BACKGROUND: Benzodiazepine receptor agonist (BZRA) use is highly prevalent in hospitalised older people although these drugs are associated with numerous and serious adverse events. Deprescribing can reduce risks associated with chronic BZRA use. The aim of this study was to measure the prevalence of, and factors associated with, BZRA deprescribing in acute geriatric units.
METHODS: During a one-year period, this multicentre retrospective study included patients aged ≥70 years, hospitalised in acute geriatric units, and using ≥1 BZRA on admission. BZRA deprescribing at discharge was defined a

INTRODUCTION: We describe a case of olanzapine-induced hypothermia and hand oedema in an older adult with behavioural and psychological symptoms of dementia (BPSD).
CASE PRESENTATION: An 82-year-old woman with hypothyroidism and dementia was reviewed by the geriatric team at a nursing home in view of lethargy and an unrecordable oral temperature. She was noted to have a bilateral hand oedema and right basal crackles. Investigations revealed high white cell count and inflammatory markers. She was treated as per hypothermia and communityacquired pneumonia protocols. The patient did not have the

BACKGROUND: Older patients using antihypertensive medication may experience Adverse Drug Events (ADEs), and thus benefit from deprescribing. The lack of a practical protocol may hamper deprescribing. Therefore, we aimed to develop a deprescribing protocol, based on a review of literature, combined with a feasibility test in a small number of patients.
METHODS: A deprescribing protocol for general practitioners was drafted and tested in older patients using multiple antihypertensive medication in a single arm intervention. Patients were included if they were 75 years or older, were using two o

BACKGROUND: Polypharmacy, defined as being prescribed 5 or more medications, has been shown to be associated with a decline in mental and physical functioning in elderly patients. Despite this, elderly patients are currently being prescribed a median of 7 medications, which often causes more harm than benefit, and emphasizes the importance of deprescribing.
METHODS: Five classes of potentially inappropriate medications for the elderly population, especially for the aging brain, are discussed, including anticholinergics, benzodiazepines, antipsychotics, opioids, and proton pump inhibitors. Rec

Polypharmacy encompasses inappropriate prescribing, overprescribing, and underprescribing. Polypharmacy in older adults is quite common and contributes to adverse effects, drug interactions, hospital admissions, and functional and cognitive decline. Efforts to reduce polypharmacy include criterion-based management tools, deprescribing protocols, and the use of electronic clinical decision support tools.

OBJECTIVE: Polypharmacy and long-term preventive medication use are common in frail older adults with limited life expectancy, despite uncertain benefits and potential risks. This systematic review and meta-analysis synthesized evidence on the effect of deprescribing preventive medications (antihypertensives, statins, anticoagulants, and antidiabetic agents) compared to continuation on clinical, physiological, safety, and patient-centered outcomes among older adults with advanced frailty, dementia, or limited life expectancy. METHODS: PubMed, Embase, Cochrane Library, Web of Science, CINAHL, a

INTRODUCTION: 37%-48% of older patients receive anticholinergic medicines, increasing the risk for falls, delirium, dementia and death. Hospital admission may provide an opportunity for deprescribing to improve outcomes. This study evaluated the feasibility and acceptability of a digital tool to identify anticholinergic medicines and support deprescribing.
METHODS: A pre-post feasibility study was conducted (May-September 2025). Patients aged ≥ 75 with an ACB score of ≥ 3, screened using a digital tool, were recruited. The identified medications were highlighted to the clinical team to prompt

Introduction: Deprescribing is crucial for improving pharmacotherapeutic safety in polymedicated patients, a common profile in community pharmacy. However, a gap exists between the available evidence and real-world clinical practice. Objective: This review aimed to identify and critically analyze validated deprescribing tools, assessing their applicability in the community setting and highlighting the leading role the pharmacist can assume. Material and Methods: A critical narrative review was conducted via a literature search in PubMed/MEDLINE and LILACS, period 2014-2024, using the terms MeS

There is a place for the use of benzodiazepines/z-hypnotics on adult mental health wards, but they are often continued beyond a length of time where they are beneficial. This can result in dependence and withdrawal effects if stopped. Timely deprescribing of these medicines is encouraged, but there is limited evidence available as to what can be a facilitator or barrier to deprescribing benzodiazepines/z-hypnotics on these wards. Semi-structured interviews, with twenty-nine NHS healthcare professionals from eleven different NHS organisations, involved in the use of benzodiazepines/z-hypnotics

BACKGROUND: Polypharmacy (concomitant prescription of ≥ 5 medications) affects a third of older people, and evidence suggests an association with sarcopenia (loss of skeletal muscle mass/quality, muscle strength, and/or physical performance). As such, deprescribing has been recommended in routine management of sarcopenia, however it's unknown whether deprescribing is beneficial. This systematic review aimed to understand effects of deprescribing on sarcopenia parameters in older adults.
METHODS: Medline, Embase, CINAHL, Web of Science, and the Cochrane Library databases were searched up to Ju

This is a protocol for a Cochrane Review (intervention). The objectives are as follows: The primary objective is to assess the effects of implementation interventions aimed at improving the uptake of evidence-based opioid deprescribing by healthcare professionals for adults with chronic non-cancer pain, compared with no intervention. The secondary objectives are: To explore whether the effects vary by intervention type (e.g. audit and feedback), target (e.g. healthcare organisations), setting of implementation intervention (e.g. primary care) and duration of opioid use (e.g. < 12 months, ≥ 12

BACKGROUND: Limited research has evaluated the validity of claims-based definitions for deprescribing.
OBJECTIVES: Evaluate the validity of claims-based definitions of deprescribing against electronic health records (EHRs) for deprescribing of benzodiazepines (BZDs) after a fall-related hospitalization.
METHODS: We used a novel data linkage between Medicare fee-for-service (FFS) and Part D with our health system's EHR. We identified patients aged ≥66 years with a fall-related hospitalization, continuous enrollment in Medicare FFS and Part D for 6 months pre- and post-hospitalization, and ≥2

OBJECTIVES: Geriatric palliative care approaches support deprescribing of antihypertensives in older nursing home (NH) residents with limited life expectancy and/or advanced dementia (LLE/AD) who are intensely treated for hypertension (HTN), but information on real-world deprescribing patterns in this population is limited. We examined the incidence and factors associated with antihypertensive deprescribing.
DESIGN: National, retrospective cohort study.
SETTING AND PARTICIPANTS: Older Veterans with LLE/AD and HTN admitted to VA NHs in fiscal years 2009-2015 with potential overtreatment of HT

OBJECTIVE: Benzodiazepines and anticholinergics are risk factors for delirium in the intensive care unit (ICU). We tested the impact of a deprescribing intervention on short-term delirium outcomes.
DESIGN: Multisite randomized clinical trial.
SETTING: ICUs of three large hospitals.
PARTICIPANTS: Two hundred adults aged 18 years or older and admitted to an ICU with delirium, according to the Richmond Agitation-Sedation Scale and the Confusion Assessment Method for the ICU (CAM-ICU). Participants had a contraindication to haloperidol (seizure disorder or prolonged QT interval) or preference a

Proton pump inhibitors (PPIs) are commonly prescribed for treatment of acid-related gastrointestinal disorders. PPI use is approved for a duration of 2 to 12 weeks, depending on complication and severity. Many users lack an appropriate indication for continued therapy past the recommended duration. Long-term PPI use is associated with several adverse drug events, including acute interstitial nephritis, fractures, and Clostridium difficile-associated diarrhea (CDAD). Cautious prescribing and regular monitoring is essential, especially for older adults, as they may be at higher risk for these ad

BACKGROUND: Anticoagulation therapy reduces stroke risk in patients with atrial fibrillation (AF), but it is often underused in older populations due to concerns about bleeding. This study aimed to compare the safety and effectiveness of anticoagulation during periods of exposure and non-exposure and across different anticoagulants in people with AF aged ≥75 years.
METHODS: Using UK primary care data from the Clinical Practice Research Datalink (2013-2017), a retrospective cohort study was conducted on patients newly prescribed oral anticoagulants (warfarin or direct oral anticoagulants). Exp

OBJECTIVE: Regarding older patients, multiple chronic conditions lead to the intake of multiple medications, involving a higher risk of adverse drug events. In older patients with advanced chronic kidney disease, the medication exposure was poorly explored. The aim of this study was to describe the use of potentially inappropriate medications and medications with anticholinergic and sedative properties in older community-dwelling patients with advanced chronic kidney disease.
METHODS: An observational study was conducted in a geriatric day-care unit. All patients aged over 65 years with advan

BACKGROUND: The loss of meaning in life often underpins a desire to die, while rediscovering purpose and forming relationships can restore the will to live. This pursuit of meaning and the ability to express one's identity are central to both rehabilitation and palliative care. This case report examines the integration of these care principles within the framework of "Omsorg," a Danish word and concept of care rooted in genuine acts of kindness, as implemented at Dagmarsminde, a nursing home for residents with moderate to severe dementia.
