Phacoemulsification with goniosynechialysis for nanophthalmos with secondary angle-closure or secondary angle-closure glaucoma: a 20-case series and stepwise management strategy.

BACKGROUND: Nanophthalmos is a rare congenital ocular disorder characterized by a short axial length (AL), which predisposes to secondary angle-closure and secondary angle-closure glaucoma. Surgical management is challenging due to high complication risks. This study aimed to evaluate the feasibility and efficacy of phacoemulsification with intraocular lens implantation and goniosynechialysis (Phaco + IOL+GSL) in nanophthalmos with secondary angle-closure or secondary angle-closure glaucoma, and to propose a stepwise complication management strategy. METHODS: This retrospective case series in

Suprachoroidal Triamcinolone Injection for the Management of Chronic Uveitis-Associated Ocular Hypotony.

PURPOSE: This study is aimed at describing the use of suprachoroidal triamcinolone injection for the treatment of refractory uveitis-associated ocular hypotony presumed secondary to ciliary body shutdown. METHODS: We report a case of a 58-year-old woman with a 26-year history of idiopathic, noninfectious bilateral panuveitis complicated by recurrent cystoid macular edema and a steroid-induced ocular hypertension. After treatment with an intravitreal dexamethasone implant and an Ahmed glaucoma valve, she developed persistent hypotony in the right eye with intraocular pressure (IOP) of 2-4 mmHg

Persistent Serous Choroidal and Retinal Detachment After Ab Interno Trabeculotomy for Glaucoma.

We describe a case of serous retinal detachment (SRD) with ciliochoroidal detachment (CCD) that persisted for 2 years and 7 months after minimally invasive glaucoma surgery (MIGS). A 71-year-old woman with primary open angle glaucoma and cataracts had a central corneal thickness of 489 μm/492 μm and an ocular axis length of 24.05 mm/24.30 mm. She underwent phacoemulsification and intraocular lens implantation in the right eye (OD), along with goniosynechialysis and microhook ab interno trabeculotomy. Postoperative intraocular pressure was 4-6 mm Hg in the OD. Five months later, SRD was observe

A Prospective Evaluation of the 2-Year Efficacy and Safety of INTERCIL: A Novel Uveal Spacer for Open-Angle Glaucoma.

INTRODUCTION: Intraocular pressure (IOP) reduction is the most commonly therapeutically modifiable risk factor in glaucoma. Minimally invasive glaucoma surgeries are generally considered safe but ineffective, constrained by physiological pressure. Invasive bleb-forming filtration surgeries are effective, but associated with high risk and high rates of failure. Technologies utilizing uveoscleral rely on creation of a cyclodialysis cleft. This study evaluated a novel supraciliary uveal spacer (INTERCIL®) designed to enhance uveoscleral outflow, avoiding anterior and posterior chamber entry or bl

Safety profile of minimally invasive glaucoma surgery.

PURPOSE OF REVIEW: This article reviews the safety profile of trabecular meshwork/Schlemm's canal-based, subconjunctival, and suprachoroidal minimally invasive glaucoma surgery (MIGS). RECENT FINDINGS: Prospective randomized clinical trials and metaanalyses published during the 2019-2020 review period provided important data regarding the safety profile of trabecular meshwork/Schlemm's canal-based MIGS. Transient hyphema and intraocular pressure spikes are among the most common complications associated with this category of MIGS, but more serious adverse events such as cyclodialysis cleft for

Spontaneous filtering bleb with subsequent secondary glaucoma June consultation #1.

A 32-year-old previously healthy woman was referred for evaluation and management of newly diagnosed hypotony in the right eye. Her ocular history was significant only for moderate myopia and an idiopathic Horner syndrome in the left eye diagnosed at age 13. The patient endorsed a history of fluctuating and decreasing vision in the right eye over the past 6 months. She denied any ocular pain, previous ocular surgery, or recent trauma. The patient was otherwise healthy and denied any previous autoimmune disease or systemic infection. She wore soft, daily disposable contact lenses and reported n
