A peculiar case of Morel-Lavelle lesion of upper limb.

Morel-Lavallee lesions (MLL) are post-traumatic, closed internal degloving injuries that can result in severe complications if not diagnosed early. It is conventionally seen in cases of the high energy injuries of the pelvis and lower limb. The accumulation of extravasated blood, secondary to fracture and soft tissue damage may cause internal degloving injury, skin necrosis, soft tissue damage and acute osteomyelitis. We report here the clinical and radiological features in a 32-year-old male referred from the emergency department of a tertiary hospital who had sustained high energy motor vehi

A Morel-Lavallee Lesion After Total Knee Arthroplasty and Aggressive Physical Therapy Manipulation: A Case Report and Review of the Literature.

Total knee arthroplasty (TKA) is one of the most common procedures performed in orthopedics. It has the potential for multiple complications. We present a case of a patient who developed a Morel-Lavallée lesion (closed degloving injury) of the knee in the setting of a postoperative seroma after aggressive physical therapy manipulation following TKA. Clinically, the presentation was similar to a prosthetic joint infection. The patient was treated with thorough open irrigation, debridement, and dead space management with internal quilting sutures, followed by the placement of negative pressure w

Free gracilis end-to-side microanastomosis to a peronea arteria magna: a case report.

BACKGROUND: Complex orthoplastic lower limb trauma in individuals with multiple injuries requires considerable resources and interdisciplinary collaboration for good outcomes. We present the first reported end-to-side free flap microanastomosis for lower limb trauma reconstruction involving a peronea arteria magna without radiographic collaterals. CASE PRESENTATION: A 55-year-old Caucasian gentleman involved in road traffic collision sustained an open tibial fracture on the anteromedial distal third of the left lower leg with local degloving and a subtotal right foot and ankle degloving. Multifocal arterial haemorrhage in a partially stable pelvic fracture after a crush injury: a case report.

INTRODUCTION: Most pelvic haemorrhages are thought to be caused by injury to small arteries or veins in the fractured cancellous pelvic bone or in the surrounding soft-tissues, and only 6-18% of patients with unstable pelvic fractures have haemorrhage from larger arteries. When arterial injuries are present, the majority involve branches of the internal iliac artery with only few published reports of injuries to the external iliac artery or its branches. MATERIALS AND METHODS: We report of a patient who sustained a combined pelvic and acetabular fracture with multifocal bleeding involving bra

Cervico-thoracic Morel-Lavallée lesion.

Morel-Lavallée lesions (MLLs) result from high-energy trauma causing separation of subcutaneous tissue from the underlying tissue, most commonly in the gluteal region or thigh. We report the case of a woman in her 40s with a fluctuant collection of the cervico-thoracic region following trauma. Further imaging identified an MLL. An orthoplastic approach resulted in non-operative management with a spinal brace. Three months from initial injury, the lesion completely resolved. She was symptom free at final follow-up and discharged. We present the only recorded case of MLL developing in the cervico-

Chronic Morel-Lavallée Lesion in a Pediatric Competitive Dancer: A Case Report and Literature Review.

Morel-Lavallée lesions (MLLs) are rare internal degloving injuries resulting from shearing trauma, with few reported cases in the pediatric population. We present the case of a 16-year-old female competitive dancer with persistent right knee pain, skin atrophy, and ecchymosis after trauma sustained during a national dance competition, with MRI demonstrating a loculated fluid collection consistent with an MLL. MLLs in children pose significant diagnostic challenges due to their non-specific symptoms overlapping with other musculoskeletal, autoimmune, and dermatological conditions. This case und

Resurfacing a circumferentially degloved hand by using a full-thickness skin graft harvested from an avulsed skin flap.

Twelve patients with circumferentially degloved hands were treated with full-thickness skin grafts harvested from defatted avulsed flaps. All injuries were industrial accidents caused by various roller machines, not crush injuries. Of these 12 patients, 9 patients were degloved from the wrist level and 3 patients were degloved from the forearm. There were 11 distally based skin flaps and one flap was completely detached. Four patients were avulsed distally to the mid palm, with volar neurovascular bundles damaged at the "fenestrae" of the palm, which resulted in devascularization of the involv

Penile Fracture with Urethral Rupture: The Feasibility of Repair through Penoscrotal Approach.

Penile fracture with an associated urethral injury is a rare urological emergency resulting from trauma to the erect penis during vigorous sexual intercourse. The patient often presents with swelling of the penis, discoloration of the penile skin, localized pain, and hematuria with a typical history of sudden detumescence during intercourse. Subcoronal penile degloving incision has been conventionally described and is frequently used by many clinicians for the management of penile fracture-urethral injury. Here, we describe a case of complex penile fracture managed through the vertical penoscr

Split-thickness skin excision: its use for immediate wound care in crush injuries of the foot.

Split-thickness skin excision (STSE) was used as an adjunctive modality in the treatment of eight crush injuries of the foot. Compartment syndromes were present in four feet and were treated with fasciotomy. Wound debridement, internal fixation of fractures, and STSE followed. This technique accurately determined the viability of the skin flap, simultaneously providing skin for local wound coverage. All flaps treated in this manner survived and all (100%) of the degloved STSE grafts healed. Additional procedures were performed in four patients (two free flaps and two split-thickness skin graft

Lower-limb reconstruction with chimeric flaps: The quad flap.

Early soft-tissue coverage is critical for treating traumatic open lower-extremity wounds. As free-flap reconstruction evolves, injuries once thought to be nonreconstructable are being salvaged. Free-tissue transfer is imperative when there is extensive dead space or exposure of vital structures such as bone, tendon, nerves, or blood vessels. We describe 2 cases of lower-extremity crush injuries salvaged with the quad flap. This novel flap consists of parascapular, scapular, serratus, and latissimus dorsi free flaps in combination on one pedicle. This flap provides the large amount of soft-tis

Morel-Lavallée Lesion Diagnosed by Point-of-Care Ultrasound: A Case Report and Review of Treatment Strategies.

BACKGROUND: Morel-Lavallée lesions, also known as an internal degloving injuries, occur hours to months after high-speed shearing trauma, usually in the peri-trochanteric region. These are uncommon injuries, and are often missed as part of the trauma examination. Failure to diagnose or treat these lesions may result in complications, such as infected seromas, chronic cosmetic deformities, capsule formation, or skin necrosis. There are no formalized societal guidelines for management, but smaller studies have recommended compression alone for asymptomatic lesions, aspiration for small symptomat

Inferior extension of the groin flap based on the descending branch of the superficial circumflex iliac artery.

The authors present a groin flap which was extended inferiorly and included the descending branch of the superficial circumflex iliac artery (SCIA) as a new alternative technique. Little research has been previously done on the descending branch of the SCIA, but the authors found clinically that this small branch was consistently present, and decided to make use of it to extend the groin flap inferiorly. The procedure extending the groin flap was successfully applied in three patients, one with a degloving injury of the leg, and two with progressive hemifacial atrophy. The extended portion was

More Than a Bruise: Two Cases of Morel-Lavallée Lesions in Active Duty Soldiers.

Soft tissue injuries are common in the military, generally producing simple hematomas without the need for intensive evaluation and management. In certain situations, a shearing effect between fascial planes whereby a disruption of the vasculature and lymphatic systems creates a "closed degloving injury" is more generally referred to as a Morel-Lavallée Lesion (MLL). With a consistent pool of blood, lymph, and pieces of soft tissue, an M7LL can cause significant poor cosmesis, chronic pain, and risk secondary infection if left undetected and untreated. Proper diagnosis can be made through the

Risk factors associated with tendon adhesions after hand tendon repair.

BACKGROUND: Tendon adhesions after hand tendon repair are one of the most difficult complications of hand surgery and can cause severe disability. This study aimed to assess the risk factors associated with tendon adhesions after hand tendon repair to provide a theoretical foundation for the early prevention of tendon adhesions in patients with tendon injuries. Moreover, this study intends to increase doctors' awareness of the issue and serves as a reference for developing new prevention and treatment strategies. METHODS: We retrospectively analyzed 1,031 hand trauma cases that underwent repa

Salvage in a case of ring avulsion injury with an immediate second-toe wrap-around flap.

A nonreplantable complete degloving injury to the small finger of a young woman was treated with the immediate microsurgical transfer of a second-toe wrap-around flap. One year after the operation, the donor foot was free of symptoms, and the reconstructed finger had an excellent cosmetic appearance, a range of motion of 0/90 degrees at both the metacarpophalangeal and the proximal interphalangeal joints, and a two-point discrimination of 6 mm. In selected patients, when the degloved skin envelope cannot be revascularized and when the skeletal and tendinous units are still intact, an immediate

The use of avulsed soft tissue for immediate full-thickness skin grafting in traumatic extremity injuries: A multi-institutional 10-year retrospective review of outcomes with technical considerations.

BACKGROUND: Avulsion or degloving injuries of the extremities are often caused by high-energy trauma and can be associated with high morbidity. Multiple techniques have been proposed for management, including immediate full-thickness skin grafting (IFTSG). Despite previous publication of our experience with this technique in 1999, awareness and implementation by surgeons remains poor. Outcomes of patients who underwent IFTSG from traumatically avulsed extremity soft tissues over a 10-year period were reviewed, with technical considerations included. METHODS: A retrospective cohort study of pa

Undiagnosed mandibular degloving following dental trauma.

A 12-year-old girl presented to the Department of Paediatric Dentistry of the Toulouse University Hospital 10 days after a facial injury, complaining of persistent pain and halitosis. Previous examinations only reported trauma of the upper incisors. Further clinical exploration showed an 8 cm laceration of the oral mucosa in the lower labial sulcus, 1.5 cm inferior to the gingival margin and exposing the symphyseal bone. The diagnosis of mental protuberance degloving was established. The treatment consisted of debridement of the wound, removal of the necrotic tissue, multiple plane sutures and
