Hydroxyethylstarch compared with modified gelatin as volume preload before spinal anaesthesia for Caesarean section.

We studied 90 patients undergoing elective Caesarean section under spinal anaesthesia who received lactated Ringer's solution 1000 ml with up to 1000 ml of modified gelatin, lactated Ringer's solution 1000 ml with up to 1000 ml of 6% hydroxyethylstarch or only up to 1000 ml of 6% hydroxyethylstarch. Lumbar puncture was performed as soon as 500 ml of the colloid were infused. The incidence of hypotension, number of patients requiring a vasopressor and doses of ephedrine required to restore arterial pressure were significantly lower in favour of those receiving the crystalloid-hydroxyethylstarch

Fluid resuscitation in diabetic emergencies--a reappraisal.

The first objective in diabetic ketoacidosis is to restore the circulating volume and improve tissue perfusion. In any form of hypovolaemic shock the most efficient way of restoring circulating volume is to be use colloid solutions rather than crystalloids. At least three times the amount of crystalloid must be used to achieve the same effect. The historical reason for using isotonic saline in diabetic ketoacidosis is related not to its similarity to the fluid lost, but to its supposed efficiency in correcting the circulating volume. Excess crystalloid expands the interstitial space which resu

The impact of different infusion solutions on postoperative recovery following colorectal surgery.

PURPOSE: The purpose of this study was to compare two groups in postoperative recovery, whether there were any complications and whether the length of their hospital stay differed. One group received intraoperatively a combination of crystalloids and a small colloid dose, while the other group received only the crystalloids intraoperatively. METHODS: This randomized prospective study included 80 patients with colorectal cancer prepared for major elective colorectal surgery. The patients were randomly assigned to either the control group (CG) which received only crystalloid solutions intraoper

Crystalloids versus colloids: implications in fluid therapy of dogs with intestinal obstruction.

Responses of jejunal transcapillary and transmucosal fluid fluxes to IV infusion of crystalloid or colloid solutions were evaluated in 12 dogs. One isolated intestinal segment in each dog was used as the control segment, and 2 segments were distended to a intraluminal hydrostatic pressure of 10 cm of H2O. The artery supplying 1 of the 2 distended (autoperfused) segments was cannulated and perfused with blood from the femoral artery. One of the 2 distended segments was autoperfused from the femoral artery. Intraluminal pressure was increased in the autoperfused segment and in 1 other segment fo

Effects on coagulation of intravenous crystalloid or colloid in patients undergoing peripheral vascular surgery.

BACKGROUND: This study investigated whether haemodilution-enhanced coagulation can be demonstrated under regional anaesthesia, whether this occurs before surgery, and whether the fluid used influences the effect. METHODS: Patients were randomly allocated to receive either crystalloid or colloid intravenous fluid. An epidural was administered. Samples of venous blood were taken before fluid administration, after completion of the epidural and initial fluid load, during surgery before heparin, and after 24 h. Thrombelastograph analysis was performed, and full blood count, international normalis

Resuscitation Targets, Fluids, and Vasoactives in Septic Shock.

Intravenous fluids and vasoactive medications represent the foundation of hemodynamic resuscitation for septic shock. Based on current evidence, initial fluid resuscitation of roughly 30 mL/kg with balanced crystalloids is reasonable for most patients. Carefully monitored peripheral vein administration of the first-line vasopressor, norepinephrine, appears safe. Ongoing multimodal assessment and therapy titration integrating blood pressure, lactate clearance, capillary refill time, and dynamic measures of fluid responsiveness yields outcomes equivalent to protocolized goal-directed resuscitati

Burn State of the Science: Fluid Resuscitation.

Recognition of fluid creep has driven a large amount of the scientific investigation in the area of acute fluid resuscitation for burn patients. The role of colloids in ameliorating fluid creep is controversial, despite the fact that a fluid-sparing effect of colloids has been recognized for some time. All but one of the available prospective studies using colloids are more than a decade old, and a modern randomized controlled trial (RCT) comparing crystalloids to colloids is long overdue. While urinary output continues to be the main endpoint for fluid titration, there has been a moderate amo

Crystalloid and colloid therapy.

Fluid therapy is a cornerstone of emergency medicine, but equine practitioners should be aware of recent developments that have modified previous recommendations. First, new emphasis on the avoidance of hyperchloremia suggests that crystalloids with a lower chloride concentration may be more appropriate for use. Second, modifications to the understanding of the Starling equation suggest that the benefits of colloids may be more limited than previously thought. In addition, the negative effects of fluid overload on morbidity and mortality are becoming increasingly recognized. Although more spec

Balanced fluid bolus: Should we prefer balanced crystalloids over "normal" saline?

A fluid bolus is administered to correct hypoperfusion, hypotension, and metabolic acidosis for resuscitation at birth or in the neonatal intensive care unit (NICU). However, there is a lack of evidence-based support for any particular fluid choice. The most used intravenous fluid is 0.9% sodium chloride or "normal saline" (NS). There is a growing body of evidence that NS is associated with hyperchloremic metabolic acidosis and acute kidney injury. Balanced crystalloid solutions such as lactated Ringer's solution and solutions with a more physiological composition similar to plasma may offer s

Fluid resuscitation in critical care.

Fluid deficits can arise as a result of a range of conditions, including haemorrhage, dehydration, vomiting and diarrhoea. Excessive fluid loss can have serious consequences for the patient and, if untreated, can lead to irreversible shock. The most common fluids used in fluid resuscitation are colloids and crystalloids, with research suggesting greater overall benefits for the use of the latter. Although it is vital to initiate fluid resuscitation early on after the patient has suffered trauma, there is some controversy as to the timing of this procedure--that is, whether it should be instiga

[Postinfusion complications by acute blood loss].

Highs and lows of infusion therapy for the treatment of the acute blood loss were studied experimentally and in clinic. Colloid and crystalloid solutions, being hemodilutants, do not transport oxygen, causing, therefore, several complications in recipient's organism. Dilutional anemic and lead to heart insufficiency, which, by-turn, cause the so called dilutional circulatory hypoxia.

Balanced Crystalloid versus Saline in Adults with Traumatic Brain Injury: Secondary Analysis of a Clinical Trial.

Balanced crystalloids may improve outcomes compared with saline for some critically ill adults. Lower tonicity of balanced crystalloids could worsen cerebral edema in patients with intracranial pathology. The effect of balanced crystalloids versus saline on clinical outcomes in patients with traumatic brain injury (TBI) requires further study. We planned an a priori subgroup analysis of TBI patients enrolled in the pragmatic, cluster-randomized, multiple-crossover Isotonic Solutions and Major Adverse Renal Events Trial (SMART) (ClinicalTrials.gov: NCT02444988, NCT02547779). Primary outcome was

Role of permissive hypotension, hypertonic resuscitation and the global increased permeability syndrome in patients with severe hemorrhage: adjuncts to damage control resuscitation to prevent intra-abdominal hypertension.

Secondary intra-abdominal hypertension (IAH) and abdominal compartment syndrome (ACS) are closely related to fluid resuscitation. IAH causes major deterioration of the cardiac function by affecting preload, contractility and afterload. The aim of this review is to discuss the different interactions between IAH, ACS and resuscitation, and to explore a new hypothesis with regard to damage control resuscitation, permissive hypotension and global increased permeability syndrome. Review of the relevant literature via PubMed search. The recognition of the association between the development of ACS a

Gastric intramucosal pH changes after volume replacement with hydroxyethyl starch or crystalloid in patients undergoing elective abdominal aortic aneurysm repair.

PURPOSE: Gastric intramucosal pH (pHi), a surrogate marker of tissue oxygenation, falls following abdominal aorta aneurysm (AAA) repair. We tested the hypothesis that volume replacement with a hydroxyethyl starch solution would result in better preserved splanchnic oxygenation than would volume replacement with crystalloid solutions. MATERIALS AND METHODS: This was a prospective, randomized, nonblinded study set in a university-affiliated community hospital. Thirty patients undergoing elective AAA repair were studied. Patients were randomly selected to receive intraoperative and postoperative

Albumin replacement in patients with severe sepsis or septic shock.

BACKGROUND: Although previous studies have suggested the potential advantages of albumin administration in patients with severe sepsis, its efficacy has not been fully established. METHODS: In this multicenter, open-label trial, we randomly assigned 1818 patients with severe sepsis, in 100 intensive care units (ICUs), to receive either 20% albumin and crystalloid solution or crystalloid solution alone. In the albumin group, the target serum albumin concentration was 30 g per liter or more until discharge from the ICU or 28 days after randomization. The primary outcome was death from any cause
