Stratifying Risk for Postpartum Depression at Time of Hospital Discharge.

OBJECTIVE: Postpartum depression (PPD) is a major contributor to postpartum morbidity and mortality. Beyond efforts at routine screening, risk stratification models could enable more targeted interventions in settings with limited resources. The authors sought to develop and estimate the performance of a generalizable risk stratification model for PPD in patients without a history of depression, using information collected as part of routine clinical care. METHODS: The authors conducted a retrospective cohort study of all individuals who delivered between 2017 and 2022 in one of two large aca

Identifying postpartum depression: Using key risk factors for early detection.

BACKGROUND: Personal and family history of psychiatric disorders are key risk factors for postpartum depression (PPD), yet their combined contribution has been understudied. OBJECTIVE: To examine personal and family psychiatric history, alone and combined, and their effect on absolute risk and relative risk (RR) of mild/moderate or severe PPD. METHODS: In this cohort study, we used data from 142 064 childbirths with PPD screenings from 2015 to 2021 merged with population registers. Exposures were personal and family psychiatric history defined as a psychiatric hospital contact or psychotropi

Prenatal exposure to ambient air pollution and persistent postpartum depression.

BACKGROUND: Ambient air pollution during pregnancy has been linked with postpartum depression up to 12 months, but few studies have investigated its impact on persistent depression beyond 12 months postpartum. This study aimed to evaluate prenatal ambient air pollution exposure and the risk of persistent depression over 3 years after childbirth and to identify windows of susceptibility. METHODS: This study included 361 predominantly low-income Hispanic/Latina participants with full-term pregnancies in the Maternal and Developmental Risks from Environmental and Social Stressors (MADRES) cohort

Bilirubin and postpartum depression: an observational and Mendelian randomization study.

BACKGROUND: Postpartum depression (PPD) is one of the most common complications of delivery and is usually disregarded. Several risk factors of PPD have been identified, but its pathogenesis has not been completely understood. Serum bilirubin has been found to be a predictor of depression, whose relationship with PPD has not been investigated. METHODS: Observational research was performed followed by a two-sample Mendelian randomization (MR) analysis. From 2017 to 2020, the clinical data of pregnant women were retrospectively extracted. Logistic regression and random forest algorithm were emp

Linking the reversal of gestational insulin resistance to postpartum depression.

BACKGROUND: Postpartum depression (PPD) constitutes a significant mental health disorder affecting almost one fifth of pregnancies globally. Despite extensive research, the precise etiological mechanisms underlying PPD remain elusive. However, several risk factors like genetic predisposition, hormonal fluctuations, and stress-related environmental and psychosocial triggers have been found to be implicated in its development. MAIN: Recently, an increased risk of PPD has been reported to be associated with gestational diabetes mellitus (GDM), which is characterized by the disruption of glucose m

Emotional Freedom Techniques for Postpartum Depression, Perceived Stress, and Anxiety.

OBJECTIVE: To examine the effect of an Emotional Freedom Techniques (EFT) group intervention on perceived stress, depression, and anxiety symptoms in women in the postpartum period. DESIGN: A one-group pre- and posttest quasi-experimental design. SETTING/LOCAL PROBLEM: Postpartum depression (PPD) and anxiety affect nearly one in five women in the first 12 months after childbirth. The COVID-19 pandemic was associated with a significant worsening of symptoms of PPD, stress, and anxiety in this population. Mental health screening is the standard of care in perinatal settings. Association between maternity care practitioner type and postpartum depression screening.

BACKGROUND: Postpartum depression (PPD) is increasingly common in the United States and poses a significant threat to maternal and neonatal health. Universal screening for postpartum depression is recommended by numerous organizations, including the American College of Obstetricians and Gynecologists, but is not achieved in practice. METHODS: A cross-sectional, weighted, state-representative study of California residents who gave birth in 2016 using the Listening to Mothers in California 2018 data set. Pharmacotherapy of Postpartum Depression: Current Approaches and Novel Drug Development.

Postpartum depression is one of the most common complications of childbirth. Untreated postpartum depression can have substantial adverse effects on the well-being of the mother and child, negatively impacting child cognitive, behavioral, and emotional development with lasting consequences. There are a number of therapeutic interventions for postpartum depression including pharmacotherapy, psychotherapy, neuromodulation, and hormonal therapy among others, most of which have been adapted from the treatment of major depressive disorder outside of the peripartum period. Current evidence of antide

PPD ACT: an app-based genetic study of postpartum depression.

Postpartum depression (PPD) is one of the most frequent complications of childbirth and particularly is suited to genetic investigation as it is more homogenous than major depression outside of the perinatal period. We developed an iOS app (PPD ACT) to recruit, consent, screen, and enable DNA collection from women with a lifetime history of PPD to sufficiently power genome-wide association studies. In 1 year, we recruited 7344 women with a history of PPD and have biobanked 2946 DNA samples from the US. This sample of PPD cases was notably severely affected and within 2 years of their worst epi

Lower allopregnanolone during pregnancy predicts postpartum depression: An exploratory study.

Current evidence is mixed on the role of progesterone and its metabolites in perinatal mood and anxiety disorders. We measured second and third trimester (T2 and T3) progesterone (PROG) and allopregnanolone (ALLO) levels by ELISA and postpartum depression (PPD) by clinician interview (DSM-IV criteria) in 60 pregnant women with a prior diagnosis of a mood disorder. Methods included multivariate and logistic regression with general linear mixed effect models. We found that, after adjustment, every additional ng/mL of T2 ALLO resulted in a 63% (95% CI 13% to 84%, p=0.022) reduction in the risk of

Probable postpartum hypomania and depression in a South African cohort.

The aim of this study was to evaluate the frequency of, and factors associated with, postpartum hypomania (PPH) and postpartum depression (PPD) in a South African sample. Data from 57 women were analysed as part of a larger prospective study of maternal stress in pregnancy. On day 3 postpartum, women were assessed for probable PPH using the Highs scale. On day 3 and at week 6, probable PPD was assessed using the Edinburgh Postnatal Depression Scale (EPDS), while social support was evaluated using the Multidimensional Scale of Perceived Social Support (MSPSS). PPH was present in 49.1% of the pa

Impact of childhood trauma on postpartum depression: a prospective study.

Studies on the impact of childhood trauma on postpartum depression show inconsistencies and methodological limitations. The present study examines the effect of childhood trauma on depression 12 and 24 weeks after childbirth, while controlling for history of depression, depression symptoms during pregnancy and type D personality. During the third trimester of pregnancy, 210 women completed self-report questionnaires assessing depression (current and/or past episodes), childhood trauma and type D personality, of whom 187 participated in the postpartum follow-up, with depression symptoms being r

Postpartum depression, marital dysfunction, and infant outcome: a longitudinal study.

This longitudinal study explores the relationship of postpartum depression (PPD) and marital dysfunction on infant outcomes from birth to 2 1/2 years of age among middle-class, postpartum women. Participants were recruited during the prenatal period. Twelve mothers completed the study throughout a 2 1/2-year period. Questionnaires, semistructured interviews, and observations were used to collect data. Content analysis of the interviews (Morse & Field, 1995) was conducted and thematic patterns were identified. Clinical PPD and marital dysfunction (defined as little or no support or closeness, o

Associations between anemia and postpartum depression among Arab Bedouin women.

BACKGROUND: Anemia and postpartum depression (PPD) are common perinatal morbidities. Recognition of postpartum anemia as a major risk factor for PPD is crucial for postpartum maternal health. This study aimed to examine the association between anemia and PPD in low-income minority Arab-Bedouin mothers in southern Israel. METHODS: The study was conducted in 2017-2018 and included 332 women aged 18 to 45 years who were 2-4 months postpartum. Retrospective data on postpartum anemia (defined as a hemoglobin level < 12 g/dL) was extracted from women's medical records in the clinics. PPD was measur

Postpartum Depression: Overlap Between Symptom-Based and Register-Based Measures.

INTRODUCTION: Postpartum depression (PPD) can be measured in various ways, including questionnaire-based symptom scores or through administrative health databases. We examined the overlap between symptom-based and register-based PPD definitions and assessed the consistency in exposure-outcome associations. METHODS: We linked Danish nationwide health registers to PPD screening records (Edinburgh Postnatal Depression Scale, EPDS) at 2 months postpartum from women screened between January 1, 2015, and December 31, 2021. We defined symptom-based PPD as an EPDS ≥ 11, and register-based PPD as an a
