Ethical analyses of institutional measures to increase health care worker influenza vaccination rates.

Health care worker (HCW) influenza vaccination rates are modest. This paper provides a detailed ethical analysis of the major options to increase HCW vaccination rates, comparing how major ethical theories would address the options. The main categories of interventions to raise rates include education, incentives, easy access, competition with rewards, assessment and feedback, declination, mandates with alternative infection control measures, and mandates with administrative action as consequences. The aforementioned interventions, except mandates, arouse little ethical controversy. However, t

[Complexities of the stress experienced by employees of the Fukushima nuclear plants].

Fukushima Daiichi Nuclear Power Plants suffered serious damage by the Great East Japan Earthquake and Tsunami. The employees of the plant worked very hard to stabilize the nuclear reactor and to prevent any secondary accidents. They were in one of the most severe situations in this disaster, but they were the people who hesitated most to request help for themselves. We started visiting the Fukushima Daini Plant office that was used as the frontline base for Daiichi Plant workers since July, 2011. These visits were held once or twice a month and we offered mental health support to the employees

Pre-Exposure Prophylaxis for HIV: Bioethical, Clinical, and Epidemiological Considerations.

Public health authorities are broadly promoting a strategy known as pre-exposure prophylaxis (PrEP) for the prevention of human immunodeficiency virus (HIV) transmission in the context of high-risk sexual activity and injection drug use. However, there are several limitations to this strategy that are underrecognized. This article reviews the primary literature supporting the use of PrEP and explores the unintended consequences associated with its use. Current public health messaging indicates that PrEP reduces the risk of HIV transmission during sex by 99 percent; however, this figure is base

Addressing global health disparities in the management of RSV infection in infants and children: Strategies for preventing bronchiolitis and post-bronchiolitis recurrent wheezing.

Background: The topic of equitable access to health care and its impact on exacerbating worldwide inequities in child health not only strikes at the heart of our health-care delivery systems but also deeply resonates with our collective social consciences. Nowhere is this better seen on a global scale than in the burden of illness caused by respiratory syncytial virus (RSV) infection, which extracts the most severe morbidity and mortality in infants and children in low- and middle-income countries (LMIC). This report addresses global health disparities that exist in the management of RSV infec

A classification of risk factors in serious juvenile offenders and the relation between patterns of risk factors and recidivism.

BACKGROUND: There has been a lot of research on risk factors for recidivism among juvenile offenders, in general, and on individual risk factors, but less focus on subgroups of serious juvenile offenders and prediction of recidivism within these. OBJECTIVE: To find an optimal classification of risk items and to test the predictive value of the resultant factors with respect to severity of recidivism among serious juvenile offenders. METHOD: Seventy static and dynamic risk factors in 1154 juvenile offenders were registered with the Juvenile Forensic Profile. Recidivism data were collected on

The origins of American health libertarianism.

This Article examines Americans' enduring demand for freedom of therapeutic choice as a popular constitutional movement originating in the United States' early years. In exploring extrajudicial advocacy for therapeutic choice between the American Revolution and the Civil War, this piece illustrates how multiple concepts of freedom in addition to bodily freedom bolstered the concept of a constitutional right to medical liberty. There is a deep current of belief in the United States that people have a right to choose their preferred treatments without government interference. Modern American his

Bacterial meningitis complicating the course of liver cirrhosis.

UNLABELLED: Meningitis is rarely reported in studies investigating bacterial infections in patients affected by liver cirrhosis. We investigated the findings of bacterial meningitis in patients affected by liver cirrhosis referred to our department in a 16-year period. MATERIALS: Patients with cirrhosis and bacterial meningitis were enrolled. Cirrhosis was defined by liver histology or clinical, laboratory, and ultrasonographic and endoscopic findings. Bacterial meningitis was defined by cerebro-spinal fluid pleocytosis (>10/mcl) and characteristic clinical presentation. Fisher exact test and

E. coli multiresistant meningitis after transrectal prostate biopsy.

Escherichia coli meningitis is a frequent pathology in children younger than 3 years old, but is an uncommon disease in adults. E. coli infection is the main cause of intrahospital bacteremia as a consequence of the employment of different medical procedures. Our patient, male, 69 years old, presented with fever, progressive difficulty in breathing, and shivers 24 h after transrectal prostate biopsy, with an absence of any other symptoms. He received prophylactic treatment with ciprofloxacin and later empirical treatment with ampicillin and tobramicin. After that, the patient presented with fe

What Children Eat in Developing Countries: Diet in the Etiology of Undernutrition?

We live in a world of paradoxes: one half of the world is overweight or obese with abundant access to food and the other half undernourished due to poor food access. It has been estimated that nearly 36% of children in Asia are stunted. Worldwide, approximately 1 in 4 children under the age of 5 years shows stunted growth, making the number of stunted children to 155 million; 52 million children under 5 years are wasted, of which 17 million are severely wasted. Given the wide-scale nature of global malnutrition and its long-term consequences, why has this scar in our consciousness not being ad

The change in motivating factors influencing commencement, adherence and retention to a supervised resistance training programme in previously sedentary post-menopausal women: a prospective cohort study.

BACKGROUND: Understanding motivators for exercise participation in post-menopausal women may impact retention to exercise programmes and inform intervention trial designs. The purpose of this investigation was to assess self-reported motivational factors influencing adherence and retention to a 24-week progressive resistance training programme. METHODS: Post-menopausal females (n = 34) were passively recruited to undertake a 24-week progressive resistance training protocol, in small-group sessions, on three non-consecutive days of the week. Attendance was recorded by the researcher. Qualitati

Mutant superoxide dismutase 1 causes motor neuron degeneration independent of cyclin-dependent kinase 5 activation by p35 or p25.

AIM: The aim of the present study was to explore food-related health perceptions and food habits among older women. BACKGROUND: Food-related health promotion has its main focus on disease prevention. With a holistic perspective on health, social and psychological aspects also need to be considered for total wellbeing. METHODS: Qualitative interviews, representing an ethnographic approach, were carried out with 18 women, aged 65-88, living alone or cohabiting, who independently managed shopping and cooking. Interviews were conducted at the women's homes, and analysed for coherent themes. The

Religion and conscientious objection: a survey of pharmacists' willingness to dispense medications.

Some US states allow pharmacists to refuse to dispense medications to which they have moral objections, and federal rules for all health care providers are in development. This study examines whether demographics such as age, religion, gender influence 668 Nevada pharmacists' willingness to dispense or transfer five potentially controversial medications to patients 18 years and older: emergency contraception, medical abortifacients, erectile dysfunction medications, oral contraceptives, and infertility medications. Almost 6% of pharmacists indicated that they would refuse to dispense and refus

Observational quality control study: insourcing multi-PCR-impact on the use of anti-infectives for patients with pleocytosis.

BACKGROUND: An analysis of the cerebrospinal fluid (CSF) is essential for diagnosis of meningitis, headache, disturbance of conscience, cranial nerves or autoimmune-related conditions of the CNS. The initial treatment of pleocytosis usually consists of both antiviral therapy and antibiotics until laboratory results enable a more specific approach. Therefore, it is crucial to rapidly and accurately detect pathogens. METHODS: In this observatory, monocentric study of quality management data, we studied insourcing of ME-PCR, CXCL 13, Antibody-specific Index (AI) for HSV, VZV (G 1 ) compared w

Adolescent conduct disorder and interpersonal callousness as predictors of psychopathy in young adults.

Unfortunately, very little research has examined the link between antisocial personality traits in childhood and adult psychopathy. This study used data from a clinic-referred sample of 177 boys, assessed annually from recruitment (ages 7 to 12) through age 19. Parent and teacher ratings of interpersonal callousness (IC) were tested at predictors of psychopathy ratings at 18 and 19. In regression models, conduct disorder (CD) and teacher-rated IC both predicted both Factor 1 (interpersonal and affective items) and Factor 2 (impulsivity and antisocial behavior items) of the Psychopathy Checklis

Acute Confusional Migraine: Case Reports and Discussion as a Distinct Entity.

Background: Acute confusional migraine (ACM) is a migraine variant manifesting with confusion, agitation, disorientation, altered mental status, and/or memory deficits. ACM has notably been excluded from the International Classification of Headache Disorders (ICHD-3 β), despite previous literature describing 120 cases and proposing a standardized classification. Considering these findings, clinicians must be aware of this condition as it can be confounded with other serious health conditions (e.g., stroke, encephalitis, and epilepsy). Objective: Herein, we describe three cases with altered con

Bloodless Total Hip Arthroplasty in Jehovah's Witness Patients: Contemporary Strategies and Outcomes.

BACKGROUND: Reported blood transfusion rates in total hip arthroplasty (THA) range between 3 and 22%. Jehovah's Witnesses (JW) do not accept blood transfusions and make conscience decisions to accept blood derivatives. This study reports on strategies and outcomes for bloodless THA. METHODS: All JW patients undergoing primary THA at our institution between 2011 and 2022 were included in this study (94 of 110 THA). The indications for THA were osteoarthritis (92%), femoral neck fracture (6%), rheumatoid arthritis (1%), and failed open reduction and internal fixation (1%). Strategies used to op

Interpersonal ambivalence in obsessive-compulsive disorder.

BACKGROUND: The social attitudes and interpersonal relationships of patients with obsessive-compulsive disorder (OCD) are subject to a longstanding controversy. Whereas cognitive-behavioural researchers emphasize exaggerated pro-social attitudes in OCD like inflated responsibility and worry for other people (especially significant others), dynamic theories traditionally focus on anti-social attitudes such as latent aggression and hostility. In two recent studies, we gathered support not only for a co-existence of these seemingly opposing attitudes in OCD, but also for a functional connection:
