Consensus, clinical decision making, and unsettled cases.

The model of clinical ethics consultation (CEC) defended in the ASBH Core Competencies report has gained significant traction among scholars and healthcare providers. On this model, the aim of CEC is to facilitate deliberative reflection and thereby resolve conflicts and clarify value uncertainty by invoking and pursuing a process of consensus building. It is central to the model that the facilitated consensus falls within a range of allowable options, defined by societal values: prevailing legal requirements, widely endorsed organizational policies, and professional standards of practice and

Cognitive biases and decision making in gambling.

Heuristics and cognitive biases can occur in reasoning and decision making. Some of them are very common in gamblers (illusion of control, representativeness, availability, etc.). Structural characteristics and functioning of games of chance favor the appearance of these biases. Two experiments were conducted with nonpathological gamblers. The first experiment was a game of dice with wagers. In the second experiment, the participants played two bingo games. Specific rules of the games favored the appearance of cognitive bias (illusion of control) and heuristics (representativeness and availabi

Assessment of capacity for medical decision making.

Determination of a patient's capacity for medical decision making can be simple or complex, depending on the issue being considered; likewise, the degree of capacity required to make decisions varies with the complexity of the situation. A capacity examination is a focused and impartial evaluation of mental status and medical, surgical, psychiatric, and psychosocial histories. The examination is composed of several basic steps, beginning with an assessment of the patient's ability to communicate, followed by determination of his or her understanding of the proposed care, followed by the patien

Anticipation of cognitive demand during decision-making.

An abundance of evidence indicates that action selection is guided, at least in certain contexts, by anticipation of action outcomes. In one particularly clear demonstration of this principle, Bechara and colleagues, studying a gambling task, observed phasic skin conductance responses just prior to actions associated with a relatively high risk of monetary loss (Bechara et al. in J Neurosci 19:5473-5481, 1999; Bechara et al. in Science 275:1293-1295, 1997; Bechara et al. in Cereb Cortex 6:215-225, 1996). In the present work, we tested for the same effect in a paradigm where choices resulted no

Vitamin D discovery outpaces FDA decision making.

The US FDA currently encourages the addition of vitamin D to milk and cereals, with the aim of reducing rickets in children and osteoporosis in adults. However, vitamin D not only regulates the expression of genes associated with calcium homeostasis, but also genes associated with cancers, autoimmune disease, and infection. It does this by controlling the activation of the vitamin D receptor (VDR), a type 1 nuclear receptor and DNA transcription factor. Molecular biology is rapidly coming to an understanding of the multiplicity of roles played by the VDR, but clinical medicine is having diffic

Dopaminergic modulation of risk-based decision making.

Psychopharmacological studies have implicated the mesolimbic dopamine (DA) system in the mediation of cost/benefit evaluations about delay or effort-related costs associated with larger rewards. However, the role of DA in risk-based decision making remains relatively unexplored. The present study investigated the effects of systemic manipulations of DA transmission on risky choice using a probabilistic discounting task. Over discrete trials, rats chose between two levers; a press on the 'small/certain' lever always delivered one reward pellet, whereas a press on the other, 'large/risky' lever

Decision-making dysregulation in first-episode schizophrenia.

Studies with chronic schizophrenia patients have demonstrated that patients fluctuate between rigid and unpredictable responses in decision-making situations, a phenomenon which has been called dysregulation. The aim of this study was to investigate whether schizophrenia patients already display dysregulated behavior at the beginning of their illness. Thirty-two first-episode schizophrenia or schizophreniform patients and 30 healthy controls performed the two-choice prediction task. The decision-making behavior of first-episode patients was shown to be characterized by a high degree of dysregu

Impulsive decision-making: learning to gamble wisely?

A decade ago, Bechara et al. [Bechara, A., Damasio, A. R., Damasio, H., & Anderson, S. W. (1994). Insensitivity to future consequences following damage to human prefrontal cortex. Cognition, 50, 7-15] published a paper in Cognition, introducing a Gambling Task which was designed to mimic everyday decision-making. Since then, the task has been computerized, its decision-making components have been scrutinized, and it has been employed in various patient groups, such as pathological gamblers, substance abusers, and so forth. Time and again it has been shown that while normal controls perform wel

Pharmacoeconomics--an aid to better decision-making.

OBJECTIVES: The first aim of this workshop was to define pharmacoeconomic concepts and terminology. Pharmacoeconomics can be defined as the branch of economics that uses cost-benefit, cost-effectiveness, cost-minimization, cost-of-illness and cost-utility analyses to compare pharmaceutical products and treatment strategies. Economic evaluations provide healthcare decision-makers with valuable information, allowing optimal allocation of limited resources. However, pharmacoeconomics is based on long-term benefits, whereas physicians are typically forced to seek immediate savings. The second aim

Neurobiology of decision-making: risk and reward.

Neurological patients with bilateral ventromedial (VM) lesions of the prefrontal cortex often deny, or they are not aware that they have a problem. Furthermore, they often pursue actions that bring some reward in the immediate run, despite severe long-term consequences such as the loss of job, home, and family. The somatic marker hypothesis, which provides an account of this defect in decision-making, posits that the impairment is the result of defective activation of somatic markers that normally function as covert or overt signposts for helping with the process of making choices that are adv

Blood donor screening. Factors influencing decision making.

A questionnaire assessing expert opinion on reducing the risk of transfusion-transmitted disease through selection and testing of blood donors was distributed to transfusion medicine professionals. A second questionnaire assessing the publics' perception of blood transfusion safety was distributed to selected nonexperts. Both groups agreed that recipient safety should be a priority. However, issues such as the impact of testing on the available donor pool, the psychosocial effects of donor deferral, and costs related to each risk-reduction strategy also must be considered. Decision making in surgery: acute anorectal sepsis.

Acute perineal inflammation is a common surgical emergency, presenting in most cases with an abscess arising from infection within the many tissue planes in this area. Simple drainage of such an abscess leads to immediate symptomatic relief but this should be accompanied by diagnostic manoeuvres that allow any underlying fistulae to be found and eradicated.

Adolescent sexuality: values, morality and decision making.

The first section of this paper presents a theoretical basis of morality and value formation which provides the background for an examination of adolescent sexual and cognitive development. Data from a study of influences on the sexual decisions of five hundred 13- to 19-year-olds are analyzed in the remainder of the article. Relationships between individual characteristics and the importance of various influences are interpreted in light of the structure of values and moral development.

Ethical decision making by Canadian family physicians.

Canadian family physicians were sent questionnaires that asked how they would handle the ethical problems posed by six sample cases and what reasons were relevant to their decisions. The ethical problems concerned how much information to divulge to patients, how extensively a physician should become involved in the lifestyles of patients and how to deal with a possible family problem. The study identified characteristics of family physicians that affect their ethical decision making and tested a theoretical model that regards ethical problems as conflicts between respecting patient autonomy an

Editorial: Making Use of What We Know: Medical Decision-Making and Antipsychotics.

Minimizing the duration of untreated psychosis and providing comprehensive early intervention including the use of antipsychotics reflects best practice in the treatment of first-episode psychosis (FEP).1 The adverse effects of second-generation antipsychotics (SGAs) are well known, namely, weight gain, hyperglycemia, and dyslipidemia that are associated with an increased risk of obesity, type 2 diabetes, and cardiovascular disease. Youth appear to be more vulnerable to deleterious cardiometabolic effects than adults.2 Child and adolescent psychiatrists (CAPs) treating FEP have an ethical duty

Randomized Trial of the Choosing Wisely Consumer Questions and a Shared Decision-Making Video Intervention on Decision-Making Outcomes.

BACKGROUND: Despite widespread use, there are few studies evaluating the consumer Choosing Wisely questions. METHODS: We evaluated the impact of the Choosing Wisely questions on consumers' decision-making outcomes. Adults living in Australia were presented with a hypothetical low-value care scenario. Using a 2×2×2 between-subjects factorial design, they were randomized to either the Choosing Wisely questions ("Questions"), a shared decision-making (SDM) preparation video ("Video"), both interventions, or control (no intervention). Primary outcomes were 1) self-efficacy to ask questions and be

Let's Open the Decision-Making Umbrella: A Framework for Conceptualizing and Assessing Features of Impaired Decision Making in Addiction.

Decision-making impairments play a pivotal role in the emergence and maintenance of addictive disorders. However, a sound conceptualization of decision making as an umbrella construct, encompassing its cognitive, affective, motivational, and physiological subcomponents, is still lacking. This prevents an efficient evaluation of the heterogeneity of decision-making impairments and the development of tailored treatment. This paper thus unfolds the various processes involved in decision making by adopting a critical approach of prominent dual- or triadic-process models, which postulate that decis
