptospira isolated from humans.

A group of strains representing species of the genus Leptospira, isolated from patients with leptospirosis in Mayotte (Indian Ocean), were previously found to be considerably divergent from other known species of the genus Leptospira. This was inferred from sequence analysis of rrs (16S rRNA) and other genetic loci and suggests that they belong to a novel species. Two strains from each serogroup currently identified within this novel species were studied. Spirochaete, aerobic, motile, helix-shaped strains grew well at 30-37 °C, but not at 13 °C or in the presence of 8-azaguanine. Draft genomes

[How to live with keratoconus].

BACKGROUND: The article is based on an undergraduate thesis that was written within the framework of investigating problems of rehabilitation in the humanities department of Berlin's Humboldt University. It deals with the issue of whether and, if so, in what spheres of life, people suffering from keratoconus may have to expect changes in lifestyle and what possibilities exist for compensation. The paper is based on the "construct of compensating impairment of sight" according to Nater, a fundamentally resource-oriented approach. Impaired vision has restrictive effects on the quality of life, w

[Polyneuropathies in vitamin B1 deficiency in Reunion and Mayotte islands in 70 patients of Maori and Comorian descent].

Beriberi is an uncommon disorder related to thiamine deficiency. It is mainly found in underdeveloped countries among populations with poorly diversified diet, consisting largely of milled white cereals, a poor source of thiamine. In industrialized countries, thiamine deficiency with cardiac failure is more frequently found than the dry beriberi in high risk groups like chronic alcoholics. Nevertheless our attention was drawn to an outbreak of 70 cases of dry beriberi which occurred from 1997 to 2005 in the French territories of Reunion and Mayotte islands. It was characterized by an acute or

Introducing rotavirus vaccine in eight sub-Saharan African countries: a cost-benefit analysis.

BACKGROUND: Stimulated by the economic challenges faced by many sub-Saharan African countries and the changes in the rotavirus burden across these countries, this study aimed to inform the decision of health policy makers of eight sub-Saharan countries, who are yet to introduce the rotavirus vaccine as of Dec 31, 2020, on the health economic consequences of the introduction of the vaccine in terms of the costs and benefits. METHODS: We did a cost-benefit analysis using a simulation-based decision-analytic model for children aged younger than 1 year, who were followed up to 259 weeks, in the C

[The epidemiology of trench fever: a pilot study in homeless people in Marseilles].

Trench fever, cause by Bartonella quintana, disappeared decades ago. Between 1992 and 1994 about 20 cases were reported in industrialized countries [6, 7]. Some of these cases were associated with HIV infection [5], but most affected the homeless. The epidemiology of B. quintana is unknown in Europe, but infection and transmission are favoured by poor housing and hygiene. This paper describes the results of a seroprevalence survey among the homeless living in Marseille. The eight major non-profit organizations caring for the homeless in the city participated for the four winter months. Four of

Spatial analysis of unimproved drinking water source in East Africa: Using Demographic and Health Survey (DHS) data from 2012-2023.

BACKGROUND: According to the WHO/UNICEF Joint Monitoring Programme, unimproved drinking water sources include unprotected wells and springs, surface water (e.g., rivers, lakes), vendor-provided water, bottled water (without improved alternatives), and tanker truck-provided water. In East Africa, 68.7% of water at very high risk comes from such sources. Using unimproved drinking water sources can lead to serious health risks, including waterborne diseases such as cholera, dysentery, typhoid fever, and hepatitis. Therefore, this study aimed to map the spatial distribution of unimproved drinking

Intimate partner violence and childhood health outcomes in 37 sub-Saharan African countries: an analysis of demographic health survey data from 2011 to 2022.

BACKGROUND: Understanding the contribution of intimate partner violence (IPV) to childhood health outcomes (eg, morbidity and mortality) is crucial for improving child survival in sub-Saharan Africa. This comprehensive study aimed to explore the associations between maternal exposure to physical, sexual, or emotional violence and adverse childhood health outcomes in sub-Saharan Africa. METHODS: We analysed Demographic Health Survey datasets from 37 sub-Saharan African countries from 2011 to 2022. A generalised linear mixed model was used to examine the associations between maternal physical v

A regional One Health approach to the risk of invasion by Anopheles stephensi in Mauritius.

BACKGROUND: Anopheles stephensi is an invasive malaria vector in Africa that threatens to put an additional 126 million people at risk of malaria if it continues to spread. The island nation of Mauritius is highly connected to Asia and Africa and is at risk of introduction due to this connectivity. For early detection of An. stephensi, the Vector Biology and Control Division under the Ministry of Health in Mauritius, leveraged a well-established Aedes program, as An. stephensi is known to share Aedes habitats. These efforts triggered multisectoral coordination and cascading benefits of integra

Prevalence of type 2 diabetes and other cardiovascular risk factors in Mayotte in 2008: the MAYDIA study.

AIM: Mayotte, a French overseas territory located in the Indian Ocean, has never had a previous estimate of diabetes prevalence, but has recently undergone socioeconomic changes leading to lifestyle modifications. For this reason, a survey was carried out in 2008 to estimate the prevalence of diabetes and cardiovascular risk factors in the island's population. METHODS: A three-step, randomized sample of 1268 individuals, aged 30-69 years, was home-screened, using capillary blood glucose and capillary HbA(1c), weight, height, waist circumference and two blood-pressure measurements. [Calcified splenic abscess, colonic fistula and ascites in a chronic carrier of Salmonella typhi].

We report the unusual case of a patient with chronic carriage of Salmonella typhi who presented with partially calcified splenic abscess linked to colic fistula and ascitis. The colic fistula could be secondary to ischemic necrosis by left colon compression due to spleen large abscess. Fistula was evidenced by abdominal computed tomography scan and confirmed by barium enema. The possible etiologies of ascitis are either tuberculosis or ascitic peritonitis secondary to the fistulisation; nevertheless, the role of segmentary portal located hypertension cannot be completely excluded. The splenic
