Steroid-eluting contact lenses for corneal and intraocular inflammation.

Ocular inflammation is one of the leading causes of blindness worldwide, and steroids in topical ophthalmic solutions (e.g. dexamethasone eye drops) are the mainstay of therapy for ocular inflammation. For many non-infectious ocular inflammatory diseases, such as uveitis, eye drops are administered as often as once every hour. The high frequency of administration coupled with the side effects of eye drops leads to poor adherence for patients. Drug-eluting contact lenses have long been sought as a potentially superior alternative for sustained ocular drug delivery; but loading sufficient drug i

Endotoxin concentration in contact lens storage cases.

BACKGROUND: The contamination of contact lens storage cases by gram-negative bacteria has been associated with ulcerative keratitis. This study investigated the concentration of endotoxin, a substance produced by gram-negative bacteria, in contact lens cases. METHODS: The limulus amebocyte lysate (LAL) test was used to measure the concentration of endotoxin in the storage cases of 27 contact lens wearers. The units of concentration used were endotoxin units per ml (EU/ml). The type of storage solution used by each patient, as well as other aspects of lens care and use, were recorded. RESULTS

The impact of inoculation parameters on the pathogenesis of contact lens-related infectious keratitis.

PURPOSE: Contact lens wear predisposes to Pseudomonas aeruginosa keratitis, but the mechanisms involved remain unclear. An in vivo model was used to study lens inoculation conditions enabling disease. METHODS: Custom-made hydrogel contact lenses were fitted to rats after incubation in P. aeruginosa approximately 10(11) cfu/mL (3 hours) or approximately 10(3) cfu/mL (24 hours). Another group was inadvertently inoculated with a suction pen previously used with high inocula, but rinsed in ethanol and stored dry (6 months). Some corneas were tissue paper-blotted to cause fluorescein staining befo

Thimerosal and the relevance of patch-test reactions in children.

Thimerosal (C(9)H(9)HgNaO(2)S) is a mercuric derivative of thiosalicylic acid that goes by many names. It has been used as a disinfectant (eg, Merthiolate) and as a preservative in some vaccines, cosmetics, tattoo inks, eye drops, and contact lens solutions. It remains one of the least likely chemicals to cause contact allergy and for this reason has been removed from many standard screening series. Positive patch-test reactions (demonstrating sensitization) may still be seen (especially in older children); however, these reactions are not clinically relevant and are expected to be fewer, refl

Corneal ulcer associated with contamination of aerosol saline spray tip.

PURPOSE: To report a complication of aerosol saline use in a contact lens wearer. METHODS: Case report. A 57-year-old woman who used soft contact lenses on an extended-wear basis developed a unilateral Pseudomonas corneal ulcer associated with the use of preservative-free aerosol saline. RESULTS: The solution inside the aerosol can was free of microbial contamination. The spray tip, however, was contaminated with P aeruginosa. CONCLUSIONS: Aerosol spray cans may minimize contamination of the solution inside the can. The spray tip is still susceptible to microbial contamination, especially w

Infectious keratitis associated with contact lens wear: REGINFECOR multicenter study.

This 32-centre multicentre study addresses the lack of knowledge about the prevalence and significance of microbial keratitis (MK) associated with contact lens (CL) wear in Spain. A total of 304 cases recruited from 32 hospitals were studied and showed that infectious keratitis associated with contact lens wear mainly affects young women during the summer months. In this study, soft lenses with monthly replacement and single solution cleaning were most commonly used, purchased and fitted in opticians' shops. Common risk factors were identified among users, such as topping off solutions, prolon

Contact lens considerations for patients infected with HIV.

BACKGROUND: As an eye care practitioner, one must be aware of the many sequelae of HIV disease and its impact on contact lens patients. With the progressing development of anti-HIV medications, patients infected with HIV are living longer and more productive lives. Thus a greater number of patients infected with HIV may be candidates for contact lens wear. PURPOSE: The eye care practitioner will play an important role in caring for patients infected with HIV who wear contact lenses. Theoretically, contact lens wear in patients infected with HIV could potentially increase their risk of corneal

Fusarium keratitis and endophthalmitis associated with lens contact wear.

INTRODUCTION: Fusarium keratitis is a severe corneal infection that is usually seen in tropical and subtropical countries after a corneal trauma. In 2005-2006, an epidemic of Fusarium keratitis, occurring predominantly among contact lens wearers, was observed in several countries. CASE REPORT: We describe the clinical course of a Fusarium keratitis which failed to respond to systemic and local voriconazole treatment, and experienced a progression to a severe keratitis with endophthalmitis, requiring early therapeutic keratoplasty ("à chaud"). After 8 months of follow up, the vision recovered

Microwave disinfection of soft contact lenses.

We evaluated the effectiveness and convenience of microwave irradiation as a method of disinfecting soft contact lenses. Soft contact lenses from each of the four Food and Drug Administration (FDA) categories were placed in sterile vials and immersed in 2 ml of saline which had been contaminated with one of three common species of bacteria. The contaminated lens vials were placed in a standard 600 W microwave oven and exposed to microwave irradiation times ranging from 30 to 180 s. Significant reductions in bacteria colony counts were found after 30 s of microwave irradiation. Few of the bacte

Advances in the management of Acanthamoeba keratitis: A review of the literature and synthesized algorithmic approach.

Acanthamoeba keratitis (AK) is a severe cause of infectious keratitis and represents a significant clinical challenge. Recent literature regarding AK epidemiology, diagnosis, treatment modalities, and prognosis is reviewed and synthesized to propose an algorithmic protocol for AK management. Globally, AK outbreaks in developed countries are ongoing, and AK rates have increased. Moreover, current outbreaks may carry a worse prognosis than prior outbreaks. Despite identification of contact lens solutions implicated in AK outbreaks and the consequent market recall of these products, outbreaks per

Epidemiology of Pseudomonas aeruginosa keratitis in contact lens wearers.

This study evaluated the epidemiology of Pseudomonas aeruginosa keratitis in contact lens (CL) wearers; the relationships between CL storage case contamination and CL hygiene practice and between CL hygiene and the development of keratitis. Sixteen CL wearers with keratitis were compared with 44 asymptomatic controls. Lens hygiene practice was assessed and CL care materials, domestic water sites and endogenous sites were evaluated microbiologically. Poor CL hygiene was not associated with Ps. aeruginosa keratitis. There was an association between keratitis and bacterial contamination of the CL

[Persistent contact lens-associated keratitis].

A 43-year-old woman presented with a 2-month history of contact lens-associated keratitis and corneal ulceration on the left eye. Without having isolated the pathogenic agent, a calculated anti-infective and anti-inflammatory therapy had already been administered with no signs of improvement. Filamentous fungi (Acremonium sp) were found in the scraping material and the contact lens solution. A keratoplasty à chaud was necessary despite immediate initiation of pathogen-specific therapy. Early identification of the pathogen via corneal debridement and culturing are standard procedures in order t

CLEAR - Medical use of contact lenses.

The medical use of contact lenses is a solution for many complex ocular conditions, including high refractive error, irregular astigmatism, primary and secondary corneal ectasia, disfiguring disease, and ocular surface disease. The development of highly oxygen permeable soft and rigid materials has extended the suitability of contact lenses for such applications. There is consistent evidence that bandage soft contact lenses, particularly silicone hydrogel lenses, improve epithelial healing and reduce pain in persistent epithelial defects, after trauma or surgery, and in corneal dystrophies. Soft contact lenses as prospective transport systems for ophthalmology.

The time course of drug adsorption and desorption, exposure of soft contact lenses in a drug solution needed for maximum saturation, and the duration of lens wearing during which the drug was transported from the lens and exhibited its therapeutic activity were studied. The possibility of using computer simulation for the choice of material, carrier for the drug (myramistin hydrochloride), was evaluated. Due to use of this method, the volume of experimental studies could be reduced and the data systematized.

Contact lens-related infectious keratitis in Malaysia.

Fifty-six contact lens-related corneal ulcers (central in 32; hypopyon in 24 and stromal abscess in 6) were studied. Culture was positive in 78.9%. Corneal ulcers healed with intense antibiotic therapy in nearly all patients. Increased awareness of lens care/disinfection and frequent replacement of storage cases and solution, and early detection of pathogens and intensive appropriate antibiotic therapy are key points in management.
