[Compartment syndrome of the leg (author's transl)].

The authors have observed ten compartment syndromes of the leg. Nine occurred after tibial fractures, one after a contusive trauma. Early signs those of muscular and neural ischemia, with an acute pain when the toes are passively mobilised. The cast must be promptly removed. In 6 cases the two compartments were involved. In all cases the pulses were palpable. Arteriographs could occasionaly verify the integrity of the main arterial trunks. The treatment is based on early fasciotomies. It was realised in 5 cases, with good results when the fasciotomy was realised more than twelve hours after th

Superior Mesenteric Artery Syndrome with Abdominal Compartment Syndrome.

Superior Mesenteric Artery (SMA) syndrome is a condition in which the duodenum becomes compressed between the SMA and the aorta, resulting in bowel obstruction which subsequently compresses surrounding structures. Pressure on the inferior vena cava (IVC) and aorta decreases cardiac output which compromises distal blood flow, resulting in abdominal compartment syndrome with ischemia and renal failure. A 15-year-old male with SMA syndrome presented with 12 hours of pain, a distended, rigid abdomen, mottled skin below the waist, and decreased motor and sensory function in the lower extremities. E

[Abdominal compartment syndrome and acute intestinal distress syndrome].

Seriously ill patients frequently present intra-abdominal hypertension (IAH) and abdominal compartment syndrome (ACS) as complications, and the associated mortality is very high. This review offers an update on the most controversial aspects of these entities: factors favoring their appearance, the most common causes, prognosis, and methods of measuring intra-abdominal pressure (IAP), physiopathological consequences in relation to the different organs and systems, and the currently accepted treatment measures (medical and/or surgical). Simultaneously to the strictly physical mechanisms of inju

Low compartment pressure and myoglobin levels in tibial fractures with suspected acute compartment syndrome.

BACKGROUND: The intense ischemic pain of acute compartment syndrome can be difficult to discriminate from the pain related to an associated fracture. Lacking objective measures, the decision to perform fasciotomy is often only based on clinical findings and performed at a low threshold. Biomarkers of muscle cell damage might help to identify and monitor patients at risk. In patients with fractures, however, markers of muscle cell damage could be elevated because of other reasons associated with the trauma, which would make interpretation difficult. In a review of all patients who underwent eme

Corrective arthrodesis to treat sequelae of compartment syndrome and ischaemic syndromes of lower leg and foot.

BACKGROUND: There is a scarcity of studies concerning the treatment of foot and talocrural deformities secondary to a compartment syndrome or ischaemia. MATERIAL AND METHODS: Five patients (4 males and 1 female) were treated by talocrural arthrodesis (1 foot) and triple tarsal arthrodesis (4 feet) due to equinovarus foot deformity secondary to compartment syndrome or ischaemia. The age at surgery ranged 28 to 50 years (mean 39 years). The deformity was a sequela to compartment syndrome due to an injury in 3 patients, popliteal artery damage in one male patient, and an embolism in the femoral

Volkmann contracture and compartment syndromes after femur fractures in children treated with 90/90 spica casts.

Nine pediatric patients (mean age 3.5 yrs) with low energy femur fractures were treated with 90/90 spica casts and developed leg compartment syndromes, Volkmann contracture, and ankle skin loss. These cases are detailed and a proposed mechanism leading to this devastating complication is explained. The authors believe the technique of an initial below knee cast, and then using that cast for applying traction while immobilizing the child in the 90/90 spica is potentially dangerous. Alternative spica application methods are advocated.

Upper limb compartment syndromes: a complication of malignant hyperthermia in a patient with ill-defined myopathy.

We report a case of compartment syndrome complicating malignant hyperthermia (MH) in a 12-yr-old girl with a history of myopathy and multiple skeletal deformities; she underwent bilateral Achilles tendon surgery. Marked oedema of both forearms became evident in the immediate postoperative period and resolved after conservative treatment. Compartment syndrome is a rare complication of MH. Early recognition and therapy may prevent the onset of muscle ischaemia and distal neurovascular deficit. The need for urgent surgery and repeated anaesthesia in the early phase of recovery from an acute episo

Who Gets Compartment Syndrome?: A Retrospective Analysis of the National and Local Incidence of Compartment Syndrome in Patients With Supracondylar Humerus Fractures.

PURPOSE: Although acute compartment syndrome (ACS) is associated with pediatric supracondylar humerus (SCH) fractures, there are limited data describing its incidence and risk factors. The purpose of our study was to report the local and national incidence of ACS with SCH and floating elbow (concomitant SCH and forearm) fracture patterns and the associated risk factors. METHODS: We retrospectively queried data for SCH fracture patients over a 4-year period from our institution (a level I pediatric trauma center) and the National Trauma Data Bank (NTDB). Data on demographics, mechanism of inju

Systemic capillary leak syndrome associated with compartment syndrome and rhabdomyolysis.

Systemic capillary leak syndrome (SCLS) is a rare disorder characterized by recurrent spontaneous episodes of hypovolaemic shock due to marked plasma shifts from the intravascular to the extravascular space. This presents as the characteristic triad of hypotension, haemoconcentration and hypoalbuminemia often with an associated monoclonal gammopathy. We describe a patient with SCLS who required aggressive fluid resuscitation and emergency fasciotomies for compartment syndrome with rhabdomyolysis. At presentation the patient was considered to have severe erythrocytosis and was therefore initial

Acute Compartment Syndrome in Congenital and Acquired Haemophilia.

BACKGROUND: Acute compartment syndrome (ACS) is a surgical emergency defined as an increase in intramuscular pressure within a compartment that compromises capillary perfusion and threatens the survival of compartment tissues. A sustained increase in compartment pressure causes irreversible necrosis of nerves and muscles, leading to permanent damage. PURPOSE: To conduct a narrative review of the literature on the diagnosis and treatment of ACS in persons with congenital and acquired haemophilia (PwH) METHODS: An English-language literature search for articles published before 28 February 2024

A case of bone tumor-induced compartment syndrome.

Compartment syndrome is a rare critical condition that can arise in individuals with cancer, presenting with significant challenges in diagnosis and management. Compartment syndrome occurs when the pressure within a closed fascial space rises to a point that restricts circulation. A 56 year-old male patient presented with 2 days of pain and swelling in the right upper extremity pain. Physical examination was remarkable for right upper extremity erythema swelling and tense compartments, concerning for compartment syndrome. Humerus X-ray showed moth eaten appearance of mid humerus with periostea

Neonatal Limb Compartment Syndrome: A Comprehensive Review.

PURPOSE: Neonatal limb compartment syndrome (NLCS) is a rare and potentially limb-threatening condition defined as an increased pressure within a confined limb compartment. Clinicians may fail to distinguish NLCS from other mimicking conditions. Misdiagnosis is possible due to a low index of suspicion for this condition, which may delay appropriate and urgent treatment. A comprehensive review of the available literature was performed to explore common themes in NLCS and identify gaps in the evidence to guide future studies. METHODS: A literature search was conducted in PubMed, Scopus, and Goo

Chronic Exertional Compartment Syndrome in a Fire Captain.

Chronic exertional compartment syndrome (CECS) is a commonly missed diagnosis. It is caused by an increase in intramuscular pressure which subsequently impedes local tissue perfusion and function. It disproportionately occurs in young females; however, the diagnosis should not be excluded in other demographics. We present a case of CECS in an otherwise healthy 53-year-old male fire captain. He presented with pain upon exertion and neurological deficits in the anterior compartment of his bilateral legs that impacted his occupation and daily functioning. Following fasciotomy, the patient returne

Phlegmasia cerulea dolens causing compartment syndrome.

BACKGROUND: Phlegmasia cerulea dolens (PCD) is a rare condition characterized by a severely swollen, cyanotic, blue extremity due to a large proximal (iliofemoral) deep venous thrombosis extending into the collateral veins. Mortality in PCD ranges 20-40%. Due to severely compromised venous drainage, compartment pressures can rapidly increase 16-fold within 6 h, but rarely result in arterial compromise. CASE REPORT: We present a case of a middle-aged woman with no prior history of deep venous thrombosis, with a blue swollen left leg in intractable severe pain unresponsive to 3 doses of hydromo
