Defecographic disorders in anal incontinent women: relation to symptoms and anal endosonographic patterns.

OBJECTIVE: The need for a defecography in incontinent women is still debatable. We prospectively evaluated the prevalence of defecographic abnormalities in incontinent women in order to determine whether any symptom or endosonographic findings could be associated with a particular defecographic pattern. MATERIAL AND METHODS: Fifty incontinent women (aged 30-87 years) underwent defecography and anal endosonography to look for pelvic floor descent, rectocele, intussusception, enterocele and the presence of anal sphincter defects. Other symptoms, i.e. straining at stools and pelvic pressure, wer

Functional disorders: rectoanal intussusception.

Rectoanal intussusception (RI) is a telescoping of the rectal wall during defecation. RI is an easily recognizable physiologic phenomenon on defecography. The management, however, is much more controversial. Two predominant hypotheses exist regarding the etiology of RI: RI as a primary disorder, and RI as a secondary phenomenon. The diagnosis may be suspected based on clinical symptoms of obstructive defecation. Diagnostic modalities include defecography as the gold standard. Dynamic pelvic magnetic resonance imaging (DPMRI) and transperineal ultrasound are attractive alternatives to defecogra

The accuracy of clinical examination in the diagnosis of rectal intussusception.

PURPOSE: This study was designed to compare routine clinical examination and defecography in the diagnosis of rectal intussusception in constipated patients and study relationships between rectal intussusception and symptoms. METHODS: A total of 127 consecutive patients with functional constipation were examined in the left-lateral position with rectal palpation and rectoscopy according to a protocol. An overall clinical judgment was made if the patient had intussusception, unclear finding, or no intussusception. Defecography was performed without knowledge of the results of the clinical eval

[The defecographic and clinical aspects of the solitary rectal ulcer syndrome].

INTRODUCTION: Solitary rectal ulcer syndrome is a complex evacuation disorder characterized by a benign ulcerative lesion of the distal rectum; the main symptom is rectal bleeding, but mucus discharge and difficult evacuation may be associated. The clinical, endoscopic and radiologic findings of solitary rectal ulcer syndrome are evaluated in this study. The role of defecography in the diagnosis of mucosal ulceration and morphofunctional alterations such as rectal prolapse and intussusception are investigated. MATERIAL AND METHODS: In the last 5 years, 27 patients (19 women and 8 men; mean ag

Balloon expulsion test facilitates diagnosis of pelvic floor outlet obstruction due to nonrelaxing puborectalis muscle.

We compared balloon expulsion, defecography, colonic transit times, anal manometry, and electromyography in 21 patients with severe constipation. Defecography demonstrated nonrelaxation of the sphincter during straining in all patients. Only 12 patients were unable to expel a balloon. Colonic transit was normal (five) or showed rectosigmoid delay (seven). All 12 patients were offered biofeedback. The nine patients able to expel a balloon had normal colonic transit (six) or colonic inertia (two). Rectosigmoid delay was due to severe intussusception in one patient. Anal manometry and pudendal ne

Thickening of the internal anal sphincter: correlation with external rectal intussusception and associated features at MR proctography and anal manometry.

OBJECTIVES: To determine if internal anal sphincter (IAS) thickness can be reproducibly measured with endoanal MRI in conjunction with MR defecography, and if IAS thickening is associated with external rectal intussusception (i.e., external rectal prolapse) at MR defecography or high-resolution anal manometry (HRM) measurements. METHODS: In this case-control study, 119 patients underwent clinical endoanal MRI, dynamic MR defecography, and HRM. Patients with intra-anal or external rectal intussusception (cases) were matched to those who had neither (controls). Five radiologists, blinded to all

Rectocele is associated with paradoxical anal sphincter reaction.

Rectocele is a frequent finding in constipated patients. However, constipation is not always relieved by rectocele repair, which may be due to other overlooked reasons for constipation. The study was designed to investigate patients with rectocele, in order to elucidate concomitant colorectal disorders and their association with rectocele. One hundred and twelve female patients suffering from severe constipation and rectal emptying difficulties were investigated using defecography, electrophysiology, anorectal manometry and colon transit time. Fifty-six patients with rectocele demonstrated by

Functional radiology of the ano-rectal region.

Chronic constipation is probably the most common symptom resulting in a referral of patients for a dynamic radiologic investigation of the GI tract. The primary usefulness of defecography in chronic constipation is to provide details about the dynamic phenomenon of evacuation which cannot be elicited by any other medical technique. It is employed to demonstrate or rule out the presence of an anatomical deformity (prolapse, rectocele, intussusception) and/or a localized dysfunction (outlet obstruction, rectal inertia) of the distal GI tract. Defecography can distinguish between a grossly obstru

Diagnosis of functional disorders of defecation causing the solitary rectal ulcer syndrome.

The solitary rectal ulcer and colitis cystica profunda are different manifestations of the solitary rectal ulcer syndrome. The cause of solitary rectal ulcer syndrome remains unknown. Since defecation disorders are common among patients with solitary rectal ulcer syndrome, defecography is indicated. Defecography was performed on 19 patients with solitary rectal ulcer syndrome. In five patients, the spastic pelvic floor syndrome had occurred. Twelve patients had internal intussusception of the rectum, and one patient had an anterior rectal wall prolapse. In one patient, no abnormalities could b

Defecographic Findings in Patients with Severe Idiopathic Chronic Constipation.

BACKGROUND/AIMS: Chronic constipation is a common gastrointestinal disorder diagnosed using Rome III criteria. Defecography is a radiographic method used to identify anatomic abnormalities of anorectum. The present study aimed to evaluate the defecographic findings in patients with severe idiopathic chronic constipation. METHODS: One hundred patients, who complained of severe idiopathic chronic constipation with abnormal balloon expulsion test, underwent defecography after injection of barium. An analysis of radiographs was performed by an expert radiologist for the diagnosis of descending pe

Evacuation proctography in normal volunteers.

Constipated patients evaluated by evacuation proctography may be subjected to vigorous medical therapy or surgery, even colectomy, based on radiographic findings that have been called "abnormal" in the literature. Criteria for normal defecography are not uniformly established, nor has correlation of structural or functional findings with symptoms been clearly documented. We prospectively studied 21 asymptomatic volunteers to assess the frequency of findings in a control population, and to establish a quantitative measure of normal rectal emptying. Standard defecography technique demonstrated r

[Diseases detected with defecographic examination in 300 patients].

Defecography is a radiological examination providing morphological details of the anorectal tract. Over almost 3 years, 300 patients were examined. The authors report on the method and the pathologic conditions they observed. The patients were seated in latero-lateral position on a radiolucent water-filled commode: lead marks were taped to define perianal skin. 150 ml of high-density barium paste were introduced into the rectum and radiographs were then acquired, at rest and during squeezing and straining. All examination phases, especially the study of dynamic evacuation, were recorded on a v

Diagnosis of enteroceles by dynamic anorectal endosonography.

PURPOSE: Enteroceles are herniations of the lining of the peritoneum and intestinal loops into the pouch of Douglas. They may accompany other pelvic and anorectal disorders or cause outlet obstruction. So far they are only diagnosed by defecography. We investigated the use of dynamic anorectal endosonography to detect this disorder. METHODS: Seventeen female patients with a defecation disorder were investigated by proctoscopy and endoluminal ultrasonography. In 14 patients defecography followed. Endosonography was performed using the curved array 7.5 MHz scanner directed to the ventral rectal

[Application of Rectal Prolapse Constipation Balloon in Single Auxiliary Defecation].

OBJECTIVE: To explore the application value of rectal prolapse constipation balloon in single auxiliary defecation. METHODS: Forty-one patients with moderate or severe rectocele were treated with a rectocele constipation balloon through the vagina. The defecography and VAS scores were compared before and after implantation. RESULTS: There was a significant difference between the anorectal angle, rectocele, and VAS scores before and after intervention in defecography (P<0.01). CONCLUSIONS: A single assisted defecation of the rectocelicular constipation balloon is feasible.

Extraperineal enterocele in male: A case report and literature review.

INTRODUCTION: Enterocele is a herniation of the small bowel through the cul-de-sac. It is uncommon and most often seen in elder females. Large enterocele manifesting as rectal prolapse is exceedingly rare and only few cases are reported previously. Due to it rarity, the best surgical treatment is not yet established especially in male patients. We present a case of enterocele causing rectal prolapse in a male patient that was treated surgically. PRESENTATION OF CASE: A 47-year-old African American male with chronic constipation and straining presented with manually reducible rectal prolapse.

MR imaging of the pelvic floor.

Pelvic floor disorders are a common clinical problem. Clinical evaluation frequently underestimates the extent of the disease. Fluoroscopic colpocystodefecography (CCD) is an established tool for diagnosing disorders of the rectum and recto- anal junction. It is however less performant when it comes to evaluate the complex pelvic floor mechanism in all its aspects. MR defecography allows to evaluate all the compartments of the pelvis without being invasive. This article describes the clinical setting, the MR technique and provides guidelines for interpretation of a MR defecography study.
