Utilizing of Removable Rigid Dressing to Decrease Below the Knee Amputations to Above the Knee Amputations Conversion Rates.

INTRODUCTION/OBJECTIVE: Below the knee amputations (BKAs) are preferred to above the knee amputations (AKAs) due to better rehabilitation and functional outcomes. Assessment of literature for best practice identified that utilization of a removable rigid dressing (RRD) improves post-operative BKA care by expediting wound healing and reduces the hospital length of stay compared to a soft dressing. We hypothesized that there would be a decrease of conversions from BKA to AKA following utilizing of RRD device. METHODS: Retrospective chart review of all BKA performed by the vascular surgery servi

Factors Related to Neuropathic Pain following Lower Extremity Amputation.

BACKGROUND: Lower extremity amputations are common, and postoperative neuropathic pain (phantom limb pain or symptomatic neuroma) is frequently reported. The use of active treatment of the nerve end has been shown to reduce pain but requires additional resources and should therefore be performed primarily in high-risk patients. The aim of this study was to identify the factors associated with the development of neuropathic pain following above-the-knee amputation, knee disarticulation, or below-the-knee amputation. METHODS: Retrospectively, 1565 patients with an average follow-up of 4.3 years

Fracture-associated osteosarcoma of the femur in a cat.

This report describes a domestic shorthair cat with fracture-associated osteosarcoma 11 years after injury. A left hind limb amputation was performed using coxofemoral disarticulation. No intravenous cytotoxic chemotherapy was used after surgery. Pulmonary metastasis was identified 3 months after amputation. Fracture-associated osteosarcoma may occur in the cat more than 10 years after initial injury.

Targeted Muscle Reinnervation in the Upper Extremity Amputee: A Technical Roadmap.

Targeted muscle reinnervation (TMR) offers the potential for improved prosthetic function by reclaiming the neural control information that is lost as a result of upper extremity amputation. In addition to the prosthetic control benefits, TMR is a potential treatment for postamputation neuroma pain. Here, we present our surgical technique for TMR nerve transfers in transhumeral and shoulder disarticulation patients.

Delayed antemortem diagnosis of adenocarcinoma of the cecum presenting as lower extremity gas gangrene.

The rare entity of nontraumatic metastatic gas gangrene is commonly associated with occult gastrointestinal malignancies. Normally the patient's course is fulminant, with rapid demise prior to ascertaining the correct etiology at autopsy. An unusual case is presented in that 65 days following hip disarticulation for gas gangrene the diagnosis of colonic cancer was appropriately suspected and then proven.

New bone formation at amputation sites in electrically burn-injured patients.

Sixty-one amputation sites in 43 electrically injured men were examined for significant new bone formation. Twenty-eight amputations were classified as long bone (6 above knee, 4 below knee, 4 above elbow, 14 below elbow); 28 small bone (9 hand or finger and 19 foot or toe); and 5 disarticulations. Significant new bone formation occurred at the amputation site in 23 (82%) of the 28 long bone cases, but none occurred in the small bone or disarticulation cases. This difference was significant at the 0.0001 level (chi square = 40.53, 2 df). Fourteen (78%) of upper and nine (90%) of lower extremit

Susceptibility Factors for Early Reamputation in Diabetic Great Toe Gangrene.

INTRODUCTION: The effectiveness of the treatment has long been significant in diabetes and its complications, especially in developing countries. Prolonged hospitalization and repeated surgery should be avoided due to clinical and economic reasons. Wound breakdowns or necrosis can occur after amputation procedures, and subsequently will require reamputation. This study analyzed susceptibility factors in diabetic foot patients undergoing prior high toe amputation that are thought to be related to early reamputation. METHODS: We performed a retrospective analysis in 107 patients that have under

Regaining Gait Using an Early Postoperative Hip Prosthesis: A Case Report of an Elderly Woman.

BACKGROUND: Hip prostheses are generally avoided in elderly patients because of cognitive decline and/or reduced muscle strength. The present report describes the case of an elderly woman who regained gait using a prosthesis prescribed during the early postoperative phase following hip disarticulation. CASE: A 78-year-old woman suffered from a pathological fracture caused by liposarcoma of the left thigh. Before hospitalization, the patient was fully independent in her activities of daily living, including gait. The right and left handgrip strengths were 12.9 and 14.2 kg, respectively, and th

An indication for continuous cervical paravertebral block (posterior approach to the interscalene space).

We present a patient who required perioperative analgesia with continuous nerve block for shoulder disarticulation, for whom the only approach possible to the brachial plexus was from posterior. A 51-yr-old woman was suffering from intractable upper extremity pain and dysfunction as a result of severe lymphedema after metastatic spread of breast cancer to the axilla. Her pain was poorly controlled despite aggressive treatment with oral, systemic, and intrathecal opiates. She presented for amputation of her arm as a last resort for management of pain. In order to provide optimal postoperative a

Necrotizing fasciitis as complication of injection into greater trochanteric bursa.

Necrotizing fasciitis is a limb- and life-threatening soft-tissue infection that frequently involves the extremities. This article describes the first case of necrotizing fasciitis developing from a single steroid injection of the greater trochanteric bursa. Despite early resuscitation and aggressive surgical management that included a hip disarticulation, the patient expired. Potential contributing factors are reviewed.

Femoral bifurcation associated with tibial aplasia: about 3 cases.

Femoral bifurcation associated with tibial aplasia of the limb is a very rare pathology. Its radical treatment is a disarticulation of the knee, followed by fitting of a prosthesis. In Benin the reluctance of parents to allow their children to undergo amputation and the access to equipment for the amputated child are a major technical and financial hindrance. Here we report three cases.

Effective Resolution of Postoperative Stoma Edema With Repeated Application of Topical 50% Glucose: A Case Report.

Although glucose-induced osmotic gradients have been used to reduce intestinal edema, no standardized technique has been established. Repeated applications of this method have not been documented, and concerns remain regarding its potential effects on glycemic control. We present a case of a patient in his 20s admitted for treatment of an advanced pressure ulcer over the left greater trochanter. His medical history included spina bifida and type 2 diabetes mellitus. The ulcer was complicated by osteonecrosis of the left femur, and a left hip disarticulation was planned as a life-saving procedu

Amputation of the lower limb as palliative treatment for debilitating musculoskeletal cancer.

Limb sparing surgery has replaced amputation surgery for treating sarcomas of the lower limb in most cases. Wide resection followed by postoperative radiation therapy can achieve acceptable local control and survival rates in patients with bone and soft-tissue sarcomas of the lower limb. Recurrent or persistent disease constitutes a major oncological problem. Local symptoms such as agonizing pain, fractures, tumor fungation, inability to walk and inability to maintain daily activities, further impair the patient's quality of life. In this clinical set-up palliative amputation of the limb shoul

A treatment algorithm for neuropathic (Charcot) midfoot deformity.

Forty-nine feet in 47 patients with midfoot neuropathic foot deformity were referred for care to a comprehensive foot salvage clinic and followed for an average of 3.6 years. Twenty-three initially presented without open ulcers. Two underwent elective Syme's ankle disarticulation amputation, and the others were kept ambulatory with a combination of periods of nonweightbearing cast immobilization, accommodative extra-depth shoes with custom orthotics, and ankle-foot orthoses. Twenty-six of the feet initially presented for care with open ulcers and/or chronic osteomyelitis. Twenty-two of these p

Allowing Standing and Assisted Steps in a Patient With Renshaw Type IV Sacral Agenesis: A Case Report of a Novel Surgical Approach.

CASE: A 6-year-old boy with a Renshaw type 4 sacral agenesis presented paraplegia and rigid, "Buddha-like" lower-limb contractures, including severe knee pterygia, which made crawling and sitting difficult. Staged surgical treatment involved bilateral knee disarticulation, soft tissue surgery, and bifocal femoral osteotomies for lower-limb reorientation. At 18 months postoperatively and after prosthetic fitting, the patient can stand and take steps with assistance. CONCLUSION: This effective surgical strategy achieves standing in a troublesome orthopaedic congenital condition. The interventio

The importance of tissue damage secondary to high voltage electrical injury to the limbs often makes the management of this kind of burn very difficult. Repair interventions are sometimes ineffective and amputations are then unavoidable. We report the case of a young patient suffering electrical injury to both upper limbs caused by a high voltage current, whose evolution was dramatically marked by bilateral disarticulation of both shoulders.

Compartment syndrome as a late presentation of brachial artery pseudoaneurysm following shaft of humerus fracture.

Peripheral artery pseudoaneurysm is rare in upper extremities compared with lower extremities. Early diagnosis and prompt management are two cornerstones of successful outcomes in these cases. Delay can lead to limb and life-threatening complications. We present a case of compartment syndrome of upper limb as a sequel to pseudoaneurysm of brachial artery for which we had to do shoulder disarticulation. The patient recovered uneventfully.

Femoral bifurcation with ipsilateral tibia hemimelia: early outcome of ablation and prosthetic fitting.

Femoral bifurcation and tibia hemimelia are rare anomalies. Hereby, we present a case report of a 2-year-old boy who first presented in our orthopedic clinic as a 12-day-old neonate, with a grossly deformed right lower limb from a combination of complete tibia hemimelia and ipsilateral femoral bifurcation. Excision of femoral exostosis, knee disarticulation and prosthetic fitting gives satisfactory early outcome.
