Computed Tomography-Based Morphometric Analysis of the Ascending Aorta in Acute Type A Dissection Beyond Diameter-Based Thresholds: A National Cohort Study from Latvia.

Background/Objectives: Ascending aortic aneurysm is a heterogeneous disease, with many cases of acute Stanford type A aortic dissection (ATAAD) presenting with aortic diameters below currently recommended surgical thresholds. Demographic factors such as age and sex, along with indexed aortic size groups, have been proposed to improve risk stratification. Methods: We included 65 adult patients who underwent surgical intervention for ATAAD. Morphometric measurements were obtained from computed tomography angiography (CTA) using centerline reconstruction. Maximum ascending aortic diameter and len

Localized dissection of ascending aorta and arch rupture due to fire suppressant blast: A rare mechanism of injury successfully managed by urgent surgical intervention.

BACKGROUND: Traumatic aortic injuries are predominantly caused by high-impact motor vehicle collisions, often due to sudden deceleration. This report presents a rare case of localized dissection and partial aortic arch rupture resulting from a fire suppressant explosion. CASE PRESENTATION: A 36-year-old male was transferred following a fire suppressant explosion at a copper smelter. He presented with central chest pain and stable hemodynamics, with no signs of external chest trauma. Computed tomography (CT) imaging revealed a localized dissection of the ascending aorta and a partial rupture o

Successful percutaneous coronary intervention in a case of acute aortic dissection complicated with malperfusion of the left main coronary artery after replacement of the ascending aorta.

A 75-year-old female was admitted to our hospital with sudden back pain and right leg ischemia. Computed tomography showed acute type A aortic dissection with the occlusion of the right common iliac artery. The patient was treated with ascending aorta replacement and femoro-femoral bypass. Three hours after the operation, the patient went into a sudden shock. Electrocardiogram showed ventricular tachycardia and ventricular fibrillation. Percutaneous cardio-pulmonary support was administered and coronary arteriogram (CAG) was proceeded for evaluation of the coronary arteries. Although CAG revea

An Unusual Case of Ascending Aorta Dissection 4 Years after Perceval Sutureless Bioprosthesis Implantation: Easier Access to Diagnosis through the Right Parasternal Longitudinal Echocardiographic Window.

Sutureless bioprostheses are more frequently used for the surgical treatment of elderly patients with high burden of comorbidity, who are candidates to aortic valve replacement (AVR). Among the sutureless bioprostheses, the Perceval valve has shown a good midterm durability, with very few reports of valve deterioration and low risk of complications. Herein, we present an unusual case of ascending aorta dissection which occurred 4 years after AVR with a Perceval sutureless pericardial bioprosthesis, likely related to the peculiar prosthetic design. A high right parasternal longitudinal view of

Delayed chronic type A dissection with a functional midline crossing right internal thoracic artery after CABG: surgical approach to an ascending aorta without dissecting the midline crossing internal thoracic artery.

We successfully treated chronic type A dissection and coronary artery disease with a functional midline crossing right internal thoracic artery (RITA) after coronary artery bypass grafting (CABG). A 68-year-old man was incidentally diagnosed as chronic type A dissection by follow-up cardiac catheterization after CABG, with 90% stenosis in the right coronary artery (RCA) No. 2. The dissecting aneurysm (max. 6 cm in diameter) was localized at the right portion of the ascending aorta with a functional RITA to the left anterior descending coronary artery. The dissecting aneurysm was treated with p

[Superior Mesenteric Artery Bypass Grafting from the Ascending Aorta Using a Saphenous Vein Graft for Treatment of Acute Aortic Dissection with Abdominal Malperfusion:Report of a Case].

A 50-year-old man presented with abdominal pain and numbness in the right leg. He was diagnosed with type A acute aortic dissection with malperfusion of the superior mesenteric artery(SMA) and the right external iliac artery. Before median sternotomy, we made median celiotomy and anastomosed a saphenous vein graft to SMA. After cardio-pulmonary bypass was instituted, perfusion to the saphenous vein graft was started. After the central repair, we selected the vascular graft of the ascending aorta as the inflow of SMA bypass because leading the vein graft to the external iliac artery was difficu

Growth rate of the ascending aorta and risk of aortic dissection or reintervention in patients with bicuspid or tricuspid aortic valves undergoing valve interventions: A systematic review and meta-analysis.

BACKGROUND: Bicuspid aortic valve (BAV) is associated with premature valve dysfunction and progressive aortic dilation, potentially leading to dissection. The evolution of aortic dimensions after valve replacement remains uncertain, particularly compared with tricuspid aortic valve (TAV) disease. This systematic review and meta-analysis aimed to compare postoperative aortic growth and the risk of major aortic events between BAV and TAV patients. METHODS: A systematic search of MEDLINE, CENTRAL, and Web of Science (from inception to April 2025) identified prospective and retrospective cohort s

[Celiac Artery Bypass Grafting from the Ascending Aorta Following Aortic Repair for Treatment of Stanford Type A Acute Aortic Dissection Complicated by Abdominal Malperfusion:Report of a Case].

A 73-year-old woman presented with sudden chest and back pain. Computed tomography (CT) revealed Stanford type A acute aortic dissection complicated by occlusion of the celiac artery and stenosis of the superior mesenteric artery. Because there was no clear sign of critical abdominal organ ischemia before surgery, central repair was performed first. Then, after cardiopulmonary bypass, laparotomy was performed to check the blood flow in the abdominal organs. Malperfusion of the celiac artery remained. We therefore made an ascending aorta-common hepatic artery bypass using a great saphenous vein

Mechanical characterization of dissected and dilated human ascending aorta using Fung-type hyperelastic models with pre-identified initial tangent moduli for low-stress distensibility.

Ascending aortic dissection (AD) is a potentially fatal vascular disease associated with degradation and fragmentation of the elastic fibers in the aortic media, increasing low-stress distensibility, and a dilated aorta may lead to dissection. In this study, a Fung-type hyperelastic model was formulated incorporating the initial tangent moduli (ITM) of stress-strain curves as an index of low-stress distensibility. ITM were correlated with the material constants by linearizing incompressible stress-strain relationships at zero strain. For uniaxial loading tests, the robustness of the material c

A Case of Paraplegia Treated with Cerebrospinal Fluid Drainage and Permissive Hypertension after Graft Replacement of the Ascending Aorta and the Total Aortic Arch for Acute Aortic Dissection Stanford Type A.

Paraplegia after an operation for acute aortic dissection Stanford type A (AADA) is fairly uncommon, and there is no consensus about optimal treatment. We present a case in which cerebrospinal fluid drainage (CSFD) and permissive hypertension were used for treatment of paraplegia. When the patient showed complete bilateral paraplegia after operation for AADA, we immediately began CSFD and maintained mean arterial blood pressure at over 90 mmHg. His neurological deficit gradually recovered, and he was eventually able to walk without support. The combination of CSFD and permissive hypertension c

[Subtotal aortic replacement in a patient with Marfan's syndrome--replacement of the ascending aorta and entire aortic arch for acute type I aortic dissection 4 years after the thoracoabdominal replacement].

We report a patient with Marfan's syndrome who underwent replacement of the ascending aorta and the entire aortic arch for acute type I aortic dissection 4 years after the thoracoabdominal replacement. As a result, she underwent subtotal aortic replacement from the ascending aorta to the bilateral common iliac arteries in three stages. A 48-years-old female was admitted to our service because of acute heart failure and cardiogenic shock caused by acute DeBakey type I aortic dissection. She had undergone replacement of the descending thoracic aorta at the age of 44 and replacement of the remain

[Current status and reflections on David procedure in the management of acute type A aortic dissection involving the root].

Acute type A aortic dissection (ATAAD) involves complex pathology in aortic root, and appropriate surgical strategies are crucial to clinical outcomes. Recently, with significant improvements in the safety and short-term efficacy of ATAAD surgical strategies, maximizing the elimination of aortic root dissection and preserving well-functioning aortic valves during emergency surgery is directly related to the long-term survival quality of patients. David procedure is an effective aortic valve repair technique which has shown satisfactory long-term outcome in patients with aortic root aneurysm an

A case of intraoperative acute aortic dissection caused by cannulation into an axillary artery.

Severe atherosclerotic disease of the ascending aorta is one of the risk factors of dissection of the ascending aorta and cerebral embolism during cardiac operations with cardiopulmonary bypass. Aortic dissection is rare, but once it happens, the mortality rate is high. For the patient with severely atherosclerotic or strongly calcified aorta, we should avoid cannulation into the aorta or clamping of it. In this case, we experienced aortic dissection although we chose the arterial cannulations into the axillary arteries because of the strong calcification of the ascending aorta and the abdomin

Retrograde ascending aortic dissection: a diagnostic and therapeutic challenge.

Aortic dissection with an entrance tear in the transverse aorta is generally considered to have the highest acute fatality rate of any type of dissection and the direction of its extension is the most difficult to predict. In a prospective study, we evaluated 61 consecutive patients (mean age 56.7 years, ranging from 21 to 75 years), presenting with ascending aortic dissection during a 36-month-period and tried to clarify the incidence of retrograde ascending aortic dissection. In 49 patients (80.3%), the intimal tear was located in the ascending aorta, whereas the dissection originated in the
