OBJECTIVE: To introduce an arthroscopic "inlay" Bristow procedure based on the Mortise-Tenon joint structure concept using suture button fixation, and to evaluate its clinical and radiology results postoperatively with a minimal 3-year follow-up.
METHODS: A total of 56 patients who received arthroscopic "inlay" Bristow procedure with suture button fixation between June 2015 to June 2016 were eventually enrolled in this study. Radiological assessment on the 3D CT scan was performed preoperatively, immediately after operation, and postoperatively at the end of 3 months, 6 months and the final f

BACKGROUND: The inferior coracoid process has traditionally been considered to be the gold standard for glenoid augmentation after anteroinferior bone loss. Other autograft sites, and more recently, osteochondral allograft sites, have been described as potential donor sources.
PURPOSE: Potential autograft and osteochondral allograft sites were compared to identify the graft source that would provide the best fit for glenoid augmentation.
STUDY DESIGN: Controlled laboratory study.
METHODS: Mose circles, a geometric tool found on a standard goniometer, were used to make radius of curvature me

OBJECTIVE: The surgical procedure aims at anatomic reduction and stabilization of the acromioclavicular joint in vertical and horizontal planes for acute separations using a trans-clavicular and trans-coracoidal suture tape fixation with additional acromioclavicular joint augmentation with a PDS cord cerclage. For chronic instability adding a tendon graft is essential for sustainable stability.
INDICATIONS: Acute und chronic acromioclavicular joint separations type Rockwood III-VI. Recurrent AC-joint instability with intact coracoid process (with tendon graft).
CONTRAINDICATIONS: Acromioclav

PURPOSE: To determine the location of coracoid inferior tunnel exit with superior-based tunnel drilling and coracoid superior tunnel exit with inferior-based tunnel drilling.
METHODS: Fifty-two cadaveric embalmed shoulders (mean age 79 years, range 58-96 years) were used. A transcoracoid tunnel was drilled at the center of the base. Twenty-six shoulders were used for the superior-to-inferior tunnel drilling approach and 26 shoulders for the inferior-to-superior tunnel drilling approach. The distances to the margins of the coracoid process, from both the entry and exit points of the tunnel, we

PURPOSE: The aim of this study was to evaluate the outcome of a minimally invasive surgical technique for the treatment of patients with acromioclavicular joint dislocation.
METHODS: Sixteen patients with complete acromioclavicular joint dislocation were enrolled in this study. All patients were asked to follow the less active rehabilitation protocol post-operatively. Computed tomography with 3-D reconstruction of the injured shoulder was performed on each patient post operatively for the assessment of the accuracy of the suture anchor placement in the coracoid process and the reduction of th

The Latarjet procedure was first described in 1954. It is a nonanatomic procedure that requires transfer of the coracoid process, together with the conjoint tendon, to the anterior glenoid margin. The aim is to prevent recurrent anterior shoulder dislocation through a "triple blocking" effect, which includes restoring bone loss, providing a dynamic sling effect, and performing capsulolabral repair. Despite the long history of the Latarjet procedure, studies evaluating its impact on scapulothoracic kinematics are sparse. However, there is a concern that scapulothoracic dyskinesia may occur owin

BACKGROUND: Multiple disruptions of the superior shoulder suspensory complex (SSSC) involving more than two components are extremely rare. In some extreme situations, three components of the SSSC structure can be involved. The ideal treatment for this type of injury is debatable.
CASE PRESENTATION: A 21-year-old woman was referred to our emergency center following a traffic accident. A three-dimensional CT scan showed triple disruption of the SSSC involving concomitant ipsilateral fractures of the coracoid, the acromion, and the distal clavicle. The connection between the upper limber and the

BACKGROUND: The prevalence of nonunion after the Latarjet procedure remains high. The purpose of the present study was to investigate healing and clinical outcomes after a novel arthroscopic coracoid process transfer procedure.
METHODS: Sixty-three patients who underwent the arthroscopic inlay Bristow procedure combined with Bankart repair were identified, and 51 patients who met the inclusion criteria were enrolled in this study. The key feature of this technique was that the coracoid process was trimmed and fixed into a trough (5 to 10 mm deep) in the glenoid neck with a metal screw. Bone g

BACKGROUND: Infraclavicular brachial plexus block has been widely used for surgical procedures below the mid humerus owing to its excellent anesthetic quality and ease of practice. However, what is the optimal upper arm position for carrying out the procedure still lacks consensus of opinion. The primary goal of this study was to determine the optimal upper arm position for coracoid infraclavicular block by ultrasonographic examination.
METHODS: High-frequency (5-10 MHz) ultrasonographic examination on the vertical line 2 cm medial to the coracoid process was performed in 40 volunteers. We as

BACKGROUND: The prevalence of subscapularis (SSC) tendon tears is often underestimated. This negatively impacts the shoulder function because the SSC muscle is a powerful internal rotator. The primary aim of this study was to compare a blended clinical and radiological preoperative index of suspicion for SSC tears to the arthroscopic findings. The secondary aim was to compare the surgeon's and radiologist's index of suspicion to determine which is more accurate.
HYPOTHESIS: Analyzing a transverse image passing under the tip of the coracoid process, in combination with clinical examination, wi

BACKGROUND: Although previous literature has described the relevant anatomy for an open anterior Bankart approach of the shoulder, there is little known regarding the anatomic relationship changes in the neurovascular structures after an open Latarjet procedure.
PURPOSE: To define the neurovascular anatomy of the native shoulder in relation to the coracoid and to define the anatomy after the Latarjet procedure in relation to the glenoid to determine distances to these neurovascular structures with and without neurolysis of the musculocutaneous nerve (MCN) from the conjoint tendon.

Non-traumatic shoulder dislocation may occasionally result from seizure-related disorders, and, in rare cases, from underlying brain tumors. We report a 23-year-old man who presented with anterior shoulder dislocation without apparent trauma. Computed tomography revealed a large Hill-Sachs lesion, a bony Bankart lesion, and a coracoid process fracture - findings inconsistent with a low-energy mechanism of injury. Because the injury occurred during sleep, an epileptic seizure was suspected. Further neurological evaluation identified focal epilepsy originating from the right frontoparietal regio

Most of the operative techniques described for the treatment of recurrent anterior dislocations or subluxations of the shoulder have two distinct disadvantages: the need to immobilize the arm for several weeks and loss of external rotation. These disadvantages provided an incentive for conducting a clinical evaluation of an operation originally described by Boytchev and modified by the author. In this procedure, the conjoined tendons of the coracobrachialis and the short head of the biceps with the detached tip of the coracoid process are rerouted posterior to the subscapularis muscle and reat

Abnormalities are described in the shoulders of 11 patients up to 17 years of age who have chronic brachial plexus paralyses from birth injuries. These abnormalities include a poorly formed and hypoplastic humeral head, a short abnormally formed clavicle, and a hypoplastic elevated scapula with a shallow glenoid fossa, inferiorly directed coracoid process, and abnormally tapered acromion. Four also had subluxated shoulders.

BACKGROUND: Proterochersis robusta from the Late Triassic (Middle Norian) of Germany is the oldest known fossil turtle (i.e. amniote with a fully formed turtle shell), but little is known about its anatomy. A newly prepared, historic specimen provides novel insights into the morphology of the girdles and vertebral column of this taxon and the opportunity to reassess its phylogenetic position.
RESULTS: The anatomy of the pectoral girdle of P. robusta is similar to that of other primitive turtles, including the Late Triassic (Carnian) Proganochelys quenstedti, in having a vertically oriented sc

The Latarjet procedure, including coracoid transfer, is indicated for anterior glenohumeral instability and significant bone loss. However, even in experienced hands, the Latarjet procedure is associated with potential complications including neurovascular injury, graft resorption leading to painful or broken hardware and secondary subscapularis damage, prominent hardware, and graft nonunion. An adjustable suture button technique may minimize hardware complications and show low rates of nonunion and resorption. (Perhaps, overly rigid fixation of the coracoid using screws contributes to the res

A case of inferior dislocation of the outer end of the clavicle is reported. The clavicle was found beneath the coracoid process and posterior to the intact tendons of the short head of biceps and coracobrachialis. Reduction was achieved by operative lateral retraction of the scapula. Open reduction appears to be the treatment of choice, especially if late, since traction may result in neurologic injury.

BACKGROUND: Some studies have advocated the use of suture-button fixation during the Latarjet procedure to reduce complications associated with screw fixation. However, the sample size of these studies is relatively small, and their follow-up period is short.
PURPOSE: To investigate the efficacy of the suture-button Latarjet procedure with at least 3 years of follow-up and remodeling of the coracoid graft.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: A total of 152 patients who underwent the suture-button Latarjet procedure between February 2013 and February 2016 were selected,

Fractures of the proximal humerus are common injuries with a bimodal age distribution. They usually present in younger patients after high-energy trauma and in elderly patients after lower-energy trauma. Fractures of the proximal humerus are rarely associated with concomitant fractures of the glenoid, and this is a complex injury pattern that indicates the presence of significant instability. Such injuries are usually treated surgically. Even more rarely, patients may present with proximal humerus fractures and fractures of the coracoid process. A male patient presented to our emergency depart
