Case Report: Early Onset Systemic Lupus Erythematosus Due to Hereditary C1q Deficiency Treated With Fresh Frozen Plasma.

Background: Hereditary C1q deficiency is associated with early-onset autoimmunity causing SLE or SLE-like disease as well as increased risk for infections with encapsulated bacteria. It is a rare genetic condition inherited in an autosomal recessive manner, caused by mutations in C1q genes. Treatment and management of this rare disease are very complex and include prophylactic vaccination, antibiotics, and immunosuppressive drugs. There are two possible modalities for the replacement of the missing protein: regular fresh frozen plasma (FFP) administration and allogeneic hematopoietic stem cell

Clinical and prognostic significance of glomerular C1q deposits in primary MN.

BACKGROUND: Although complement activation is believed to be important in mediating PMN, the pathways involved and clinical consequences remain controversial. Many cases of idiopathic or primary membranous nephropathy (PMN) present with subepithelial C1q deposits along with IgG and C3 on glomerular capillary walls but without deposits of IgA or IgM ("full house") by immunofluorescence or any causes of secondary MN. We sought to define the clinical and pathological significance of these C1q deposits in PMN by comparing a variety of clinical parameters, outcomes and other serum and urine factors

Intrathecal complement activation by the classical pathway in tick-borne encephalitis.

Tick-borne encephalitis (TBE) is one of the most prevalent viral central nervous system (CNS) infections in Eurasia and neurological sequelae are common. The immune responses are considered crucial for the pathogenesis. The aim of this study was to explore the activation of the complement system in TBE. The complement system is a part of the innate immune response in the CNS, which previously has been reported to be activated in other flavivirus infections. We analyzed complement factors in 44 paired cerebrospinal fluid (CSF) and serum samples from 20 cases of TBE in the acute and later stages

The complement system and human autoimmune diseases.

Genetic deficiencies of early components of the classical complement activation pathway (especially C1q, r, s, and C4) are the strongest monogenic causal factors for the prototypic autoimmune disease systemic lupus erythematosus (SLE), but their prevalence is extremely rare. In contrast, isotype genetic deficiency of C4A and acquired deficiency of C1q by autoantibodies are frequent among patients with SLE. Here we review the genetic basis of complement deficiencies in autoimmune disease, discuss the complex genetic diversity seen in complement C4 and its association with autoimmune disease, pr

Targeting microglial C1q alleviates blood-brain barrier disruption in the thalamus after cortical infarction.

Focal cerebral infarction induces secondary damages in non-ischemic remote brain regions, severely affecting the prognosis and life quality of stroke patients. Disruption of blood–brain barrier (BBB) contributes to secondary thalamic damage after distal middle cerebral artery occlusion (dMCAO). However, the underlying molecular mechanisms that initiate BBB disruption and aggravate secondary thalamic damage after dMCAO remain unclear. It has been well-established that the complement pathway involves in the maintaining BBB integrity in the brain. Herein, using bulk RNA-sequencing and proteomics,

Activation of the Complement System in Patients with Cancer Cachexia.

Systemic inflammation is thought to underlie many of the metabolic manifestations of cachexia in cancer patients. The complement system is an important component of innate immunity that has been shown to contribute to metabolic inflammation. We hypothesized that systemic inflammation in patients with cancer cachexia was associated with complement activation. Systemic C3a levels were higher in cachectic patients with inflammation (n = 23, C-reactive protein (CRP) ≥ 10 mg/L) as compared to patients without inflammation (n = 26, CRP < 10 mg/L) or without cachexia (n = 13) (medians 102.4 (IQR 89.4

Development and Characterization of a Multiplex Assay to Quantify Complement-Fixing Antibodies against Dengue Virus.

Antibodies capable of activating the complement system (CS) when bound with antigen are referred to as "complement-fixing antibodies" and are involved in protection against Flaviviruses. A complement-fixing antibody test has been used in the past to measure the ability of dengue virus (DENV)-specific serum antibodies to activate the CS. As originally developed, the test is time-consuming, cumbersome, and has limited sensitivity for DENV diagnosis. Here, we developed and characterized a novel multiplex anti-DENV complement-fixing assay based on the Luminex platform to quantitate serum antibodie

Complement inhibitor C4b-binding protein interacts directly with small glycoproteins of the extracellular matrix.

Components derived from cartilage have been suggested to maintain the inflammation in joints in arthritis. Small leucine-rich repeat proteins (SLRPs) are structural components of cartilage important in organizing the meshwork of extracellular matrix components. It has recently been shown that the SLRP fibromodulin interacts with complement initiator C1q, leading to complement activation. The complement response is limited since fibromodulin also interacts with the complement inhibitor factor H. We have now found that osteoadherin, chondroadherin, fibromodulin, and proline arginine-rich end leu

Systemic and pulmonary C1q as biomarker of progressive disease in experimental non-human primate tuberculosis.

Tuberculosis (TB) causes 1.6 million deaths annually. Early differential diagnosis of active TB infection is essential in optimizing treatment and reducing TB mortality, but is hampered by a lack of accurate and accessible diagnostics. Previously, we reported on complement component C1q, measured in serum by ELISA, as a candidate biomarker for active tuberculosis. In this work we further examine the dynamics of C1q as a marker of progressive TB disease in non-human primates (NHP). We assessed systemic and pulmonary C1q levels after experimental infection using high or low single dose as well a

Lumbar degenerative disease: association of serum complement levels with diagnosis, structural severity, and postoperative prognosis.

PURPOSE: The clinical evaluation of lumbar degenerative disease (LDD) severity relies on symptoms and imaging, lacking biologically specific diagnostic markers. This study investigated the role of serum complements in LDD to assess their potential as diagnostic biomarkers, their correlation with anatomical stenosis, and their ability to predict postoperative recovery. We hypothesized that specific serum complement levels correlate with both structural severity and clinical outcomes. METHODS: A prospective investigation was conducted on 82 patients with LDD and 90 healthy controls. To minimize

Complement Dysregulation During the Early Phases of Synucleinopathy.

Parkinson's disease (PD) is characterized by progressive degeneration of nigrostriatal dopamine neurons and synucleinopathy, which is the accumulation of aggregated α-synuclein (α-syn). Increasing evidence implicates α-syn-associated neuroinflammation as a contributor to PD pathogenesis; however, immune mechanisms linking synucleinopathy to neurodegeneration remain incompletely defined. Activation of the complement cascade occurs in PD and other neurodegenerative disorders, but most studies report complement activation after overt neurodegeneration, making it difficult to conclude if complemen

Could DTI Unlock the Mystery of Subjective Tinnitus: It's Time for Parameters That Go A Little Out of the Routine.

OBJECTIVES: Immune complexes (ICs) from sera of juvenile idiopathic arthritis (JIA) patients show increased complement opsonisation; however, a definitive role for involvement of the classical or alternative pathway is not entirely clear. To delineate the role of these pathways, we measured activated complement products bound to circulating IC (CICs) in the sera of JIA patients. METHODS: Sera from 100 JIA patients and 22 healthy children were collected. C1q, C4, C3, C3d, and membrane attack complex (MAC) bound to CICs were measured by enzyme-linked immunosorbent assay. Data was compared to Ig

First component of complement and thrombosis.

The previous demonstration of an association between the collagen-like region of C1q (subcomponent of the first component of complement) and human platelets and of the inhibitory properties of C1q on platelet adhesion and aggregation to or by collagen has led us to consider the first component of complement as a possible modulator of the platelet-collagen interaction. Since collagen represents a major component of the vessel wall, the modification of C1 could play a role in the development of thrombosis. We have therefore studied two patients with abnormalities of complement (low or undetectab

Infiltrating dendritic cells contribute to local synthesis of C1q in murine and human lupus nephritis.

Lupus nephritis causes morbidity and mortality in patients affected by Systemic Lupus Erythematosus (SLE). Recent data have shown that dendritic cells (DC) play a central role in SLE pathogenesis, by enhancing the presentation of auto-antigens and the induction of autoimmunity. In this paper we demonstrated in a mouse model of progressive lupus nephritis that C1q, the recognition unit of complement classical pathway, is locally produced in the kidney. This local renal synthesis of C1q increased in a time dependent manner in accordance with the recruitment of infiltrating MHC II+ antigen presen

Complement dysregulation in human tauopathies.

Dysregulation of the complement system plays an important role in the pathogenesis of neurodegenerative diseases, including Alzheimer's disease (AD). In post-mortem AD brains, complement is deposited in and around amyloid plaques, and peri-plaque complement activation drives synapse loss in AD mouse models. Studies to date have focused on amyloid pathology; however, aggregated tau is also involved in neuronal loss in AD. Primary tauopathies are characterised by tau pathology in the absence of amyloid. The role of complement in human tauopathies remains largely unexplored. Here, we address this

Immunofluorescence studies on C1Q in mast cells.

Evidence has been obtained of deposition of complement component C1q in mast cells, by the immunofluorscence technique, using biopsy specimens from the scalp from 5 of 7 normal subjects and 14 of 17 patients with alopecia areata and also from skin lesions of 1 of 2 patients with urticaria pigmentosa studied. The fluorescence in mast cells was of a granular pattern in most instances. No deposition of immunoglobulins (IgG, IgA, IgM, IgE) or other complement components (C3, C4, C5, C9) was demonstrable in the mast cells. The specificity of the C1q deposition in mast cells was confirmed by various

Activation of the classical complement pathway by Bacillus anthracis is the primary mechanism for spore phagocytosis and involves the spore surface protein BclA.

Interactions between spores of Bacillus anthracis and macrophages are critical for the development of anthrax infections, as spores are thought to use macrophages as vehicles to disseminate in the host. In this study, we report a novel mechanism for phagocytosis of B. anthracis spores. Murine macrophage-like cell line RAW264.7, bone marrow-derived macrophages, and primary peritoneal macrophages from mice were used. The results indicated that activation of the classical complement pathway (CCP) was a primary mechanism for spore phagocytosis. Phagocytosis was significantly reduced in the absence

Intraocular complement activation is independent of systemic complement activation and is related to macular vascular remodelling in retinal vein occlusion.

BACKGROUND: Retinal vein occlusion (RVO) is a major cause of vision loss. The pathogenesis remains poorly defined although inflammation is known to play a critical role. In this study, we investigated the levels of complement proteins in the aqueous humour and plasma of RVO participants and the relationship between complement levels and retinal pathologies. METHODS: The plasma and aqueous humour were collected from 20 treatment-naive RVO and 20 cataract patients. Retinal lesions were examined by fundus stereoscopy and optical coherence tomography angiography. The levels of C1q, C2, C4, C4b, C

Direct Quantification of Complement Biomarkers Using Liquid Chromatography-Tandem Mass Spectrometry (LC-MS/MS).

The classical complement pathway is a critical therapeutic target for numerous diseases, but the lack of robust, translatable biomarkers hinders drug development. Here, we report the development and qualification of a quantitative liquid chromatography-tandem mass spectrometry (LC-MS/MS) platform to measure key classical pathway proteinsC1q, C 1s (total and active), and the native C1q/C 1s complex. The platform was established across human, cynomolgus monkey, and mouse samples, including serum, plasma, cerebrospinal fluid, and brain homogenates. We employed two complementary workflows: an aff
