Sprint Fidelis implantable cardioverter-defibrillators lead patient management and survival: Single center study.

BACKGROUND: Over the last several years significant rises in the use of implanted cardioverter-defibrillators (ICD) have also resulted in a number of associated complications. This number includes lead failure. Sprint Fidelis (SF) ICD lead is regarded as a lead with elevated failure risk. Every center acting in accordance with the guidelines should observe patients more thoroughly especially with recalled leads and run a registry of their follow-up. The aim of this research was to present follow-up of the patients with SF leads (types 6948, 6949) from a single implantation center. METHODS: Th

The cost effectiveness of implantable cardioverter defibrillators: a systematic review of economic evaluations.

BACKGROUND: Sudden cardiac death (SCD) is the most common cause of death in developed countries, with more than 3 million people dying yearly. Implantable cardioverter defibrillators (ICDs) are considered to be an effective treatment in the primary and secondary prevention of SCD; however, their cost is considerable and this poses questions regarding whether they are worth the investment relative to less expensive pharmacotherapy. OBJECTIVE: The aim of this systematic review is to investigate existing evidence regarding the cost effectiveness of ICD therapy and to identify the key drivers of

Factors related to quality-of-life pattern changes in recipients of implantable defibrillators.

PURPOSE: The implantable cardioverter defibrillator (ICD) has proven life-saving, yet it is important to understand its psychological effects on recipients. This study examined longitudinal changes in patterns of quality-of-life (QOL) scores in the first 3 months after an implant, and determined what variables tested as predictors of patterns. METHODS: This longitudinal, prospective, descriptive, correlational survey study followed 80 ICD patients, with data collection at a baseline preinsertion, and 1 and 3 months after implant. RESULTS: Findings revealed eight patterns of QOL change that w

Psychosocial considerations for children and young adolescents with implantable cardioverter defibrillators: an update.

BACKGROUND: The extension of the life-saving benefits of the implantable cardioverter-defibrillator (ICD) to prevent sudden cardiac arrest in children and young adults represents a key clinical development. The psychosocial adjustment in young ICD patients has not been as well-studied as in adults and remains an area of needed research. METHODS: The current paper reviews the quantitative research regarding ICD patient psychosocial outcomes in the adult and pediatric literature, and highlights existing intervention research on adult ICD patients and its implications for young ICD patients. Effects of antiarrhythmic drugs on inappropriate shocks in patients with implantable cardioverter defibrillators.

BACKGROUND: Patients with atrial fibrillation (AF) or congestive heart failure (CHF) are more vulnerable to inappropriate shocks from implantable cardioverter-defibrillators (ICDs), but the effect of antiarrhythmic drugs in these patients remains unknown. METHODS AND RESULTS: A total of 55 patients with AF and/or CHF (New York Heart Association functional class > or =III) who had ICDs were divided into 3 groups [amiodarone (n=24), sotalol (n=12), beta-blocker (n=19)] and the cumulative rates of inappropriate shocks were compared. The baseline characteristics of the 3 groups were not significa

An updated review of implantable cardioverter/defibrillators, induced anxiety, and quality of life.

During the past 2 years the number of studies examining psychopathology and quality of life after ICD implantation has increased dramatically. Variables assessed have included recipient age, gender, and social support network. How recipients respond to having the device, particularly after experiencing firing, has been evaluated in light of new depression and anxiety disorder diagnoses as well as premorbid personality structure. Now the picture of what is known is, if anything, cloudier than it was 2 years ago, with little definitive and much contradictory data emerging in most of these catego

Implantable cardioverter defibrillators in adults with congenital heart disease: a single center experience.

Sudden death is the most frequent cause of late mortality in adults with congenital heart disease. This study reviews the experience of an Adult Congenital Heart Center with the use of implantable cardioverter defibrillators (ICDs). The charts of adults with congenital heart disease who had an ICD implantation were reviewed for diagnosis, residual lesions, reoperations, reason for implantation, complications, and recurrence of arrhythmias. Since 1995, 13 patients (mean age 43 years) had an ICD implantation for aborted sudden death (4), and spontaneous (6), or induced (3) ventricular tachycardi

Advanced rhythm discrimination for implantable cardioverter defibrillators using electrogram vector timing and correlation.

INTRODUCTION: Discrimination of ventricular and supraventricular arrhythmias remains one of the major challenges for appropriate implantable defibrillator (ICD) therapy delivery. The electrogram vector timing and correlation (VTC) algorithm was developed for such rhythm discrimination. The VTC algorithm differentiates normally conducted supraventricular beats from abnormally conducted ventricular beats by comparing the timing and correlation of rate and shock channel electrograms. METHODS AND RESULTS: Rate and shock channel electrograms of sinus rhythm and induced arrhythmias were collected f

Inappropriate therapy from atrial fibrillation and sinus tachycardia in automated implantable cardioverter defibrillators.

BACKGROUND: Inappropriate therapy from supraventricular tachyarrhythmias (atrial fibrillation [AF] and sinus tachycardia [ST]) in patients with implanted cardioverter defibrillators is a major challenge. We tested the performance of stability algorithms from 3 manufacturers for episodes of inappropriate therapy delivered because of AF and an onset algorithm for all episodes of inappropriate therapy caused by ST. METHODS: Therapy was classified as caused by ventricular tachycardia (VT), ST, or AF from review of stored intracardiac electrograms, history, clinical information, and R-R data befor

The potential usage of dual chamber pacing in patients with implantable cardioverter defibrillators.

UNLABELLED: Bradycardia support by ICDs has been limited to fixed rate, ventricular pacing. Concomitant placement of a pacemaker and an ICD exposes a patient to potentially life-threatening device interactions. ICDs capable of dual chamber pacing have recently become available. The number of ICD recipients who stand to benefit from the addition of dual chamber pacing is debated, but no data have addressed this question. This retrospective study analyzed all patients who received nonthoractomy ICD system placement at the Mayo Clinic in Rochester, MN between March 1991 and October 1996 in order

Pectoral implantation of single-lead implantable cardioverter-defibrillators in patients without transvenous access.

With the introduction of the single-lead "Active-Can" implantable cardioverter-defibrillators, the implantation of the internal defibrillators has become a technically easy procedure. With these systems lowest defibrillation thresholds are achieved with a very low complication rate. For patients with thrombosis of both subclavian veins, however, a transvenous implantation technique is not possible. These patients are still equipped with epicardial patch electrodes. This article describes an alternative technique for implantation of this system in such patients, eliminating the need for epicard

[Travelling with a pacemaker or implanted defibrillator].

There are no guidelines for patients travelling with implanted pacemakers or defibrillators. Only few publications deal with specific problems that this patient group might face. In this article different aspects of travelling with implanted electric devices are summarized. Patients with pacemakers and implanted defibrillators have nearly no limits when travelling. An exception to that rule is scuba diving, which mostly is limited because of the device. In general it is the underlying heart disease or arrhythmia that limits patients' travel activities. It is reasonable to travel after implanta

The Subcutaneous Implantable Cardioverter Defibrillator.

The subcutaneous implantable cardioverter defibrillator (S-ICD) is a subcutaneous alternative to conventional transvenous ICD (TV-ICD) systems, which have previously been shown to treat life-threatening ventricular tachyarrhythmias in cardiac disease patients. A review of the literature reveals that S-ICDs have similar shock efficacy rates for both induced and spontaneous ventricular tachyarrhythmias when compared with TV-ICDs. Furthermore, S-ICDs seem to have a higher specificity for withholding therapy when supraventricular tachycardia is present compared with TV-ICDs. The advantages of the

Implantable cardioverter defibrillator therapy.

Sudden cardiac death (SCD) is a major healthcare problem worldwide. The majority of SCD events occur in patients with clinically recognized heart disease and most episodes result from ventricular tachyarrhythmias. Implantable cardioverter defibrillator (ICD) therapy prevents SCD in specific patient populations. Significant progress in the design and technology has been made since the Food and Drug Administration first approved the ICD in 1985. First-generation ICDs were large, were implanted in the abdomen, required a thoracotomy for placing epicardial defibrillation patches, and were nonprogr
