Conjunctivitis: Diagnosis and Management.

Conjunctivitis caused by viruses, bacteria, or allergies is one of the most common eye conditions in primary care. There is no single sign or symptom that accurately differentiates viral from bacterial conjunctivitis. A comprehensive history and physical examination can guide diagnosis. Viral and allergic conjunctivitis are more common in adults and typically present with watery discharge. Supportive care options for viral conjunctivitis include artificial tears, cold compresses, and antihistamine eye drops. Strict personal hygiene, including frequent handwashing, is essential to decrease the

Exclusion of students with conjunctivitis from school: policies of state departments of health.

PURPOSE: To use current state department of health regulations regarding exclusion of students with conjunctivitis from school as a starting point in developing uniform recommendations for schools. METHODS: State departments of health were asked to state their policy regarding when a child with conjunctivitis may return to school. This information was collated and examined for trends. The results were compared with current literature on infectious conjunctivitis. RESULTS: Of the 43 states that responded, 7 allow children with conjunctivitis to remain in school, 8 allow their return once anti

Conjunctivitis in infants and children.

BACKGROUND: Acute bacterial conjunctivitis, chiefly affecting young children, is generally caused by one of three common pathogens, Haemophilus influenzae, Streptococcus pneumoniae and adenovirus. H. influenzae is the most prevalent causative organism. Some patients presenting initially with conjunctivitis subsequently develop acute otitis media, with H. influenzae the most common etiologic agent of what has been termed the "conjunctivitis-otitis" syndrome. Optimal treatment of acute conjunctivitis would result in a clinical and bacteriologic cure of the conjunctivitis and the prevention of th

Epidemiology and diagnosis of acute conjunctivitis at an inner-city hospital.

Chlamydia has been found to be the cause of a high percentage of cases of neonatal and chronic conjunctivitis in an inner-city population. To assess the etiology and epidemiology of acute conjunctivitis in this population, conjunctival scrapings were sampled from 45 patients presenting to an ocular emergency room; and replicate chlamydial, viral, and bacterial cultures and cytology tests were done. No cases of chlamydial conjunctivitis were diagnosed, although a viral etiology was established in 36% of the cases and a bacterial etiology in 40%. Twenty-four percent of the cases were not diagnos

Cytological and microbiological evaluation of conjunctiva in camels with and without conjunctivitis.

OBJECTIVE: To describe the cytological analysis of conjunctiva from normal camels and camels with bacterial conjunctivitis. ANIMALS STUDIED: This study was conducted on 7 normal camels and 15 camels affected with conjunctivitis. The affected camels had a history of conjunctivitis with signs including chemosis, blepharospasm, frequent blinking, and mild-to-moderate serous, mucoid, or purulent ocular discharge. PROCEDURES: Bacterial swabs were collected from the inferior conjunctival sac of the affected eye without topical anesthetics. Conjunctival smears were obtained from the conjunctival su

Epidemiology of neonatal conjunctivitis.

Chlamydial conjunctivitis was diagnosed in 46 of 100 infants less than two months old presenting with conjunctivitis. Streptococcus, Staphylococcus, and Hemophilus species were most frequently identified in the remainder. All infants with chlamydial conjunctivitis were black and were born by vaginal delivery (P less than 0.001). Average age at presentation was 13 days for chlamydial and 21 days for other causes of bacterial conjunctivitis (P less than 0.001) with symptoms present an average of five to six days prior to presentation. There was no statistically significant difference in sex, bir

Effect of bacterial secondary infection in an animal model of trachoma.

In trachoma the interaction between chronic chlamydial and acute bacterial conjuntivitis has been suggested as important in determining the severity of disease and, therefore, blindness. We investigated the effect of acute conjunctival infection with each of three common human pathogens, Haemophilus influenzae, Haemophilus aegyptius , and Streptococcus pneumoniae, in a model of trachoma established in cynomolgus monkeys. Although acute conjunctivitis developed, animals with trachoma were not more susceptible to infection than other monkeys, nor did they develop more severe disease as a result

Characterization of Polybacterial versus Monobacterial Conjunctivitis Infections in Pediatric Subjects Across Multiple Studies and Microbiological Outcomes with Besifloxacin Ophthalmic Suspension 0.6.

INTRODUCTION: The choice of empiric therapy for bacterial conjunctivitis should be guided by an awareness of typical causative pathogen distributions. Bacterial conjunctivitis can be polybacterial, although pediatric-specific data are lacking. METHODS: This was a post-hoc analysis of data in pediatric subjects (1-17 years) from five bacterial conjunctivitis trials evaluating besifloxacin ophthalmic solution 0.6%. RESULTS: Of the 730 pediatric subjects with culture-confirmed conjunctivitis, nearly one-fourth (23.6%) had polybacterial infections and three-fourths (76.4%) had monobacterial infe

A Review of the Differential Diagnosis of Acute Infectious Conjunctivitis: Implications for Treatment and Management.

The diagnosis of acute infectious conjunctivitis can be difficult. Clinical ambiguity exists between the acute viral and bacterial as well as the allergic forms, which can confound diagnosis. Also, discrimination between viral or bacterial origins of infectious conjunctivitis based on historical, nonspecific, clinical signs and symptoms contributes to a high rate of misdiagnosis and overuse of antibiotic treatment. Therefore, in order to effectively treat acute infectious conjunctivitis, physicians must be aware of the clinical signs and symptoms and available diagnostic tests that can provide

Lacrimal canaliculitis as a cause of recurrent conjunctivitis.

BACKGROUND: Bacterial conjunctivitis is routinely managed by optometrists. This case report describes a case of lacrimal canaliculitis, which is a cause for nonresolving conjunctivitis and is commonly misdiagnosed. CASE REPORT: A 48-year-old man presented with a 5-month history of chronic conjunctivitis in the right eye. This was diagnosed as bacterial in etiology but had a short-lived response to various topical antibiotics. Examination found signs of canaliculitis, and Actinomyces israelii was cultured from the discharge. This was treated successfully by canaliculotomy with curettage follow

Neonatal conjunctivitis at a Brazilian tertiary center: the current relevance of Chlamydia trachomatis.

PURPOSE: The microbiology pattern of neonatal conjunctivitis has changed over time, and the incidence of gonococcal conjunctivitis is almost nil. This study aimed to determine the etiology of neonatal conjunctivitis cases referred to a tertiary health center in Brazil. METHODS: From 2017 to 2020, conjunctival swabs were taken from neonates with clinical signs of conjunctivitis and tested with bacterial culture and polymerase chain reaction for Neisseria gonorrhoeae and Chlamydia trachomatis. RESULTS: A total of 51 neonates were included in the 3-year study. Chlamydial conjunctivitis was diag

[Primary meningococcal conjuctivitis with the development of a systemic disease].

Neisseria meningitidis is a rare cause of acute bacterial conjunctivitis. Systemic meningococcal disease follows meningococcal conjunctivitis in approximately one quarter of patients. Systemic antibiotic treatment is indicated in the case of meningococcal conjunctivitis to prevent spread of infection. We report 6-month-old boy who presented with meningococcal conjunctivitis and developed sepsis and meningitis in 24 hours. The course of the disease was favorable.

Conjunctivitis due to Phthirus pubis: a trap for the unwary.

It is not uncommon for patients to present to eye casualties with a red eye and where treatment has already been started by the general practitioner, usually in the form of antibiotic drops or ointment for presumed bacterial/viral conjunctivitis. Occasionally, the diagnosis is not simply a bacterial conjunctivitis and this can only be clarified by close ocular inspection.

Immunoassay of tear lysozyme in conjunctival diseases.

The tear lysozyme content in 111 normal subjects and in 159 patients with various conjunctival diseases was determined by a single radial immunodiffusion technique. Tear lysozyme level in normal people was 1.33/mg/ml. (SI conversion: mg/ml = g/l.) The mean tear lysozyme levels in patients with chronic irritative conjunctivitis (0.97 mg/ml) and nutritional deficiency with epithelial xerosis (0.76 mg/ml) were significantly lower than in the normal controls. The mean tear lysozyme levels in tears from patients with vernal conjuctivitis (1.20 mg/ml), phlyctenular conjunctivitis (1.10 mg/ml), and a

[Summary of the practice guideline 'The red eye' (first revision) of the Dutch College of General Practitioners (NHG)].

The revised NHG-guideline 'The red eye' provides recommendations for the diagnosis and therapy in patients with a red eye. In the presence of pain, decreased visual acuity and photophobia (alarm symptoms) should be considered as sight threatening conditions. In most instances a red eye results from conjunctivitis. The complaint of (an) early morning glued eye(s) makes a bacterial origin of acute infectious conjunctivitis more likely. Itching and a history of infectious conjunctivitis make the probability of bacterial involvement less likely. The type of discharge does not help to adequately di
