Evaluating the potential costs and impact of digital health technologies for tuberculosis treatment support.

Ensuring adherence and support during treatment of tuberculosis (TB) is a major public health challenge. Digital health technologies could help improve treatment outcomes. We considered their potential cost and impact on treatment for active or latent TB in Brazil.Decision analysis models simulated two adult cohorts with 1) drug-susceptible active TB, and 2) multidrug-resistant TB, and two cohorts treated with isoniazid for latent TB infection (LTBI): 1) close contacts of persons with active TB, and 2) others newly diagnosed with LTBI. We evaluated four digital support strategies: two differen

Isoniazid vs. rifampin for latent tuberculosis infection in jail inmates: toxicity and adherence.

This open-label randomized trial compared isoniazid (9 months) to rifampin (4 months) on toxicity and completion in a jailed population with latent tuberculosis infection. Rifampin resulted in fewer elevated liver function tests (risk ratio [RR] 0.39, 95% confidence interval [CI] [0.18, 0.86]) and less toxicity requiring medication withdrawal (RR 0.51, 95% CI [0.13, 2.01]), although one participant receiving rifampin experienced an allergic reaction. Completion was achieved for 33% receiving rifampin compared to 26% receiving isoniazid (p = .10). With careful monitoring rifampin is a safe and

Trends of Zambia's tuberculosis burden over the past two decades.

OBJECTIVES: To study trends in Zambia's TB notification rates between 1990 and 2010 and to ascertain progress made towards TB control. METHODS: Retrospective review of TB notification returns and TB programme reports for the period from 1990 to 2010. RESULTS: Two distinct TB trend periods were identified: a period of rising trends up to a peak between 1990 and 2004 and a period of moderately declining trends between 2004 and 2010. Treatment outcomes improved over the two decades. Data on trends in paediatric TB, TB in prisoners and TB in pregnant women remain scanty and unreliable owing to p

Fighting the tuberculosis epidemic in the Western Pacific region: current situation and challenges ahead.

INTRODUCTION: Tuberculosis (TB) remains a major public health problem in the Western Pacific Region. More than 20% of the global burden of TB is found in the Region. In 2007, the latest year for which data is available, there were an estimated 1.9 million incident cases (109 per 100,000 population). Four countries (Cambodia, China, the Philippines and Vietnam) account for 93% of the total estimated incident cases in the Region. Every year an estimated 300 thousand persons die due to TB. The Region is host to an estimated 135,000 multi-drug resistant TB cases, most of which can be found in Chin

Don't forget the bigger picture: the impact of societal issues on a community-based TB programme, Swaziland.

PURPOSE: The purpose of this paper is to explore the wider societal issues that can impact on the success of a TB programme. DESIGN/METHODOLOGY/APPROACH: The paper is a qualitative study of the experiences of people involved in a community-based DOTS programme in Lubombo, Swaziland, involving patients, DOT treatment supporters, clinic nurses and other key informants. FINDINGS: The paper finds that study participants spontaneously raised two main societal issues, which had major impact on the success of the TB programme: health beliefs and poverty. It is seen that health beliefs can have a ma

Resistance and renewal: health sector reform and Cambodia's national tuberculosis programme.

Following the destruction of Cambodia's health infrastructure during the Khmer Rouge period (1975-1979) and the subsequent decade of United Nations sanctions, international development assistance has focused on reconstructing the country's health system. The recognition of Cambodia's heavy burden of tuberculosis (TB) and the lapse of TB control strategies during the transition to democracy prompted the national tuberculosis programme's relaunch in the mid-1990s as WHO-backed health sector reforms were introduced. This paper examines the conflicts that arose between health reforms and TB contro

Active community surveillance of the impact of different tuberculosis control measures, Tiruvallur, South India, 1968-2001.

BACKGROUND: Tuberculosis is curable, but community surveys documenting epidemiological impact of the WHO-recommended DOTS strategy on tuberculosis prevalence have not been published. We used active community surveillance to compare the impact of DOTS with earlier programmes. METHODS: We conducted tuberculosis disease surveys using random cluster sampling of a rural population in South India approximately every 2.5 years from 1968 to 1986, using radiography as a screening tool for sputum examination. In 1999, DOTS was implemented in the area. Prevalence surveys using radiography and symptom sc

In closely monitored patients, adherence in the first month predicts completion of therapy for latent tuberculosis infection.

BACKGROUND: Current therapy for latent TB infection (LTBI) is long, and requires close follow-up. This results in sub-optimal adherence-the major reason for failure of therapy. METHODS: In an open label randomised trial comparing 4 months of rifampicin with 9 months of isoniazid, the proportion and regularity of doses taken, measured with an electronic monitoring system (MEMS), and provider estimates of adherence in the first month of therapy, were assessed as predictors of treatment completion. RESULTS: Of 104 patients analysed, 86 took more than 80% of doses within the expected interval, 1

Scaling up tuberculosis case finding via private providers in Ghana: an impact evaluation using interrupted time series.

BACKGROUND: Although TB services are free in Ghana, TB case detection remains low and mostly limited to public facilities. To address this, a Public-Private Mix (PPM) Directly Observed Therapy (DOT) model was introduced, involving community private healthcare providers and the National Health Insurance Scheme (NHIS) to boost TB case detection rates. METHODS: This impact evaluation focuses on four key interventions targeting vulnerable populations in Ghana's two largest metropolitan areas between the last quarter of 2018 and the first quarter of 2020. Screening and TB register data were collec

Reminder systems to improve patient adherence to tuberculosis clinic appointments for diagnosis and treatment.

BACKGROUND: People with active tuberculosis (TB) require six months of treatment. Some people find it difficult to complete treatment, and there are several approaches to help ensure completion. One such system relies on reminders, where the health system prompts patients to attend for appointments on time, or re-engages people who have missed or defaulted on a scheduled appointment. OBJECTIVES: To assess the effects of reminder systems on improving attendance at TB diagnosis, prophylaxis, and treatment clinic appointments, and their effects on TB treatment outcomes. Some current concepts on childhood tuberculosis.

As children acquire infection with Mycobacterium tuberculosis from adults in their environment, the epidemiology of childhood tuberculosis (TB) follows that in adults. While global burden of childhood tuberculosis is unclear, in developing countries the annual risk of tuberculosis infection in children is 2- 5 per cent. Nearly 8-20 per cent of the deaths caused by tuberculosis occur in children. It has been suggested that BCG vaccination is responsible for decrease in the occurrence of disseminated and severe disease. Localized forms of illness, e.g., intrathoracic lymphadenopathy, and localiz

Acceptability of and Implementation Supports for Video Directly Observed Treatment to Enhance Methadone Dosing Flexibility in a Multisite Opioid Treatment Program: Qualitative Rapid Needs Assessment Study.

BACKGROUND: Methadone is a first-line treatment for opioid use disorder, which is delivered in federally regulated opioid treatment programs (OTPs). Federal policies require directly observed dosing of methadone followed by graduated provision of nonobserved doses to take at home (ie, "take-home" dosing) after demonstrated stability is achieved. Policy changes since the COVID-19 pandemic have greatly expanded take-home dosing. Video directly observed treatment (video DOT) is an approach in which patients submit videos of themselves taking medications, which are asynchronously reviewed to verif

Patient-centered and technology-enabled strategies to improve tuberculosis treatment adherence.

INTRODUCTION: Tuberculosis (TB) treatment adherence (how regular patients follow the prescribed medication) has a major role in TB control. In high‑burden countries such as India, poor adherence contributes to treatment failure, relapse, and the emergence of drug-resistant TB (DR-TB), despite substantial programmatic efforts to improve treatment outcomes. AREAS COVERED: This review examines a broad range of TB treatment adherence strategies, including digital adherence technologies, behavioral and psychosocial interventions, directly observed therapy models, medication dispensing and monitori

Enhancing Primaquine Adherence for Plasmodium vivax Malaria: A Cluster-Randomized Controlled Trial in Myanmar.

The rising prevalence of Plasmodium vivax (P. vivax) malaria challenges elimination efforts, particularly in the Greater Mekong Subregion, although adherence to the required 14-day primaquine (PQ) regimen remains a major obstacle. This study evaluated the effectiveness of a family-administered, directly observed treatment intervention in improving adherence to PQ among patients with confirmed P. vivax from October 2022 to March 2023. A cluster-randomized controlled trial was conducted in five intervention villages and five control villages in Kachin State, Myanmar. A total of 427 patients in t

Benefits of a self-myofascial release program on health-related quality of life in people with fibromyalgia: a randomized controlled trial.

BACKGROUND: Fibromyalgia (FM) is a disease with symptoms that significantly limit the life of affected patients. Earlier studies have shown that the application of self-myofascial release provides benefits in variables such as fatigue, range of motion (ROM) or perceived muscle pain in a healthy population. Despite this, the self-myofascial release technique has not yet been used in people with FM. This study aimed to find out the benefits of applying a self-myofascial release program on health-related quality of life in people with FM. METHODS: Sixty-six participants with FM were randomized i

Tuberculosis a re-emerging disease.

Two billion people, nearly one-third of the population worldwide, are infected by M. tuberculosis. Since 2008, the number of foreign TB cases in Italy has overtaken native cases. Even though TB prevalently affects elderly Italian natives, the diagnosis of the disease in these subjects is difficult and frequently delayed because of the low index of suspicion due to loss of clinical experience. Moreover, the large use of TNF inhibitors has created a new risk group for the disease, highlighting the need for tougher screening practices to reduce new cases of severe TB. Immigration from high-incide
