Cutaneous melioidosis: An updated review and primer for the dermatologist.

Melioidosis is an emerging infection with increasing endemic foci and global distribution. It is underrecognized and underdiagnosed because of factors including limited awareness of the disease, nonspecific clinical presentation, lack of diagnostic facilities in some locations, misidentification in laboratories inexperienced with culture, and identification of Burkholderia pseudomallei. Cutaneous findings are reported in approximately 10% to 20% of melioidosis cases and dermatologists may play a significant role in its recognition and management. The most dynamic situation of melioidosis recog

Harmful alcohol use and alcohol dependency can be defined by the updated ICD-11 coding system and screening can be completed using many tools including the Cut down, Annoyed, Guilty, Eye-Opener (CAGE), Alcohol Use Disorders Identification Te

Chronic lymphocytic leukemia and the skin: implications for the dermatologist.

B-cell chronic lymphocytic leukemia (CLL) is the most prevalent leukemia in the United States, and its diagnosis can have many dermatologic implications. For one, the cutaneous manifestations of CLL include several entities, most notably leukemia cutis, eosinophilic dermatosis of hematologic malignancy, and a heightened risk of skin infections. Additionally, CLL patients are at an increased risk of secondary malignancies, most commonly of the skin. Furthermore, a number of commonly utilized treatments for CLL have cutaneous implications which should be considered in the interdisciplinary manag

Melanoma in the setting of nevus of Ota: a review for dermatologists.

Nevus of Ota, also known as oculodermal melanocytosis or nevus fuscoceruleus ophthalmomaxillaris, is a benign dermal melanocytic nevus that most commonly affects Asian women. While the lesion is largely a cosmetic concern, it has the potential to undergo malignant transformation to melanoma. Patients with nevus of Ota often present to a dermatologist at some point for examination or removal with laser therapy. Therefore, it is important for dermatologists to understand the cutaneous and extracutaneous risks these lesions carry and how they may present to further optimize management and appropr

Update in Herpes Zoster Prevention and the Role of Dermatologists.

A recombinant vaccine (HZ/su) was approved in 2017 to prevent herpes zoster (HZ) infection and associated sequelae with greater efficacy and safety than its live precursor. Though dermatologists regularly encounter patients with HZ infection, recommendation of vaccination by dermatologists and other physicians has been minimal in past years. Overall patient awareness and utilization of the HZ vaccines has subsequently been low. While HZ/su touts several improvements over the live vaccine, dermatologists still face obstacles to vaccine recommendation and administration including concerns of eff

The role of the dermatologist in Raynaud's phenomenon: a clinical challenge.

Raynaud's phenomenon (RP) is a functional vascular disorder involving extremities. In his practice, the dermatologist may frequently encounter RP which affects mainly women and is categorized into a primary benign form and a secondary form associated with different diseases (infections, drugs, autoimmune and vascular conditions, haematologic, rheumatologic and endocrinologic disorders). Still today, the differential diagnosis is a clinical challenge. Therefore, a careful history and a physical examination, together with laboratory tests and nailfold capillaroscopy, is mandatory. RP is generall

Enthesitis and Dactylitis in Psoriatic Disease: A Guide for Dermatologists.

Psoriatic arthritis (PsA) is an inflammatory arthritis that is estimated to affect approximately 30% of patients with psoriasis. Enthesitis and dactylitis, two hallmarks of PsA, are associated with radiographic peripheral/axial joint damage and severe disease. Clinical symptoms of enthesitis include tenderness, soreness, and pain at entheses on palpation, whereas dactylitis is recognized by swelling of an entire digit that is different from adjacent digits. Both ultrasound and magnetic resonance imaging can be used to diagnose enthesitis and dactylitis, especially in patients in whom symptoms

Bird's Nest View from a Dermatologist's Eye.

Plica neuropathica also known as "birds nest" is irreversible matting of hair seen in otherwise healthy individuals and is usually associated with use of ionic/herbal soaps, shampoos, parasitic infestations, psychiatric alterations, or neglect. Anagen effluvium is a known side effect of many immunosuppressive drugs. Rarely matting of hair has been reported following azathioprine. We report plica neuropathica in two chronically ill patients on immunosuppressive drugs, that is, azathioprine and methotrexate along with a review of literature of its etiological factors and pathogenesis.

[Andrologically relevant findings from the perspective of the dermatologist].

In clinical practice dermatologists may be faced with andrological findings raising interdisciplinary problems. Consideration of them may not only improve the clinical course of the patient but also the dermatologist's interdisciplinary standing. This overview describes and explains the most important andrological findings, which may be apparent in a dermatologic whole-body examination.

Hereditary angioedema: diagnosis and management-a perspective for the dermatologist.

Hereditary angioedema (HAE) is a relatively rare, but potentially life-threatening genetic disorder characterized by marked, diffuse mucosal edema that, in extreme cases, can affect the airway leading to asphyxiation. The clinical picture is similar to that of other forms of angioedema; therefore, misdiagnosis or delayed diagnosis is common. HAE is caused by a deficiency in, or a dysfunction of, C1 esterase inhibitor, which has a wide variety of physiologic functions, of which regulation of the contact (kallikrein-kinin) system is most relevant to this condition. Effective management of HAE mu

[Differential diagnosis of CRPS from a dermatologist's viewpoint].

Because of clinical similarities many dermatological diseases must be differentiated from both the acute and chronic phases of the complex regional pain syndrome (CRPS). The differential diagnosis of CRPS in the acute phase includes erythema migrans, early inflammatory lesions of acrodermatitis chronica atrophicans, hypodermitis in chronic venous insufficiency, eosinophilic fasciitis, erythromelalgia and an acute contact dermatitis. The chronic phase of CRPS with trophic disturbances of the skin, joints, and bones needs to be distinguished from the atrophic stage of acrodermatitis chronica atr

The dermatologist's guide to hereditary syndromes with renal tumors.

Patients with hereditary syndromes with renal tumors initially may present to the dermatologist. It is essential that dermatologists recognize these syndromes because the early diagnosis of renal cancer may prove to be lifesaving. The 4 hereditary syndromes with cutaneous manifestations are von Hippel-Lindau (VHL) syndrome, Birt-Hogg-Dube (BHD) syndrome, tuberous sclerosis (TS), and hereditary leiomyoma renal cell carcinoma (HLRCC) syndrome. This article reviews these disorders, emphasizing their cutaneous features and renal manifestations.
