Spontaneous inferior epigastric artery hemorrhage in a COVID-19 patient with membranous nephropathy on anticoagulant therapy: a Case Report.

BACKGROUND: The management of Coronavirus disease 2019 (COVID-19) is complicated by coagulopathies that increase both thrombotic and hemorrhagic risks, particularly in patients with comorbidities such as membranous nephropathy (MN) who require anticoagulation. Spontaneous inferior epigastric artery (IEA) hemorrhage is a rare but life-threatening complication in this setting. CASE PRESENTATION: A 63-years-old woman with M-type phospholipase A2 receptor (PLA2R)-positive MN was admitted for COVID-19 pneumonia and acute respiratory distress syndrome (ARDS). Despite prophylactic anticoagulation wi

Evidence-Based Approaches to Anticoagulation in Reconstructive Microsurgery-A Systematic Literature Review.

This systematic review addresses the crucial role of anticoagulation in microsurgical procedures, focusing on free flap reconstruction and replantation surgeries. The objective was to balance the prevention of thrombotic complications commonly leading to flap failure, with the risk of increased bleeding complications associated with anticoagulant use. A meticulous PubMed literature search following Evidence-Based-Practice principles yielded 79 relevant articles, including both clinical and animal studies. The full-texts were carefully reviewed and evaluated by the modified Coleman methodology

Efficacy and safety of commonly used thromboprophylaxis agents following hip and knee arthroplasty.

AIMS: We investigated the efficacy and safety profile of commonly used venous thromboembolism (VTE) prophylaxis agents following hip and knee arthroplasty. METHODS: A systematic search of PubMed, Embase, Cochrane Library, Web of Science, and OrthoSearch was performed. Prophylaxis agents investigated were aspirin (< 325 mg and ≥ 325 mg daily), enoxaparin, dalteparin, fondaparinux, unfractionated heparin, warfarin, rivaroxaban, apixaban, and dabigatran. The primary efficacy outcome of interest was the risk of VTE, whereas the primary safety outcomes of interest were the risk of major bleeding e

Prophylaxis for venous thromboembolism in traumatic brain injury: protocol for a randomised controlled trial.

INTRODUCTION: Venous thromboembolism (VTE) is a common complication of traumatic brain injury (TBI) and is associated with increased morbidity and mortality. Low molecular weight heparin (LMWH) is recommended for prophylaxis against VTE after trauma but may increase the risk of progression of intracranial bleeding. Limited evidence exists to guide clinicians regarding the optimal timing of VTE prophylaxis in patients with acute TBI. This randomised controlled trial (RCT) will directly compare the safety and effectiveness of early versus delayed initiation of LMWH in patients with moderate to s

Low-molecular-weight heparins for managing vaso-occlusive crises in people with sickle cell disease.

BACKGROUND: Sickle cell disease is one of the most common and severe genetic disorders in the world. It can be broadly divided into two distinct clinical phenotypes characterized by either haemolysis or vaso-occlusion. Pain is the most prominent symptom of vaso-occlusion, and hypercoagulability is a well-established pathogenic phenomenon in people with sickle cell disease. Low-molecular-weight heparins might control this hypercoagulable state through their anticoagulant effect. This is an update of a previously published version of this review. OBJECTIVES: To assess the effects of low-molecul

[Prevention and treatment of venous thromboses and thromboembolism: pentasaccharides as novel anticoagulants selectively blocking Xe factor, their position and potential (data of the XIX International Congress on Thromboses and Hemostasis)].

Drug prevention and treatment of venous thromboses and thromboembolism remain a serious problem in the management of surgical and therapeutic patients. Pentasaccharides, novel anticoagulants selectively blocking Xe factor have a great potential for their use in the given area. It has been demonstrated at the preliminary stages of the study of these antithrombotic agents that their pharmacokinetics permits subcutaneous drug injection once a day (for fondaparinux) and once for 7 days (for indraparinux) without the necessity of making routine coagulologic control. The drugs are marked by bioavail

Cerebral venous thrombosis in pregnancy.

Headaches in pregnancy are a common occurrence, and most differentials are benign but cerebral venous thrombosis (CVT) is a life-threatening secondary headache that is more common in pregnancy and postpartum. CVT is caused by an occlusion of the dural venous sinus and/or cerebral veins and presents with various non-specific presentations and has numerous causes and risk factors which make clinical diagnosis difficult. A woman in her late 30s was diagnosed with extensive CVT at 34 weeks gestation after reporting a persistent headache, visual disturbance and intermittent right-sided paraesthesia

Fondaparinux as alternative anticoagulant to warfarin or low-molecular-weight heparin for recurrent venous thrombosis.

OBJECTIVE: Recurrent venous thromboembolism (VTE) occurs in some patients despite treatment with the standard drugs, warfarin and low-molecular-weight heparin (LMWH). Fondaparinux is currently licensed by the Food and Drug Administration for the prophylaxis of deep-vein thrombosis in patients undergoing orthopedic or abdominal surgery and also in the treatment of VTE. Well-documented use of this agent beyond these indications and for prolonged periods is currently limited. CASE SUMMARY: Two cases of "refractory" VTE, managed effectively with long-term fondaparinux are described. In the first

Hepatic veno-occlusive disease/sinusoidal obstruction syndrome after hematopoietic stem cell transplantation for thalassemia major: incidence, management, and outcome.

Hepatic veno-occlusive disease or sinusoidal obstruction syndrome (VOD/SOS) is a potentially life-threatening complication of allogeneic hematopoietic stem cell transplantation (allo-HSCT). In the present prospective study, we aimed to investigate the incidence, management, and outcome of VOD/SOS in patients with thalassemia major (TM) who received allo-HSCT. VOD/SOS was diagnosed and classified based on the modified Seattle criteria. The prophylactic regimen for VOD/SOS was a combination treatment of dalteparin and lipo-PGE1. VOD/SOS was managed through an approach consisting of adequate supp

Fondaparinux: an update on new study results.

Fondaparinux (Arixtra) is the first selective factor Xa inhibitor approved for use in thromboprophylaxis after orthopaedic surgery. New recently completed trials have also demonstrated the potential of fondaparinux in the prevention of venous thromboembolism (VTE) in other surgical and medical settings and in the treatment of established VTE. In the randomized double-blind PEGASUS study in high-risk abdominal surgery patients, fondaparinux reduced the incidence of VTE from 6.1% with dalteparin to 4.6% (odds ratio reduction = 25.8%, P = 0.14), without increasing the bleeding risk. The role of low-molecular-weight heparin in the management of acute coronary syndromes.

A substantial number of clinical studies have consistently demonstrated that low-molecular-weight heparin (LMWH) compounds are effective and safe alternative anticoagulants to unfractionated heparins (UFHs). They have been found to improve clinical outcomes in acute coronary syndromes and to provide a more predictable therapeutic response, longer and more stable anticoagulation, and a lower incidence of UFH-induced thrombocytopenia. Of the several LMWH agents that have been studied in large clinical trials, including enoxaparin, dalteparin, and nadroparin, not all have shown better efficacy th

Thrombosis prophylaxis in patient populations with a central venous catheter: a systematic review.

BACKGROUND: Central venous catheters (CVCs) are used in a wide variety of patients. Associated complications are thrombosis and infection. It is a matter of debate whether thromboprophylaxis is beneficial. METHODS: We performed a systematic review of 3 different patient populations to render the available information in the literature more accessible to clinical practice: patients receiving parenteral nutrition (PN), patients with cancer, and patients admitted to intensive care units. RESULTS: Prophylaxis with heparin added to PN was found to give a nonsignificant reduction in the incidence

LMWH in the prevention of preeclampsia and fetal growth restriction in women without thrombophilia. A systematic review and meta-analysis.

Placental mediated pregnancy complications such as preeclampsia and fetal growth restriction (FGR) are common, serious, and associated with increased morbidity and mortality. We conducted a systematic review and meta-analysis to determine the effect of treatment with low-molecular-weight heparins (LMWHs) for secondary prevention of these complications in non thrombophilic women. We searched the electronic databases PubMed, Scopus, and Cochrane Library for randomised controlled trials addressing this question. Five studies including 403 patients met the inclusion criteria, 68 developed preeclam

Patient evaluation of outpatient venous thromboembolism prophylaxis service following lower limb injuries using a developed validated questionnaire.

This paper explores patients' perceptions of a new service and protocol for managing outpatient venous thromboembolism (VTE) prophylaxis, using either subcutaneous Dalteparin or oral off-license Dabigatran in patients with lower limb injury requiring immobilisation. Establishing a patient's perspective is part of good practice as, when this is positive, it aids patient compliance and protocol dissemination. A questionnaire consisting of fifty questions was given at random to one hundred patients over a six month period when they attended the trauma clinic. Each question was scored on a five po

The role of low-molecular-weight heparins in the management of unstable angina and non-ST elevation myocardial infarction.

Non-ST elevation myocardial infarction (NSTEMI) is a subset clinical manifestation of acute coronary syndrome (ACS), which is usually caused by disruption of vulnerable atherosclerotic plaque, followed by thrombosis resulting various degree of occlusions in coronary arteries. The exposure of tissue factors following the plaque rupture causes activation of coagulation cascades and formation of factor Xa. The thrombin formation will cause fibrin deposition, platelet activation and finally lead to the formation of a stable plaque. Given the central role of thrombin in the pathogenesis of ACs, and

Pravastatin potentiates the anticoagulant effects of low molecular weight heparin.

INTRODUCTION: Statins have been shown in randomized trials to reduce coronary events independent of baseline LDL-C level. In the case of pravastatin sodium (PS), there is conflicting evidence as to what is the actual mechanism of its non-lipid lowering beneficial effects. Because pravastatin has been found to prolong the clotting time in vitro, we conducted a study to determine if pravastatin plus low molecular weight heparin (LMWH) would result in a synergistic effect on the in vitro clotting time, thus supporting the hypothesis that pravastatin exerts antithrombotic effects through reduction

ANCA-associated vasculitis with central retinal artery occlusion developing during treatment with methimazole.

A 63-year-old woman suddenly developed central retinal artery occlusion following a slight fever while being treated with methimazole (MMI) for hyperthyroidism. She was diagnosed to have anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) based on increased inflammatory reactions with positive myeloperoxidase-ANCA in the serum. Her visual acuity remained low despite immediate treatment with corticosteroids and cyclophosphamide after cessation of MMI, which may have played a role in the pathogenesis of AAV. Central retinal artery occlusion is a rare manifestation of AAV; how
