Evaluation of outcome of proximal femur locking compression plate (PFLCP) in unstable proximal femur fractures.

INTRODUCTION: Pertrochanteric fractures are most frequent factures of the proximal femur, accounts for nearly 50% of all proximal femur fractures and are most devastating and also a major cause of disability in elderly. The aim and objective of this study was to evaluate the role of proximal femoral locking compression plate in unstable proximal femur fractures. MATERIAL AND METHOD: A total of 20 cases were included in our study (M:F - 16:4), who suffered proximal femur fracture due to various modalities and all of them were operated using proximal femoral locking compression plate. This prox

[A roentogenographical study on the alignment of the lower extremities in secondary coxarthrosis].

Alignment of the lower extremities in secondary coxarthrosis (273 lower extremities) and controls (240 normal lower extremities) were studied using roentogenograms of whole lower extremities including the pelvis taken in the standing position with both legs. Secondary coxarthrosis, which is caused by congenital dislocation of the hip joint, was classified into four groups: unreduced dislocation, high dislocation, incomplete dislocation, and dysplasia of the acetabulum. Comparative studies were conducted using statistical data on measured items, obtained from secondary coxarthrosis cases and co

Proximal femoral reconstruction for hip involvement in hereditary multiple exostoses.

PURPOSE: Hip involvement in hereditary multiple exostoses (HME) may lead to coxa valga, femoral impingement and progressive hip subluxation. This study aimed to evaluate the outcomes of a single-stage technique combining proximal femoral varus osteotomy with femoral neck osteochondroma trimming performed through the osteotomy site. METHODS: A retrospective series of fifteen patients (23 hips) with HME was reviewed. Clinical outcomes were assessed using the Postel-Merle d'Aubigné score. Radiographic evaluation included femoral and acetabular parameters assessed pre-operatively, immediately pos

Rate of Osteonecrosis After Femoral Lengthening with Intramedullary Lengthening Nails.

BACKGROUND: There has been historical concern that the use of intramedullary nails could present undue risk of osteonecrosis of the femoral head due to compromise of the femoral blood supply. Intramedullary lengthening nails are rapidly becoming commonplace in lengthening procedures. As such, the primary objective of this study was to analyze the rates of osteonecrosis following femoral lengthening. The secondary objective was to characterize general trends in femoral lengthening. METHODS: This retrospective cohort study evaluated patients who had undergone femoral lengthening at a single ins

A family of Melnick-Needles syndrome: a case report.

BACKGROUND: Melnick-Needles syndrome (MNS) is an extremely rare osteochondrodysplasia caused by a mutation of FLNA, the gene encoding filamin A. MNS is inherited in an X-linked dominant manner. In this study, we describe three members of the same family with MNS, who exhibited different phenotypic severity despite having an identical FLNA gene mutation. CASE PRESENTATION: The patient was 16 months old, with a history of delayed physical development, multiple upper respiratory infections and otitis media episodes. She was referred to our orthopedic clinic because of bowed legs and an abnormal

Mucopolysaccharidosis IVA and glycosaminoglycans.

Mucopolysaccharidosis IVA (MPS IVA; Morquio A: OMIM 253000) is a lysosomal storage disease with an autosomal recessive trait caused by the deficiency of N-acetylgalactosamine-6-sulfate sulfatase. Deficiency of this enzyme leads to accumulation of specific glycosaminoglycans (GAGs): chondroitin-6-sulfate (C6S) and keratan sulfate (KS). C6S and KS are mainly produced in the cartilage. Therefore, the undegraded substrates are stored primarily in cartilage and in its extracellular matrix (ECM), leading to a direct impact on cartilage and bone development, and successive systemic skeletal dysplasia

Unexpected angular or rotational deformity after corrective osteotomy.

BACKGROUND: Codman's paradox reveals a misunderstanding of geometry in orthopedic practice. Physicians often encounter situations that cannot be understood intuitively during orthopedic interventions such as corrective osteotomy. Occasionally, unexpected angular or rotational deformity occurs during surgery. This study aimed to draw the attention of orthopedic surgeons toward the concepts of orientation and rotation and demonstrate the potential for unexpected deformity after orthopedic interventions. This study focused on three situations: shoulder arthrodesis, femoral varization derotational

The Evolution of Proximal Femur Hemiepiphysiodesis in Children with Cerebral Palsy.

Hip displacement is a common and progressive musculoskeletal complication in children with cerebral palsy (CP), often driven by coxa valga, muscle imbalance, and spasticity. While proximal femoral and pelvic osteotomies remain the standard treatment for advanced deformity, proximal femoral guided growth (temporary medial hemiepiphysiodesis) has emerged as a minimally invasive alternative for selected patients with remaining growth potential. This narrative review summarizes the current evidence regarding indications, surgical technique, radiographic outcomes, complications, and future directio

A rare late progression form of Sly syndrome mucopolysaccharidosis.

Mucopolysaccharidoses VII, or Sly syndrome, is linked to mutations in the beta-glucuronidase encoding gene. Sly syndrome is a rare condition and presentation is highly variable, ranging from a prenatal form with severe, lethal fetal hydrops to more benign adolescent or adult forms with simple thoracic kyphosis. Molecular diagnosis of this adult male patient identified two missense mutations in the GUSB gene that led to a deficiency in beta-glucuronidase catalytic activity and the resulting accumulation of chondroitin sulfate glycosaminoglycans. During childhood, bilateral inguinal hernia was r

A Case of Cleidocranial Dysplasia with a Novel Mutation and Growth Velocity Gain with Growth Hormone Treatment.

Cleidocranial dysplasia (CCD) is a rare congenital autosomal dominant skeletal disorder that is characterized by hypoplasia or aplasia of clavicles, failure of cranial suture closure, dental anomalies, short stature and other changes in skeletal patterning and growth. The gene responsible for pathogenesis has been mapped to the short arm of chromosome 6p21, core binding factor alpha-1 (CBFA1) or runt related transcription factor-2 (RUNX2). Here we describe a CCD patient with a novel mutation in the RUNX2 gene. A five-and-a-half year old girl presented with severe short stature, dysmorphic faci
