Postoperative pain management after craniotomy: evaluation and cost analysis.

OBJECTIVE: Patients undergoing craniotomies have traditionally received opiates for the management of their postoperative pain. The use of narcotic pain medications can be costly, can decrease early walking, can lengthen hospital stay, and can alter a patient's neurological examination results. The use of alternative pain medications such as cyclooxygenase-2 (COX-2) inhibitors may benefit patients by resolving many of these issues. Compared with traditional nonsteroidal anti-inflammatory drugs, these anti-inflammatory medications may be used safely in neurosurgical patients because of their se

Early prediction of infection after craniotomy for brain tumours.

Reliably predicting infection soon after craniotomies would prevent infection and reduce treatment costs and hospitalization expenses. Therefore we analysed potential risk factors and blood count data after craniotomies for brain tumours in order to predict infection as soon as possible after surgery. We analysed 139 patients who underwent craniotomies for brain tumours from January 1997 to December 2001, and divided them into four categories (Types A to D) according to the following: increase in their white blood counts (WBCs) from Day 0 to Day 1, maximum WBC between Day 0 and Day 2, and maxi

Pontine hemorrhage after frontal craniotomy. Report of a case.

This report describes the complication of pontine hemorrhage in a patient who had undergone supratentorial craniotomy for treatment of a frontal intracerebral hematoma. A literature review revealed no previous cases of pontine hemorrhage in this clinical scenario. Abnormal findings on neurological examination in the early postoperative period should alert the physician to the possibility of this complication.

Applying circular posterior-hinged craniotomy to malignant cerebral edemas.

Malignant brain edemas are often fatal, regardless of whether they are treated conservatively with sedation, blood pressure management, mannitol-therapy, hyperventilation and hypothermia, or non-conservatively with routine trepanation. Unfortunately, temporal trepanation may result in significant brain damage through herniation of the cerebrum at the edges of the trepanation openings. In one case of a 26-year-old male with severe head injury, a circular posterior-hinged craniotomy (CPHC) was performed after an ineffective unitemporal trepanation for evacuation of an acute subdural hematoma. Repair of craniotomy defects using bone marrow stromal cells.

BACKGROUND: Techniques used to repair craniofacial skeletal defects parallel the accepted surgical therapies for bone loss elsewhere in the skeleton and include the use of autogenous bone and alloplastic materials. Transplantation of a bone marrow stromal cell population that contains osteogenic progenitor cells may be an additional modality for the generation of new bone. METHODS: Full thickness osseous defects (5 mm) were prepared in the cranium of immunocompromised mice and were treated with gelatin sponges containing murine alloplastic bone marrow stromal cells derived from transgenic mic

Cerebellar hemorrhage complicating supratentorial craniotomy: report of two cases.

One of the rare complications of supratentorial craniotomy is the occurrence of a hematoma in the cerebellum. Only seven previous cases have been published, and these have been ascribed to disturbed blood coagulation or decreased intracranial pressure. We present two similar patients, in whom, however, the pathogenetic role of these factors seems improbable.

Cerebellar hemorrhage after supratentorial craniotomy--report of three cases.

We report on three cases of remote cerebellar hemorrhage after supratentorial craniotomy, which had much in common in their computed tomographic, operative, and clinical findings. We speculate that, when the patient is in the supine position, displacement of the cerebellum causes stretching of the superior vermian veins and their tributaries, resulting in tearing of these vessels. Postoperative cerebrospinal fluid overdrainage or massive air reflux into the cranial cavity through the drainage tube may accelerate this process. Meticulous management of the drainage system is necessary to prevent

A retrospective comparison of prophylactic fosphenytoin and levetiracetam in awake craniotomy.

BACKGROUND: Awake craniotomy with direct cortical stimulation (DCS) mapping is the gold standard for resection of pathology in eloquent regions. Prophylactic anti-seizure medications (ASMs) are commonly used to prevent intraoperative seizure. There is a preference for levetiracetam (LEV), due to its low side effect profile. We investigated whether a prophylactic dose of fosphenytoin (FOS) compared to LEV reduced the risk of intraoperative complications. METHODS: We performed a retrospective analysis of consecutive adult and pediatric awake craniotomy cases from 2018 to 2024 who received eithe

Dural Tenting in Elective Craniotomies: A Randomized Clinical Trial.

BACKGROUND AND OBJECTIVES: Dural tenting sutures are a well-known neurosurgical technique. However, claims of them preventing extradural hematomas (EDHs) lack evidence-based support. For that reason, we decided to evaluate the noninferiority of routinely not tenting the dura in elective supratentorial craniotomies. METHODS: A randomized, multicenter, investigator-blinded and participant-blinded, controlled interventional trial with 1:1 allocation. We included adults undergoing elective, supratentorial craniotomies. Not tenting the dura was an intervention, and the control group consisted of p

[Keyhole Craniotomy and Closure for Trigeminal Neuralgia and Facial Spasms].

Facial spasms and trigeminal neuralgia are functional diseases, which have alternative treatment options. The working space for each pathology can be obtained by a routine approach to the caudal and rostral sides of the cerebellum and can be provided through a small craniotomy, because CSF drainage provides sufficient space for manipulation. However, it is necessary to expose fully the structures that define the operative field, such as the margins of the venous sinuses. Familiarity with the muscular anatomy required for exposure is also important.

[Innovations in Combined Surgery of Endoscopic Transsphenoidal Surgery and Craniotomy].

Combined endoscopic transsphenoidal surgery and craniotomy may be useful for tumors extending into the suprasellar region or ventricles and for tumors extending simultaneously into the nasal sinuses and intracranial space. This method allows two surgeons to share the surgical field while compensating for each other's blind spots and allows for safe tumor removal by separating the normal structure from the tumor and protecting the normal structure. Simultaneous combined endoscopic transsphenoidal surgery and craniotomy require a lot of equipment; however, by devising the layout of the equipment

Cottonoid Retention After Craniotomy: Causes and Ways to Avoid It.

Nonabsorbable surgical material left in an surgical wound may cause early postoperative infections and specific types of granulomas; thus, it represents a dangerous complication in neurosurgery. The authors have analyzed their experience and present four cases of cottonoid retention after intracranial tumor resection. During 5-year period (from 2013 until 2017), the incidence of such an undesirable event after craniotomy for various indications was 0.07%. It was not related to the professional experience of the operating neurosurgeon, but cases of deep-seated lesions, the presence of brain ede

Retromastoid Craniotomy: An Adaptable and Panoramic Approach to the Cerebellopontine Angle.

There are many approaches to the cerebellopontine angle, all of which involve different degrees of disruption of the normal anatomy. The retromastoid craniotomy has stood the test of time for the efficiency, panoramic exposure, and flexibility it provides while allowing excellent control over the vital cerebrovascular structures including the brainstem. This approach allows the surgeon not to get distracted by how to get there but instead to focus on what to do when he or she is there. Herein, I discuss the nuances of this simple approach with its "extended" variations.

Watertight Dural Closure in Pediatric Craniotomies-Is It Really Necessary?

BACKGROUND: Dural closure after intracranial procedures is considered crucial to reduce postoperative complications such as pseudomeningocele (PM), cerebrospinal fluid (CSF) leaks, hydrocephalus, and infections. However, watertight dural closure (WTDC) is often difficult to achieve, and dural substitutes often are used. We describe our experience with non-WTDC in children. METHODS: Data were collected retrospectively. Redo and craniectomy cases were excluded. Collected data included demographics, surgical etiology, various radiologic parameters, ventricular opening, usage of drains and shunts

Preservation of the nerves to the frontalis muscle during pterional craniotomy.

OBJECT: There continues to be confusion over how best to preserve the branches of the facial nerve to the frontalis muscle when elevating a frontotemporal (pterional) scalp flap. The object of this study was to examine the full course of the branches of the facial nerve that must be preserved to maintain innervation of the frontalis muscle during elevation of a frontotemporal scalp flap. METHODS: Dissection was performed to follow the temporal branches of facial nerves along their course in 5 adult, cadaveric heads (n = 10 extracranial facial nerves). RESULTS: Preserving the nerves to the fr

A method to map the visual cortex during an awake craniotomy.

Much has been reported regarding the technique of performing an awake craniotomy with cortical mapping for the functional cortex responsible for sensorimotor activity and language. However, documentation for mapping the visual cortex during an awake craniotomy with a description of its technical details is rare. The authors report the case of a patient who underwent an awake craniotomy with mapping of the visual cortex to remove a glioma situated in the left medial occipital lobe. The techniques that made such a mapping procedure possible are discussed.

Anesthetic considerations in a patient with severe aortic stenosis for craniotomy.

Severe aortic stenosis can pose a serious hazard for noncardiac surgery as patients with this condition have a high risk for developing perioperative cardiac complications. We describe the anesthetic management of a patient with a malignant brain tumor who was found to have severe aortic stenosis during preanesthetic evaluation. She underwent palliative balloon aortic valvuloplasty prior to surgery, and this combined with appropriate monitoring and drug therapy resulted in a satisfactory outcome. Adverse intraoperative events like hypotension and left ventricular failure occurred; however, the

[Decompressive craniotomy and its effect on recovery and rehabilitation of the patient].

The rehabilitation of patients who have to be operated on because of severe craniocerebral trauma and in whom the roof of the cranium must be removed because of severe post-traumatic cerebral oedema, is extremely difficult. The slow rehabilitation and recovery of these patients, who also show psychological changes in most cases, must be attributed to the pathological intracranial conditions which have arisen in consequence of large defects in cranial bones. A causal correlation between severe pathological intracranial conditions, which have arisen not only from trauma but also from bone flap d
