Syphilis Point-of-Care Tests (POCTs): Implementation Considerations in Canada.

Starting in the early 2000s, the rate of infectious syphilis in Canada began rising. Since the mid-2010s, the rate among females, particularly females in their reproductive years, has increased rapidly. Coincident with this shift, congenital syphilis has re-emerged in Canada in recent years. Infectious and congenital syphilis disproportionately impact communities affected by social and structural determinants of health, including homelessness and inadequate or inaccessible health care services. These realities underscore the importance of emerging strategies, including rapid diagnostic tests t

Severe Acute Respiratory Syndrome-Coronavirus-2 (SARS-CoV-2): A Perspective Through the Lens of the Veterinary Diagnostic Laboratory.

The SARS-CoV-2 pandemic has resulted in unprecedented challenges to veterinary diagnostic laboratories. These challenges include partial or complete shutdowns, interrupted courier services, disruptions in workflow and diagnostic testing, new physical distancing practices, protocol development or enhancement for handling samples from high-risk or susceptible species, and fulfilling requirements for pre-test permission approval from state and federal veterinary agencies, all of which have been implemented to prevent or minimize exposure and transmission of SARS-CoV-2 locally or regionally. As in

Intramuscular injection of sevoflurane detects malignant hyperthermia predisposition in susceptible pigs.

BACKGROUND: The authors hypothesized that intramuscular sevoflurane injection allows diagnostic differentiation between malignant hyperthermia-susceptible (MHS) and -nonsusceptible (MHN) pigs by measurement of intramuscular lactate and carbon dioxide partial pressure (PCO2), and that dantrolene reduces the sevoflurane-induced PCO2 increase. METHODS: With approval of the local animal care committee, microdialysis probes with attached microtubing for sevoflurane injection were placed in the adductor muscles of nine MHS and six MHN pigs, and PCO2 probes with microtubing were positioned in the tr

Novel and optimized diagnostics for pediatric TB in endemic countries: NOD-pedFEND study protocol.

BACKGROUND: Pediatric tuberculosis is a major global public health challenge, with reliable diagnosis being a main obstacle to identifying and treating affected children. New and improved diagnostics, ideally on non-sputum samples, are urgently required, especially in the most vulnerable group of children under five years of age. Studies to date have been limited by small sample sizes and few bacteriologically-confirmed cases. Here, we describe the study protocol of the NIH-funded NOD-pedFEND study, which will be one of the largest diagnostic studies to date of children at greatest risk of tub

Rapid and Accurate Diagnosis of Pediatric Tuberculosis Disease: A Diagnostic Accuracy Study for Pediatric Tuberculosis.

INTRODUCTION: An estimated 1.2 million children develop tuberculosis (TB) every year with 240,000 dying because of missed diagnosis. Existing tools suffer from lack of accuracy and are often unavailable. Here, we describe the scientific and clinical methodology applied in RaPaed-TB, a diagnostic accuracy study. METHODS: This prospective diagnostic accuracy study evaluating several candidate tests for TB was set out to recruit 1000 children <15 years with presumptive TB in 5 countries (Malawi, Mozambique, South Africa, Tanzania, India). Assessments at baseline included documentation of TB sign

[Adverse events of immune checkpoint inhibitors].

After immune checkpoint inhibitor therapy was approved for renal cell carcinoma last year, this new immune therapy has spread to urology. Further approvals (atezolizumab, nivolumab, pembrolizumab) are expected in 2017 for metastatic urothelial carcinoma that has progressed following treatment with platinum-based chemotherapy. With expanding use of immune checkpoint inhibitors, experience in diagnosing and managing immune-mediated adverse events increases. Although of low incidence, grade 3/4 toxicities play a central role. Organs most common for immune-mediated adverse events are skin, liver (

Evaluation of the Alinity m Resp-4-Plex Assay for the Detection of Severe Acute Respiratory Syndrome Coronavirus 2, Influenza A Virus, Influenza B Virus, and Respiratory Syncytial Virus.

The rapid emergence of the coronavirus disease 2019 (COVID-19) pandemic has introduced a new challenge in diagnosing and differentiating respiratory infections. Accurate diagnosis of respiratory infections, including severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), is complicated by overlapping symptomology, and stepwise approaches to testing for each infection would lead to increased reagent usage and cost, as well as delays in clinical interventions. To avoid these issues, multiplex molecular assays have been developed to differentiate between respiratory viruses in a single tes

Repurposing HER2 IHC: Pan-HER2 and Low-HER2.

The recent clinical trials and regulatory approval of new antibody drug conjugates targeting HER2 protein (trastuzumab deruxtecan, T-DXd) has moved the bar in terms of therapy for breast cancer and other solid tumors. With new drugs and expanding indications comes a need for refined companion diagnostics. Pathologists are asked to adapt IHC and other assays (mutation testing, circulating tumor DNA) accordingly. This brings attention to lower HER2 expression levels in breast cancer ("low" and even "ultralow"), and a tumor agnostic approval for cancers with high HER2 expression (3+ according to

Novel Treatment Paradigms in Acute Myeloid Leukemia.

Acute myeloid leukemia (AML) is a heterogeneous disease marked by the presence of several driver mutations and molecular subgroups even in a single patient. The genetic and molecular heterogeneity is also reflected by a progressive shift from a morphologic classification to one informed by causative genomic changes. Cytogenetic results and somatic mutations are increasingly being utilized to guide use of intensive chemotherapy and low-intensity chemotherapy, particularly among older adults. Utilization of next-generation sequencing in AML has led to increasing use of targeted treatments for ac

Conception and stages of development of the microbiological cancer test.

Pathology and experimental bacteriology have, in scientific medicine, created fundamental methodical presuppositions for the description and recognition of etiologic entities. With the tumour-tetanus phenomen, two pathogenic principles--cancer growth and tetanus infection--coincide in a so far unknown manner. Analytical experiments have led to the novel idea of the mitosis-stimulated anaerobic tetanus infection. This specific pathomechanism and its mathematical model formed the basis for the development of the serological Clostridium-tumour test with nontoxic, apathogenic Butyricus Clostridia

A 5-year review of genomic medicine in breast cancer: insights from C-CAT data on 3776 Japanese patients.

BACKGROUND: In Japan, despite 5 years since CGP tests were covered by insurance in 2019, low drug accessibility rates remain a critical issue. We evaluated drug accessibility in 3776 breast cancer from the C-CAT database using two criteria: the proportion first linked to PMDA-approved drugs with phase III trial evidence for breast cancer through CGP tests but not existing Companion diagnostics [CDx] (*), and the proportion first linked to PMDA-approved drugs including based on phase I and II trial evidence (**). Additionally, cases linked to investigational drugs for non-PMDA-approved drugs we

Current practices for assessing usability of novel point-of-care diagnostics for infectious diseases: a scoping review protocol.

INTRODUCTION: Novel diagnostics, particularly point-of-care (POC) tests, play a crucial role in the early detection and management of infectious diseases, especially in resource-limited settings. Ensuring test performance and quality while minimising the risk of human error becomes more relevant when shifting testing tasks from highly controlled settings like centralised laboratories to people with minimal training. Applying usability and human factors engineering principles can reduce the challenges related to human errors. Despite existing frameworks and tools, the practical application of u
