Therapy of polymyositis and dermatomyositis.

High-dose oral prednisone (at an initial dose of 1mg/kg/day) is the mainstay of therapy for PM/DM, and should be subsequently tapered slowly based on patients' clinical response. First-line combination of prednisone with intravenous immunoglobulins may be considered in patients with PM/DM-related severe systemic complications, especially in the subgroup exbititing life-threatening esophageal involvement. In patients who failed to respond to prednisone, the first-line immunosuppressive therapy includes methotrexate or azathioprine. Intravenous immunoglobulin therapy should be considered in pati

Polymyositis and dermatomyositis: pathophysiology.

Recent advances have increased the understanding of the pathogenesis of polymyositis and dermatomyositis. Clearly, the pathogenesis is complex, and adaptive (eg, autoimmune) and innate and nonimmune pathways play a role in the disease mechanisms, but the relative contribution may vary between patients and in different phases of the disease. Phenotyping patients using autoantibody profiling has resulted in information on molecular pathways that may be relevant in certain subsets of patients with polymyositis or dermatomyositis, but combining the autoantibody profiles with molecular signatures o

Immunopathogenesis of juvenile dermatomyositis.

There is increasing evidence for involvement of the mechanisms of the innate immune system in the pathogenesis of idiopathic inflammatory myopathies (IIMs), especially in the adult and juvenile forms of dermatomyositis. Juvenile dermatomyositis (JDM) is the most common form of childhood IIM, and this review focuses on recent advances in understanding the actions of the innate immune system in this condition. Over the last few years, great strides have been made in understanding immune dysregulation in IIM, including JDM. Novel autoantibodies have been identified, and new genetic contributions

[New autoantibodies in dermatomyositis].

Dermatomyositis is an idiopathic inflammatory myopathy that affects skeletal muscle and the skin. Idiopathic inflammatory myopathies are characterized by the production of autoantibodies directed against different cell structures. Some of these autoantibodies are specific to idiopathic inflammatory myopathies (myositis-specific antibodies) whereas others are found in a range of overlap syndromes (myositis-associated antibodies). Although they are all associated with certain clinical and physiopathological characteristics of myositis, myositis-specific antibodies are essentially the most useful

Tuberculous myofasciitis in dermatomyositis.

Extraspinal musculoskeletal tuberculosis (TB) was an uncommon manifestation with a frequency about 1 to 2% in all TB patients. According to the report, tuberculous arthritis and osteomyelitis remained the two leading etiologies, while others constituted less than 2% of all extraspinal musculoskeletal tuberculous infections; less than 0.04% of those who suffered from TB were represented as extraspinal musculoskeletal tuberculous infections, neither tuberculous arthritis- nor osteomyelitis-associated. We presented one extremely rare case of primary tuberculous myofasciitis manifesting painful sw

Dermatomyositis and supraventricular tachycardia.

BACKGROUND: Dermatomyositis is an idiopathic inflammatory myopathy, often associated with an underlying malignancy. Its prevalence rate is approximately one per 100,000 in the general population, and is even rarer without evidence of a cancer. Dermatomyositis rarely involves myocardial muscle fibers, but has shown to be associated with cardiac arrhythmias. CASE PRESENTATION: We present a case of a young female patient with known history of dermatomyositis who presented to hospital with a flare up of her disease. She also complained of paroxysms of palpitation. Telemetry monitoring revealed se

Immunomodulatory treatment for dermatomyositis.

Dermatomyositis and polymyositis are classified as members of idiopathic inflammatory myopathies. It has recently become evident that the pathogenesis of the muscle disease for these two entities is likely not the same. Despite this observation, therapies directed at the muscle disease are similar, although therapy for the skin differs slightly. Corticosteroids remain the initial approach to management. The systemic dose of corticosteroids needed to control the disease, particularly the skin disease, is quite high; therefore, most authorities add a corticosteroid-sparing agent early. Some pati

Dermatomyositis and esophageal cancer.

OBJECTIVE: A case of dermatomyositis and esophageal cancer is described. CASE REPORT: A 58-year-old male recently diagnosed with esophageal cancer was admitted to the hospital with complaints of progressive dysphagia, generalized muscle weakness and skin rash. The weakness started symmetrically in the proximal limb muscles. He also developed a characteristic skin rash on the eyelids, the upper chest and around the nails that was thought to be most indicative of dermatomyositis. Creatine kinase, aspartate aminotransferase and aldolase were elevated. A muscle biopsy showed various degrees of de

The heart in dermatomyositis and polymyositis.

Cardiovascular manifestations constitute a major cause of death in myositis. Despite this, clinically manifest cardiac involvement in polymyositis and dermatomyositis is relatively rare. In contrast, subclinical manifestations are frequently reported and are predominated by conduction abnormalities and arrhythmias detected by ECG. The most frequently reported clinically overt manifestations are congestive heart failure, conduction abnormalities, that may lead to complete heart block, and coronary artery disease. The underlying pathophysiological mechanisms that may cause cardiac manifestations

Pruritus in adult dermatomyositis.

Dermatomyositis has a significant clinical component of pruritus that has not yet been studied. Pruritus can significantly affect the life of patients. The aim of the present work was to study the degree of pruritus experienced by patients. A four-question survey was sent to patients with documented dermatomyositis. The survey used a 100-mm Visual Analogue Scale (VAS) to describe current, worst and daily pruritus, and the effect this has on daily activities. Twenty-six subjects returned completed questionnaires: four had no pruritus; the majority had a significant amount with means above 50 on

Dermatomyositis with tuberculous fasciitis.

A case of dermatomyositis with tuberculous fasciitis is described. A 69-year-old Japanese man treated with prednisolone for dermatomyositis developed erythema and bilateral swelling of arms and forearms. A diagnosis of tuberculous fasciitis was confirmed by the presence of acid-fast bacilli in a biopsy specimen and the growth of Mycobacterium tuberculosis from tissue cultures. The unusual presenting clinical features resulted in delayed diagnosis. We emphasize that tuberculous fasciitis should be considered in an immunocompromised patient who is treated with steroids, especially if the results

Dermatomyositis: evolution of a diagnosis.

BACKGROUND AND PURPOSE: As direct access evolves, physical therapists will increasingly encounter patients with pathology that might have an underlying systemic origin. The purpose of this case report is to describe the diagnostic process that led a patient's physical therapist to recognize signs and symptoms of dermatomyositis. CASE DESCRIPTION: The patient was an 18-year-old woman who was referred for physical therapy by her primary care physician on 3 occasions with 3 separate musculoskeletal diagnoses. During the third episode, the physical therapist recognized signs and symptoms that cou
